Central neuropathic pain is common in multiple sclerosis (MS), and its prevalence increases with physical disability. Sufficient evidence links dysesthetic pain, trigeminal neuralgia, Lhermitte's sign, and painful tonics spasms to plaque formation in the spinal cord and brain, whereas the association with headache and back pain remains unclear. Management varies according to the pain in question. For dysesthetic pain, drugs in use for neuropathic pain in general are recommended as first-line treatment, and emerging evidence suggests some benefit from cannabinoids and levetiracetam. Because of …

IMPORTANCE OF THE FIELD: The cause and cure for multiple sclerosis (MS) remain unknown. Immunomodulatory agents are only partially effective and many patients do not tolerate the side effects or fail them. Immunosuppressive agents act non-specifically and are associated with serious complications. An emerging group of biologic agents with great potential for treatment of immune-mediated disorders such as MS are monoclonal antibodies. A review of alemtuzumab in MS is presented.
AREAS COVERED IN THIS REVIEW: Mechanisms of action of alemtuzumab and the results of Phase II clinical trials in MS.

This study describes a case of a patient with OSMS who presented with somnolence, periodic fever, memory impairment, and amenorrhea. Serum prolactin levels were found to be higher than normal. MR imaging showed a bilateral involvement of the hypothalamus. The clinical presentation, laboratory examination, and MR imaging findings suggested a diagnosis of hypothalamus syndrome in this patient.

The etiology of multiple sclerosis is, at present, not definitely known, but genetic and environmental factors play a role in its causation. Environmental causes have a predominant impact. Epidemiologic research has contributed considerably to the identification of external risk factors in this multifactorial setting, but methodological constraints still play a major part. Viral and other microbial agents have drawn much attention, although none of them is a necessary condition for the disease. This is true also for the Epstein-Barr virus, for which most data, including prospective data, suppo…

Patients with multiple sclerosis commonly describe visual symptoms that result from several eye movement abnormalities that occur from disruption of critical pathways in the brainstem, cerebellum, and cerebral hemispheres. These abnormalities include internuclear ophthalmoplegia, ocular motor palsy, ocular misalignment, pathologic nystagmus, impaired saccades, saccadic intrusions, and impaired pursuit. Detailed knowledge of these problems and their neuroanatomic localization will aid the physician by guiding diagnosis and therapeutic decision making.

Multiple sclerosis is an inflammatory disease of the central nervous system and a common cause of disability among young people. MS is thought to be an autoimmune disease involving both inheritance and environmental factors. The disease is characterized by relapses and the symptoms and course are highly variable. The diagnosis is primarily clinical and supported by results of diagnostic studies. The importance of timely diagnosis has increased with the availability of effective treatment. The purpose of this article is to review symptoms, signs, diagnosis and treatment of multiple sclerosis.

Ophthalmic manifestations are a prominent feature of multiple sclerosis (MS). Optic neuritis accounts for 18% of initial MS symptoms and 40-70% of all patients with MS have at least one episode of optic neuritis during their disease course.

OBJECTIVE: To review our multicenter experience with cyclophosphamide in the treatment of children with multiple sclerosis (MS).
METHODS: Retrospective chart review of children with MS treated with cyclophosphamide. Demographic, clinical, treatment, and MRI parameters were collected.
RESULTS: We identified 17 children with MS treated with cyclophosphamide. All but one had worsening of Expanded Disability Status Scale scores or multiple relapses prior to treatment initiation. Children were treated with one of three regimens: 1) induction therapy alone; 2) induction therapy with pulse maintena…

Multiple sclerosis (MS) is associated with reduced bone mass and higher frequency of osteoporosis. Although high-dose short-term intravenous glucocorticoid regimens cause a decrease in bone formation, this effect is usually reversible and osteoporosis in MS patients may be independent of the short-term corticosteroid treatment. Clinical evidence suggests an important role of vitamin D as a modifiable risk factor in MS. Low circulating levels of vitamin D have been found in MS patients, especially during relapses, suggesting that vitamin D could be involved in the regulation of the clinical dis…

Epstein-Barr virus (EBV) is a human DNA herpesvirus infecting more than 90% of the world's population. EBV is the etiological agent of infectious mononucleosis (Pfeiffer's disease). Furthermore, diverse malignancies such as Burkitt and Hodgkin lymphoma have been associated with EBV. More recently, a possible role for EBV has been suggested in chronic inflammatory/autoimmune diseases like rheumatoid arthritis and systemic lupus erythematosus as well as in multiple sclerosis (MS). MS is currently regarded as a disease with multifactorial etiology, EBV being one possible factor in MS manifestatio…

In this article I review the last 10 years of progress in the imaging of the optic nerve with a particular focus on applications to multiple sclerosis (MS). Development of magnetic resonance imaging (MRI) of the optic nerve has lagged behind imaging of other parts of the CNS. These limitations are due to technical challenges related to the small size and mobility of the optic nerves and artefacts caused by surrounding cerebrospinal fluid, orbital fat, and air-bone interfaces. Nonetheless the last 10 years has seen significant progress with regard to detecting optic nerve atrophy following opti…

The authors report clinical features of ocular manifestations in patients with multiple sclerosis (MS), those that affect the visual sensory system and those that affect the ocular motor system. Disturbances of visual sensory function may precede, manifest coincidentally or follow the neurologic manifestations. Visual disturbances are common in MS and often a result of acute demyelinating optic neuropathy. Careful examination of MS patients, who have never suffered optic neuritis, may also reveal asymptomatic visual loss. Asymptomatic visual loss seems to be a universal feature of MS. Patients…

Magnetization transfer (MT) is a relatively new way of generating contrast in magnetic resonance (MR) images that is sensitive to the density of the macromolecules found throughout tissue structures such as membranes, myelin, and organelles. MT imaging (MTI) can provide a quantitative measure of macromolecular density, and therefore of tissue damage, and has been applied in the central nervous system in multiple sclerosis (MS) and other diseases. This article introduces the contrast mechanisms behind MTI and gives some practical guidance about implementing MTI and about quantitative analysis o…

METHODS: From January to December 2001, 89 patients with multiple sclerosis (MS) were treated in our hospital; 24 of them were diagnosed for the first time. All patients underwent a complete ophthalmologic examination including a three-mirror contact lens examination when photopsias were found or the disease was diagnosed primarily.
RESULTS: Two patients showed vitreoretinal tractions with signs of periphlebitis before clinical-neurological manifestation of multiple sclerosis. The third patient, in whom the disease had been known for years, showed distinct neovascularizations, vasculitis, and…

The most frequent first symptoms of multiple sclerosis are the following: sensory symptoms, optic neuritis, motor and cerebellar syndromes. Three female patients have been diagnosed with multiple sclerosis on the basis of Poser's criteria. The onset of the disease was manifested in these cases with non-typical and rare symptoms. Patient 1, a woman, aged 24. The first symptom of the disease was an isolated lesion of nerve VI on the right side. Patient 2, a women, aged 30. The first symptom of the disease was acute pain localised within the area of the lumbar-sacral spine. Patient 3, a woman, ag…
