Occupational Therapy Practice Patterns for Military Service Members With Upper Limb Amputation.

INTRODUCTION: Military Service Members (SMs) with upper limb (UL) amputation have unrestricted access to occupational therapy (OT) services. Identifying OT interventions used based on clinical rationale and patient needs can provide insight toward developing best practice guidelines. The purpose of this retrospective observational study was to identify preferred OT practice patterns for U.S. Military SMs treated in Military Treatment Facilities, who have sustained various levels of deployment-related UL amputation. METHODS: The study sample was ascertained from the Expeditionary Medical Encou

Sexual Health Problems among Service Men: The Influence of Posttraumatic Stress Disorder.

Military operational stressors, such as combat exposure, may increase the risk of sexual health problems. This study examined factors associated with sexual health problems, and tested the mediating effect of probable posttraumatic stress disorder (PTSD) on the association between stressors (i.e., combat deployment and sexual assault) and sexual health problems among U.S. service men. Using multivariable logistic regression (n = 16,603) and Cox proportional hazards models (n = 15,330), we estimated the risk of self-reported sexual health difficulties and sexual dysfunction medical encounters,

A prospective field study of U.S. Army trainees to identify the physiological bases and key factors influencing musculoskeletal injuries: a study protocol.

BACKGROUND: Musculoskeletal injuries (MSKIs) are common in military trainees and present a considerable threat to occupational fitness, deployability, and overall military readiness. Despite the negative effects of MSKIs on military readiness, comprehensive evaluations of the key known and possible risk factors for MSKIs are lacking. The U.S. Army Research Institute of Environmental Medicine (ARIEM) is initiating a large-scale research effort, the ARIEM Reduction in Musculoskeletal Injury (ARMI) Study, to better understand the interrelationships among a wide range of potential MSKI risk factor

Pulmonary vascular disease in Veterans with post-deployment respiratory syndrome.

Exertional dyspnea has been documented in US military personnel after deployment to Iraq and Afghanistan. We studied whether continued exertional dyspnea in this patient population is associated with pulmonary vascular disease (PVD). We performed detailed histomorphometry of pulmonary vasculature in 52 Veterans with biopsy-proven post-deployment respiratory syndrome (PDRS) and then recruited five of these same Veterans with continued exertional dyspnea to undergo a follow-up clinical evaluation, including symptom questionnaire, pulmonary function testing, surface echocardiography, and right he

Blast-related ear injuries among U.S. military personnel.

Blast-related ear injuries are a concern during deployment because they can compromise a servicemember's situational awareness and adversely affect operational readiness. The objectives of this study were to describe blast-related ear injuries during Operation Iraqi Freedom, identify the effect of hearing protection worn at the point of injury, and explore hearing loss and tinnitus outcomes within one year after injury. The Expeditionary Medical Encounter Database was used to identify military personnel who survived blast-related injury, and it was linked with outpatient medical databases to o

Role of occupation on new-onset post-traumatic stress disorder and depression among deployed military personnel.

The purpose of this study was to examine the role of military occupation on new-onset post-traumatic stress disorder (PTSD) and depression among U.S. combat veterans recently returned from deployment to Iraq. Enlisted, active duty Navy and Marine Corps personnel without a history of mental disorder were identified from deployment records and linked to medical databases (n = 40,600). Multivariate logistic regression was used to examine the association between occupation and postdeployment PTSD and depression diagnoses by branch of service. Navy health care specialists had higher odds of new-ons

Methodology for characterizing emissions from small (0.5-2 MTD) batch-fed gasification systems using multiple waste compositions.

A compact, containerized gasification system was characterized for air emissions while burning four waste types. A methodology is presented for developing a standardized test waste composition and demonstrated using three military and one civilian waste types. Batch charges of waste were processed through a gasification chamber, afterburner, and wet scrubber. The 0.5-2 metric ton per day (MTD) system was designed for mobile deployment by the military in forward operations but would be applicable to small scale civilian applications. Emissions data from these types of small capacity, cyclically

Anthrax vaccination and risk of optic neuritis in the United States military, 1998-2003.

BACKGROUND: Numerous case reports have suggested a possible association between optic neuritis and receipt of several different vaccines. The most frequently identified vaccines associated with optic neuritis in the literature are influenza and hepatitis B, and a report describing 2 US military cases suggests an association with the currently used anthrax vaccine (anthrax vaccine adsorbed). OBJECTIVE: To test the hypothesis that optic neuritis may be associated with anthrax, smallpox, hepatitis B, and influenza vaccines. DESIGN: We conducted a matched case-control study among US military per

Accumulation of Risk and Promotive Factors Among Young Children in US Military Families.

In the families of the new cohort of war veterans now entering the civilian population in the United States are over two million young children (Cozza, Haskins & Lerner, 2013; Institute of Medicine, 2013). Several noteworthy studies have shown that children exposed to separation from a parent due to combat-related deployment are at elevated risk for a variety of negative consequences (Lester & Flake, 2013). Cozza et al. (2013) argue that existing studies of military children focus too much on the stresses or deficits they experience, failing to give sufficient attention to their strengths, the

Disaster Report: Military Helicopter Crash in a Deployed Environment - Providers' and Patient's Perspectives.

The crash of a military helicopter during a 2023 United States Army deployment produced a mass-casualty incident (MASCAL) in an austere setting. One of the casualties was an Emergency Medicine (EM) physician who oversaw point-of-injury (POI) triage and early casualty evacuation with a Special Operations Forces (SOF) medic to the nearest Role II Military Treatment Facility (MTF), followed by movement of the six most severely injured patients to Role III, and subsequent transfer to Landstuhl Regional Medical Center (LRMC) via Critical Care Air Transport Team (CCATT). Reduction in reintegration stress among post-9/11 Veterans in a clinical trial for trauma-related guilt.

Reintegration stress is commonly reported by returning Veterans with post-trauma distress and associated with mental health and functioning difficulties. Interventions are needed to reduce reintegration stress and provide a pathway to improve Veterans' connections with their families, friends, and communities. The present study compared the effectiveness of Trauma Informed Guilt Reduction Therapy (TrIGR) and Supportive Care Therapy (SCT) in reducing reintegration stress, assessed by the Military to Civilian Questionnaire (M2C-Q) at post-treatment and 3- and 6-month follow-up. Data were derived

Associations of Warzone Veteran and Intimate Partner PTSD Symptoms with Child Depression, Anxiety, Hyperactivity, and Conduct Problems.

Warzone deployment increases risk for posttraumatic stress disorder symptoms (PTSS), including among service members who have children. Parental PTSS are associated with child depression, anxiety, hyperactivity, and conduct problems, yet few studies of child behavioral health outcomes in military populations have accounted for PTSS in both warzone veterans and their partners. Fewer still incorporate non-clinically-recruited samples of nationally dispersed warzone veterans and their families. The current research examines whether children whose parent(s) have higher levels of PTSS exhibit more

Stressful life events and trajectories of depression symptoms in a U.S. military cohort.

Depression is a common mental disorder that may comprise distinct, underlying symptom patterns over time. Associations between stressful life events throughout the civilian lifecourse-including during childhood-and adult depression have been documented in many populations, but are less commonly assessed in military samples. We identified different trajectories of depression symptoms across four years in a military cohort using latent class growth analysis, and investigated the relationship between these trajectories and two domains of civilian life experiences: childhood adversity (e.g., being
