Posaconazole for the management of mucosal candidiasis.

Mucocutaneous candidiasis (MC) is one of the first signs of HIV infection. In the pre-highly active antiretroviral therapy (HAART) era, more than 90% of patients with HIV infection eventually developed some form of oral candidiasis during their illness, and an additional 10% developed esophageal candidiasis (EC). Although several antifungal agents are available, systemic azoles (e.g., fluconazole and itraconazole) have replaced older topical antifungals (e.g., gentian violet and nystatin) in the management of MC in these patients. Overall, the azoles are safe and effective agents in HIV-infect

Dermoscopic features of mucosal melanosis.

BACKGROUND: Melanosis (lentiginosis, labial melanotic macula) is a benign pigmented lesion of mucosa characterized by pigmentation of basal keratinocytes with melanocytic normal or slightly increased in number. Melanosis, particularly when occurring on genitalia, can clinically mimic mucosal melanoma thus creating concern in both the patient and the physician. OBJECTIVE: In this study dermoscopic features from a series of clinically equivocal (n=11) or clinically typical (n=10) mucosal melanosis were analyzed. METHODS: All the women consecutively seen at the Vulva Clinic of the Department of

Azithromycin in the treatment of mucosal leishmaniasis.

This report describes three elderly patients with mucosal form of American tegumentary leishmaniasis associated with chronic cardiopathy. Due to the known toxicity of classical drugs with activity against Leishmania sp., the patients received three oral courses of azithromycin therapy in single 500 mg daily dose during ten days, every other month. All lesions healed after the third series. One of the patients relapsed and a new series of azithromycin was prescribed. Azithromycin may be an alternative drug for the treatment of leishmaniasis in special situations due to its optimal mucosal and i

Neuropeptides and gastric mucosal homeostasis.

The role of central nervous system (CNS) in regulation of gastric function has long been known. The dorsal vagal complex (DVC) has an important role in regulation of gastric mucosal integrity; it is involved both in mucosal protection and in ulcer formation. Neuropeptides have been identified in DVC, the origin of these peptides are both intrinsic and extrinsic. Neuropeptides are localized also in the periphery, in afferent neurons. The afferent neurons also have efferent-like function in the gastroinetestinal tract, and neuropeptides released from the peripheral nerve endings of primary affer

Perspectives in endoscopic mucosal resection.

The birth of endoscopic mucosal resection probably can be traced back to the early days of colonoscopy and polypectomy. With the advent of colonoscopy, endoscopists began encountering pedunculated and sessile mass lesions arising from the mucosal surface. The concept of snare polypectomy developed quickly, and this technique has become the standard of care in the management of colon polyps. Large sessile lesions, however, remain problematic, especially when they arise from the thin wall of the cecum and ascending colon. Serosal burns and frank perforations represent the most significant compli

The immunology of mucosal models of inflammation.

In recent years the status of the inflammatory bowel diseases (IBDs) as canonical autoimmune diseases has risen steadily with the recognition that these diseases are, at their crux, abnormalities in mucosal responses to normally harmless antigens in the mucosal microflora and therefore responses to antigens that by their proximity and persistence are equivalent to self-antigens. This new paradigm is in no small measure traceable to the advent of multiple models of mucosal inflammation whose very existence is indicative of the fact that many types of immune imbalance can lead to loss of toleran

Leishmaniasis in Sudan. Mucosal leishmaniasis.

Sudanese mucosal leishmaniasis is a chronic infection of the upper respiratory tract and/or oral mucosa caused mainly by Leishmania donovani. The disease occurs in areas of the country endemic for visceral leishmaniasis, particularly among Masalit and other closely related tribes in western Sudan. The condition may develop during or after an attack of visceral leishmaniasis, but in most cases it is a primary mucosal disease. Unlike South American mucocutaneous leishmaniasis, mucosal leishmaniasis in Sudan is not preceded or accompanied by a cutaneous lesion. Pathologically, the lesions show a

Mechanisms of oesophageal mucosal defence.

The integrity of the oesophageal mucosa depends upon an equilibrium between aggressive factors, predominantly acid and pepsin, and protective mechanisms. Protective mechanisms operate within the oesophageal mucosa as pre-epithelial, epithelial and post-epithelial defences. Only the protective components of the oesophageal pre-epithelial defence can be tested in vivo in humans. It has been recently demonstrated that human oesophageal submucosal glands elaborate mucous secretion rich in bicarbonate and non-bicarbonate buffers, mucin, prostaglandin E(2), epidermal growth factor (EGF) and transfor

Mucosal melanoma of the head and neck.

Two hundred and fifty-nine patients with mucosal melanoma of the head and neck were reviewed. The data of these patients were obtained from the records of the Department of Head and Neck Oncology at the University of Liverpool and from the Merseyside and Cheshire Cancer Registry. Survival curves were constructed using the life table method and differences were investigated by the Log Rank Test. Prognostic factors were further analysed by Cox's proportional hazards model. Melanomas of the nasal cavities and sinuses accounted for 69%; 22% occurred in the oral cavity and 9% in the pharynx, larynx

Oral mucosal Langerhans' cells.

Langerhans' cells (LC) are dendritic, antigen-presenting cells present within the epithelium of skin and mucosa, including that of the oral cavity. This article reviews the literature on the phenotypic and functional features of oral mucosal Langerhans' cells, and speculates on other aspects by extrapolating from data on their epidermal counterparts.

Neonatal gastrointestinal mucosal immunity.

The gastrointestinal mucosal immune system is a complex system with multiple interacting cells, which express different markers and elaborate a variety of mediators in response to noxious stimuli and pathogens. At birth this system is quite immature, putting the newborn mammal at risk for a variety of infectious and noninfectious diseases. With age and antigenic stimulation, maturation occurs and intestinal host defense then functions quite effectively to protect the host from ingested noxious substances and microorganisms.

Buccal mucosal urethral replacement.

Graft substances, such as skin and bladder mucosa, have been previously used for urethral replacement when local epithelial tissue was not available. However, these substances have been associated with meatal prolapse, stricture and fistula formation. We have used buccal mucosa as a tissue for urethral substitution in these situations during the last 8 years. We review our clinical experience in 18 urethral reconstructions performed for urethral replacement in 4 cases of exstrophy/epispadias, 12 complex hypospadias repairs and 2 cases of complex bulbar urethral strictures. There have been 5 ca

Prospects for human mucosal vaccines.

The selective induction of antibodies in external secretions and mucosal T cell-mediated immunity are desirable for the prevention of various systemic as well as predominantly mucosa-restricted infections. An enormous surface area of mucosal membranes is protected primarily by antibodies that belong, in many species, to the IgA isotype. Such antibodies are produced locally by large numbers of IgA-containing plasma cells distributed in subepithelial spaces of mucosal membranes and in the stroma of secretory glands. In humans and in some animal species, plasma-derived IgA antibodies do not enter

Association of mucosal leishmaniasis with HLA.

In the search for genetic variability in individual susceptibility to mucocutaneous leishmaniasis, a disease caused mainly by Leishmania (Viannia) braziliensis, HLA typing was performed on 43 patients presenting mucosal lesions and 111 matched controls. Antigen specificities of the HLA-A, -B, -C, -DR, and -DQ loci were determined and their frequencies in patients and controls were compared. There was a significant decrease in the frequency of HLA-DR2 [1 out of 38 (2.6%) patients vs. 29 out of 102 (28.4%) controls, corrected p value 0.004, relative risk 0.07, preventive fraction of the total po

Mucosal T-cell function.

Lymphocytes in the intestinal lamina propria probably differ from lymphocyte populations in the circulation or in other tissue sites in a number of ways. First, lamina propria lymphocytes are phenotypically distinct in that few of these cells normally express the Leu-8 or CD45RA antigens. The presence of these molecules on CD4 T cells correlates with suppressor and suppressor-inducer function, and therefore the majority of CD4 T cells in the intestinal lamina propria have the phenotype associated with high helper activity. A substantial proportion of lamina propria lymphocytes also have eviden

Complications of bladder mucosal graft.

A bladder mucosal graft for urethral reconstruction was performed on 79 patients who had complicated conditions in which local penile or preputial skin was not available. The major and minor complication rates for this procedure were 15.2 and 43 per cent, respectively, the latter representing mucosal glanular protrusion obviated by a technical alteration. Given the difficulty of these cases these complication rates are deemed acceptable in such patients.

Oral mucosal manifestations of AIDS?

Oral lesions of opportunistic infections and neoplasms are associated with immunosuppression caused by the human immunodeficiency virus (HIV). These include oral candidiasis, viral lesions such as warts, herpes simplex, herpes zoster and hairy leukoplakia, as well as Kaposi's sarcoma and an aggressive form of periodontal disease. Many of these can occur as the first clinical signs of HIV infection. Thus, careful oral examination is an important part of the clinical evaluation.
