What does built-in software of home ventilators tell us? An observational study of 150 patients on home ventilation.

BACKGROUND: Recent home ventilators are equipped with built-in software which provides data such as compliance, estimations of leaks, tidal volume, minute ventilation, respiratory rate, apnea and apnea-hypopnea indexes, and percentage of inspirations triggered by the patient (or ventilator). However, for many of these variables, there is neither consensus nor documentation as to what is to be expected in a population of stable patients under noninvasive ventilation (NIV). OBJECTIVES: To document the values and distribution of specific items downloaded from ventilator monitoring software, by d

[Neostigmine and edrophonium. Antagonism of profound and shallow mivacurium blockade].

UNLABELLED: Mivacurium has a short duration of action because it is rapidly hydrolysed by plasma cholinesterase. There is ongoing controversy concerning the antagonism of mivacurium-induced neuromuscular block, firstly because of its short spontaneous recovery time, and secondly because the metabolism of mivacurium may be inhibited by anticholinesterases. We therefore compared neostigmine and edrophonium reversal of deep and moderate mivacurium-induced blocks. METHODS: After approval by the local ethics committee, 48 ASA class I and II adult patients were investigated during nitrous oxide-fen

Neuromuscular Scoliosis: When, Who, Why and Outcomes.

Scoliosis has a very high prevalence among patients with neuromuscular disease involving the thoracic spine and truncal muscles. Physical examination and radiographs are used to screen for presence of scoliosis and monitor progression. Management includes therapy participation, optimizing equipment and orthotic use, and possible surgical intervention. Unlike idiopathic adolescent scoliosis, curves tend to progress despite orthotic use compliance. Associated pelvic obliquity creates risk for pressure sores and pain. As such, education of caregivers is a key point of optimizing management.

Spinal cord monitoring.

Spinal cord surgery carries the risk of spinal cord or nerve root injury. Neurophysiologic monitoring decreases risk of injury by continuous assessment of spinal cord and nerve root function throughout surgery. Techniques include somatosensory evoked potentials (SEPs), transcranial electrical motor evoked potentials (MEPs), and electromyography (EMG). Baseline neurophysiologic data are obtained prior to incision. Real-time signal changes are identified in time to correct compromised neural function. Such monitoring improves postoperative neurologic functional outcomes. Challenges in neurophysi

Construct Validity and Reliability of the OMNI Scale in Children and Adolescents With Neuromuscular Diseases.

INTRODUCTION/AIMS: Children and adolescents with neuromuscular diseases often demonstrate muscle weakness and mobility limitations, which may increase perceived exertion during functional tasks. The OMNI scale was developed to assess perceived exertion in pediatric populations; however, its measurement properties in neuromuscular conditions remain unclear. This study aimed to examine the construct validity of the OMNI scale, using the Borg scale as a reference, and its inter-rater reliability during the 6-minute walk test (6MWT) in children and adolescents with Charcot-Marie-Tooth disease (CMT

Force Plate Assessment of Neuromuscular Jump Performance Under Loaded and Unloaded Conditions in Military Personnel.

(1) Background: Military personnel are required to perform high-intensity actions and tactical tasks under external load, which increases system weight and alters movement mechanics. Understanding how these loaded conditions influence neuromuscular performance is essential for informing physical preparation and readiness monitoring. This study quantified the effects of tactical equipment on countermovement jump (CMJ) and countermovement rebound jump (CMRJ) force-time characteristics in active military personnel and evaluated the within-session reliability of these metrics under loaded and unlo

Pyridostigmine-induced complete atrioventricular block in a patient with Musk antibody-negative myasthenia gravis a case report : Pyridostigmine-ind advanced AV block in MG patient with MuSK-neg.

BACKGROUND: Myasthenia gravis (MG) is a rare autoimmune neuromuscular disorder in which autoantibodies impair neuromuscular junctions. MG can be associated with thymoma and with antibodies to the acetylcholine receptor (AChR), and is less commonly associated with antibodies to muscle-specific tyrosine kinase (MuSK). Treatment of AChR antibody-positive myasthenia gravis with the cholinesterase inhibitor, pyridostigmine, has known cardiac conduction side effects. Some reports indicate these cardiac effects, including bradyarrhythmias, occur more often with MuSK-ab positive MG. This report is of

Muscle or Fascial System Lesion (Part II): The Medial Gastrocnemius and the "Tennis Leg" Paradigm.

Muscle injuries have traditionally been interpreted as disruptions of contractile fibers. Increasing evidence, however, highlights the central role of the intramuscular connective tissue system - including endomysium, perimysium, epimysium, aponeuroses, and extracellular matrix - in force transmission, injury susceptibility, and recurrence risk. Medial gastrocnemius strain, or "tennis leg," exemplifies how myoconnective architecture dictates injury mechanics. The biomechanical interplay between the medial gastrocnemius (MG) and lateral gastrocnemius (LG) and between the MG and soleus (Sol) may

Ocular Adverse Events Associated With Antidepressants: A Large-Scale Data Analysis From the FAERS Database.

OBJECTIVE: Global antidepressant use has risen substantially, raising concerns about their potential ocular adverse effects. Nevertheless, large-scale comprehensive assessments of ocular side effects associated with various classes of antidepressants remain limited. METHODS: A retrospective pharmacovigilance analysis was performed using the FDA Adverse Event Reporting System (FAERS) from Q1 2015 to Q4 2024. Reports associated with antidepressants were systematically extracted, and disproportionality analyses were performed to identify potential associations with ocular adverse events (AEs).

Ventilation-induced gastric rupture after extubation in the ICU: a case report.

BACKGROUND: Gastric rupture is a rare yet potentially fatal complication that has been reported during cardiopulmonary resuscitation (CPR). This is generally attributed to acute gastric overdistension caused by chest compressions combined with positive-pressure ventilation. Ventilation-induced gastric rupture after planned extubation in the intensive care unit (ICU) is rare. CASE PRESENTATION: An 80-year-old man developed acute gastric rupture shortly after planned ICU extubation following lumbar spinal surgery. Quantitative neuromuscular monitoring was not performed prior to extubation. Four…

Diagnostic yield of a practical electrodiagnostic protocol discriminating between different congenital myasthenic syndromes.

Congenital myasthenic syndromes (CMS) are a group of heterogeneous diseases of the neuromuscular junction. We report electrodiagnostic testing (EDX) and genetic findings in a series of 120 CMS patients tested with a simple non-invasive EDX workup with surface recording of CMAPs and 3Hz repetitive nerve stimulation of accessory, radial and deep fibular nerves. Five ENMG phenotypes were retrieved based on the presence or not of R-CMAPs and the distribution pattern of decremental CMAP responses which significantly correlated with genetic findings (p <0.00001). R-CMAPs were found in all COLQ-mutat

Sleep abnormalities associated with neuromuscular disease: pathophysiology and evaluation.

The development of respiratory failure is common in patients with neuromuscular disorders that involve the respiratory muscles. However, the high incidence of sleep-related breathing problems in this population is less well known. In patients with neuromuscular disease, nocturnal breathing abnormalities frequently precede respiratory failure during wakefulness by months or even years. These nocturnal breathing problems are caused by multiple factors, including diaphragm and upper airway muscle weakness, scoliosis, obesity, and central respiratory control problems. Advances in the understanding

Recovery of early postoperative muscle strength after deep neuromuscular block by means of ultrasonography with comparison of neostigmine versus sugammadex as reversal drugs: study protocol for a randomised controlled trial.

INTRODUCTION: Despite the use of quantitative neuromuscular monitoring together with the administration of reversal drugs (neostigmine or sugammadex), the incidence of residual neuromuscular blockade defined as a train-of-four ratio (TOFr) <0.9 remains high. Even TOFr >0.9 cannot ensure adequate recovery of neuromuscular function when T1 height is not recovered completely. Thus, a mathematical correction of TOFr needs to be applied because the return of a normal TOFr can precede the return of a normal T1 twitch height. On the other hand, different muscles have different sensitivities to neurom…

The respiratory impact of novel therapies for spinal muscular atrophy.

The phenotype of spinal muscular atrophy (SMA) has been changing with the recent availability of three FDA-approved treatments: intrathecal nusinersen, intravenous onasemnogene abeparvovec-xioi, and enteral risdiplam. The degree of improvement in muscle strength and respiratory health varies with SMA genotype, severity of baseline neuromuscular and pulmonary impairment, medication used, and timing of the first dose. A spectrum of pulmonary outcomes has been reported with these novel medications when used early and in conjunction with proactive multidisciplinary management of comorbidities. In

Parathyroid disorders.

Disorders of the parathyroid glands most commonly present with abnormalities of serum calcium. Patients with primary hyperparathyroidism, the most common cause of hypercalcemia in outpatients, are often asymptomatic or may have bone disease, nephrolithiasis, or neuromuscular symptoms. Patients with chronic kidney disease may develop secondary hyperparathyroidism with resultant chronic kidney disease-mineral and bone disorder. Hypoparathyroidism most often occurs after neck surgery; it can also be caused by autoimmune destruction of the glands and other less common problems. Evaluation of patie

[Obstructive sleep apnea syndrome].

Four children were found to have clinically significant obstructive sleepapnea. Hypertrophy of the nasopharyngeal lymphoid tissue, specifically the adenoid and tonsils causing obstructive sleepapnea in children, is a well defined clinical entity with nocturnal CO2 retention, retarded growth and impaired physical and psychological status. A small number of these children may develop pulmonary hypertension, cor pulmonale and ultimately death. Children with only moderately enlarged tonsils in association with neuromuscular hypotonia and anatomical defects may also develop this syndrome. Polysomno
