Anomalous tibial nerve. A case report.

A rare anomaly of a nonterminating tibial nerve is reported. It was found to be associated with tarsal tunnel syndrome. The nerve failed to divide in the tarsal tunnel and in the talocalcaneal canal distally to the Knot of Henry. The branches of the medial and lateral plantar nerves as well as the medial calcaneal nerve, the nerve to the abductor digiti quinti, and motor branches to the abductor hallucis and to the joints, vessels, and skin took origin directly from the tibial nerve. The presence of this nerve anomaly and its relevance to surgery in the posterior ankle region is discussed. Sur

Nerve crush injuries--a model for axonotmesis.

Nerve crush is a commonly used experimental model in the rat; however, a standard method of inducing this injury has not been defined. This study examined six crush techniques that are frequently used and characterized the subsequent nerve injury. Five types of nerve crush using a No. 5 jeweler's forceps and a sixth using a 30-s single crush with a serrated hemostat were studied in the posterior tibial nerve of the Lewis rat. Regeneration was evaluated using serial walking track assessments at 1, 2, 4, 6, and 8 weeks postoperatively. Nerve conduction studies and histological examination were p

Single- versus double-level nerve root compression. An experimental study on the porcine cauda equina with analyses of nerve impulse conduction properties.

Double-level nerve root compression had a greater effect on nerve impulse conduction, recorded as tail muscle action potential amplitude, than single-level compression at both 10 and 50 mm Hg. If the distance between the two compression sites at 10 mm Hg compression was increased from one vertebral segment (10 mm) to two vertebral segments (30 mm), the reduction of muscle action potential amplitude was further enhanced. These observations may give new insight into the pathophysiology of cauda equina compression in spinal disorders such as stenosis.

Multifocal motor neuropathy.

We believe that our present understanding of these syndromes is insufficient to separate CIDP from MMN clearly and consider MMN to be a remarkably multifocal, predominantly motor variant of CIDP with the very unusual electrophysiologic feature of conduction block confined to motor axons. Further studies are clearly needed to clarify fully the relationship between CIDP and MMN and the relationship of MMN and other motor syndromes to various ganglioside antibodies.

AAEM minimonograph #38: neuropathies in connective tissue disease.

Neuropathies are common in patients with known or suspected connective tissue disease. A vasculitic mononeuropathy multiplex is often seen in patients initially presenting with polyarteritis nodosa or developing arteritis as a complication of rheumatoid arthritis. However, vasculitic neuropathy may become confluent and present as as distal symmetrical polyneuropathy or occur without systemic necrotizing vasculitis. Distal symmetrical polyneuropathies without associated vasculitis are also common in many connective tissue diseases. Compression neuropathies, especially carpal tunnel syndrome, oc

Continuous muscle activity and distal spinal muscular atrophy.

A young man presented with myokymias, cramp-like difficulty in muscle relaxation and peroneal atrophy. EMG studies revealed continuous muscle activity (CMA) manifested as grouped potentials and high frequency discharges. Sensory nerve conduction studies and sural nerve biopsy gave normal results, and he was thought to suffer from distal spinal muscular atrophy with CMA. This association suggests that the lower motor neuron may have an important role in the generation of the continuous muscle activity.

Penetrance of the hereditary motor and sensory neuropathy Ia mutation: assessment by nerve conduction studies.

The clinical expression of hereditary motor and sensory neuropathy type I (HMSN I) is age-dependent. Autosomal dominant HMSN I is heterogeneous at a molecular level with genes localized on chromosomes 1, 17, and possibly other chromosomes. In order to define accurately the penetrance of a single HMSN I gene mutation, we performed nerve conduction studies in HMSN I families whose genetic defect was linked to chromosome 17 (HMSN Ia). All HMSN Ia subjects tested had slow nerve conduction velocities with a mean median velocity 20 +/- 6 m/sec, which did not change with age. The range of conduction

Volume expansion of nonmyelinated nerve fibers during impulse conduction.

Nonmyelinated nerve fibers undergo rapid volume expansion while carrying an impulse. This volume expansion is incurred as a consequence of a lateral expansion of the excited portion of the fibers, where the superficial layer is transformed into a low-density structure.

Normocalcemic tetany abolished by calcium infusion.

Tetany is a disorder of increased neuronal excitability usually associated with hypocalcemia. We studied a patient with typical tetanic cramps that responded to intravenous infusion of calcium despite normal serum concentrations of total and ionized calcium. Three generations were affected by similar symptoms, suggesting that this is a dominantly inherited disorder of neuronal hyperirritability.

Hereditary motor and sensory neuropathy type I and motor neuron disease. An unusual association.

A 34-year-old man experienced progressive muscle weakness and wasting in the shoulder girdle later spreading distally to the upper limbs. Neurological examination revealed signs of pyramidal and bulbar involvement as well as widespread fasciculations. Pes cavus and distal hypoesthesia were also observed. MCV and nerve biopsy findings were consistent with HMSN-I, while EMG pattern suggested a MND. The association of HMSN-I with MND has not been previously described in literature.

Neuromuscular involvement in primary hyperparathyroidism.

Sixteen consecutive patients with primary hyperparathyroidism (HPT) were examined by a combination of electrophysiological, morphological and biochemical measurements. Six patients had, preoperatively, subjective impairment of neuromuscular function. Three of them were improved by the normalization of parathyroid function after surgery. In the other patients, who either had vague symptoms or appeared to be asymptomatic, the operation did not cause subjective changes in their neuromuscular function during a 3-month follow-up period. Altogether seven patients, three of whom had symptoms, had imp

Guillain-Barré syndrome: disease process.

Primarily a disease of the Peripheral Nervous System, GBS occurs most frequently following a viral infection. The exact cause is unknown. The diagnosis of GBS depends mainly on clinical, laboratory, and electrodiagnostic findings. Generally, the patient will present with a progressive motor weakness of more than one extremity and areflexia. There will also be a predictable progression including symmetry of motor involvement, mild sensory symptoms, and recovery. Cranial nerves as well as the ANS have also been involved in GBS. Recovery from the disease is generally good although it may take man

The pneumatic tourniquet: a biomechanical and electrophysiological study.

The pneumatic tourniquet is used routinely in extremity surgery, not only to prevent unnecessary blood loss, but also to increase the safety of the operative procedure. The mechanical part of this study shows that the system can fail in either of two major ways, each causing serious problems. The tourniquet may be inadequate if the pressure delivered to the cuff is not accurately displayed on the gauge or if the pre-set pressure is not maintained for the duration of the operative procedure. In our hospital, three of seven tourniquets (43%) in daily use showed significant errors in this regard.

Anomalous reinnervation as a sequel to obstetric brachial plexus palsy.

In 3 patients with sequelae of brachial plexus birth injury we observed associated movements and also synchronous motor unit potentials and motor axo-axonic reflexes between different muscles, sometimes with antagonist function. To explain these synkineses is necessary to assume that there is simultaneous innervation by the same motoneurone of 2 or more "motor subunits" in different muscles. Therefore we suggest that the motoneurone branches originate proximally from the primary trunk of the brachial plexus. Careful clinical and neurophysiological study can show these complex simultaneous inne
