Organophosphate neuropathy due to methamidophos: biochemical and neurophysiological markers.

Neuropathy target esterase (NTE), the putative target enzyme for organophosphate induced delayed polyneuropathy (OPIDP), can be measured in lymphocytes but has rarely been assessed in acute human poisoning. Serum autoantibodies to nervous system proteins develop in hens poisoned with neuropathic insecticides and also have not been studied after human poisoning. Serial lymphocyte NTE (LNTE) was measured in a 16-year-old boy after acute poisoning with methamidophos for evaluation as a predictor of subsequent neuropathy. The profiles of serum autoantibodies to neurofilament triplet proteins, myel

[Neurophysiological assessment of children with obstetric brachial plexus palsy].

INTRODUCTION: The objectives of the neurophysiological evaluation of infants with brachial plexus palsy are to determine the time of occurrence of the lesion, to locate the lesion and to determine its course. METHODS AND CONCLUSIONS: These objectives are achieved by studying affected upper extremity muscles by needle electromiography (EMG) and affected nerves by motor and sensory conduction studies. EMG is performed in the first week of life in those patients with brachial plexus palsy of unknown etiology to determine the age of the lesion for medico-legal reasons. EMG is performed before sur

A clinical, MRI and neurophysiological study of acute transverse myelitis.

There is paucity of studies correlating the MRI and evoked potential changes in acute transverse myelitis (ATM). We studied ten patients with ATM (age range 14-57 years; 8 men, 2 women) who were subjected to clinical, MRI and neurophysiological evaluation. The latter included median and tibial somatosensory evoked potentials (SEP), motor evoked potentials (MEP) to upper and lower limbs and concentric needle EMG. The outcome was defined on the basis of three month Barthel Index score. All the patients had pronounced lower limb and three had upper limb weakness. Magnetic resonance imaging scans

[A neurophysiological analysis of the sensory disorders in trigeminal nerve neuropathy].

52 patients with various types of trigeminal nerve neuropathy were examined in terms of trigeminal somatosensory evoked potentials (TSEP). The increase of latent periods and the decrease of N5-N33 amplitude components as well as disappearance of some separate components of this period were observed in patients with moderate facial pain and hypersthesia when sore side was stimulated. Meanwhile the alterations level of TSEP correlated with trigeminal hypesthesia severity. In a course of such stimulation all TSEP components disappeared in patients with severe sensory deficit and this turned out t

Stiff man syndrome: neurophysiological findings in eight patients.

The neurophysiological findings in eight patients with the stiff man syndrome (SMS), including four of six tested with autoantibodies against glutamic acid decarboxylase, are presented. Neurophysiological findings did not make it possible to discriminate between patients with and those without autoimmunity against GABAergic neurons. Investigation of mono- and polysynaptic reflexes revealed abnormal results in a variable number of SMS patients, the abnormalities largely corresponding to those seen in spastic paresis. A stereotyped motor response to electrical stimulation of peripheral nerves wa

Neurophysiologic assessment of urinary dysfunction in children with thoracic syringomyelia.

Isolated syringomyelia of the thoracic portion of the spinal cord is relatively uncommon. In children, signs and symptoms may involve only the urinary system. Five children who presented for evaluation of urinary tract dysfunction were found to have syringomyelia of the thoracic cord not associated with any other central nervous system abnormality on radiographic imaging. Each child underwent a complete neurologic examination and a complete electrophysiologic/urodynamic evaluation. One patient underwent surgical drainage of the syrinx due to progression of neurologic deficit, with slight impro

Neuropsychological assessment in MS: clinical, neurophysiological and neuroradiological relationships.

We assessed cognitive performance and its relationship with clinical and anatomic disease severity in MS with mild to moderate handicap; 34 definite MS and 18 healthy subjects matched for age and education were submitted to a neuropsychological test battery. Both groups were examined for anxiety. MS patients underwent magnetic resonance imaging examination. MS performed worse than controls on all WAIS-P subtests and had learning, short- and long-term verbal memory impairment. Cognitive deficits were not related to abnormal emotional states, but were found to be associated with attentional proc

Neurophysiologic diagnosis in uncooperative trauma patients: confounding factors.

Median, ulnar, and posterior tibial nerve somatosensory evoked potential (SSEP) studies were performed on a total of 239 comatose or uncooperative trauma patients with head injuries. Twenty-six of those patients were suspected of having an additional spinal cord injury. One patient had more than one suspected spinal cord injury and two patients had a suspected spinal cord injury and a suspected peripheral nerve injury. Eleven of the suspected spinal cord injuries were confirmed and 13 were not confirmed by the SSEP studies. In three patients the SSEP study proved inadequate to add further info

Seizure and acute osmotic change: clinical and neurophysiological aspects.

There are a number of clinical situations where overhydration may occur. If the reduction in plasma osmolality is acute, passive water influx swells brain cells, shrinking the extracellular space around them. It is during this time that susceptibility to generalized tonic-clonic seizure dramatically increases. Common clinical examples include hastened rehydration therapy, the dialysis disequilibrium syndrome, compulsive polydipsia, the syndrome of inappropriate ADH secretion (SIADH) and post-TURP syndrome. Treatments that tend to restore normal cellular volume (dehydration, mannitol infusion)

[Cryoglobulinemic neuropathy. A Clinical and neurophysiologic study of 13 cases].

Thirteen patients with essential mixed cryoglobulinemia and neuropathy were studied and followed up clinically and neurophysiologically during last nine years. All these patients had a mild distal, symmetrical motor sensory neuropathy. Few patients had a mononeuropathy. Three patients developed an acute severe tetraparesis due to overlapping mononeuritis multiplex. Neurophysiological study and nerve biopsy were indicative of axonal neuropathy. We discuss the pathogenesis of this neuropathy and the therapeutical approach.

Neurophysiological heterogeneity and the definition of dyslexia: preliminary evidence for plasticity.

Developments in the field of quantified electroencephalography have enhanced visualization of brain function in the learning disabilities. Optimal utilization of these techniques requires that populations under study be unambiguously defined. Evidence from the literature demonstrates that brain electrical activity of children with reading disability is more extensive and differs from that seen in children with "dyslexia-pure". Preliminary data are presented demonstrating that electrophysiological change seen in children with dyslexia-plus (dyslexia and attentional deficit disorder) could not b

[Neurophysiological studies on group A xeroderma pigmentosum in early childhood].

Neurophysiological studies were performed on 8 patients with group A xeroderma pigmentosum during early childhood. EEG, ABR and NCV were normal during this period. In contrast, various sleep parameters detected by polysomnography showed abnormal findings even in the neurologically normal patient. Decreased % sleep REM was seen in a case, and decreased frequency of REMs were seen in another. Body movements were extremely high or low in frequency in 3 cases in whole night sleep. The distribution of body movements were abnormal; in control subjects, the frequency was higher in SREM and stage 1 th

Neurochemical, neurophysiological, and neuropathological studies in vitamin E deficiency.

It is now recognized that vitamin E is essential for normal neurological structure and function in both man and experimental animals, with severe deficiency resulting in a characteristic neurological syndrome. The reasons why the neurological system should be particularly susceptible to a deficiency of this fat-soluble vitamin, and the mechanisms involved, are not known. In this review, the neurochemistry, neuropathology, and neurophysiology associated with vitamin E deficiency are described and correlated. A deficiency of vitamin E results in a "distal or dying back" axonal neuropathy which p…

Neurophysiologic intraoperative monitoring: II. Facial nerve function.

Intraoperative facial nerve monitoring provides a potentially useful adjunct to recent surgical advances in neurotology and neurosurgery. These measures further aid the surgeon in preserving facial nerve function by enhancing visual identification with electrical monitoring of mechanically evoked facial muscle activation. Facial nerve monitoring in neurotologic surgery may achieve the following goals: (1) early recognition of surgical trauma to the facial nerve, with immediate feedback made available to the surgeon through monitoring of mechanical activation; (2) assistance in distinguishing t

[Neurophysiological diagnosis of AIDS dementia complex: importance of electroencephalography].

This study reports the results obtained by an Electroencephalographic screening of 450 HIV-seropositive patients, belonging to different stages of infection. The major bioelectrical changes of cerebral activity were evident in AIDS group (87.2%) and in ARC group (64.5%), whereas mild abnormalities were present in LAS group (38.8%) and in asymptomatic infection (19.2%), often without any evidence of neuropsychological impairment. Therefore, we emphasize the usefulness and the sensibility of Electroencephalography and, in particular, of Electroencephalography coupled with Computerized Spectral A

Neurophysiologic maps of the cutaneous innervation of the external genitalia of the ewe.

The area of skin supplied by the afferent fibers in a peripheral nerve is called the cutaneous area (CA) of that nerve. The CA responsive to movement of wool or hair in the genital regions were mapped in 17 ewes, with the identifications of the peripheral nerves and of the spinal nerves contributing to the pudendal plexus being checked at necropsy. Differences were found in the origins and extent of CA of the cutaneous branches from the sacral plexus. The CA of the caudal rectal nerves and of a nerve that passed caudally between the caudal vertebrae and the ventral sacrococcygeus muscle lay la

Neurophysiologic maps of cutaneous innervation of the external genitalia of the ram.

The area of skin innervated by the afferent fibers in a peripheral nerve is called the cutaneous area (CA) of that nerve. Mapping of those areas that were responsive to movement of wool/hairs in the genital region of 18 anesthetized rams, combined with subsequent identification of spinal nerves and dissection of the pudendal nerve plexus, indicated considerable differences in the extent of the CA and the origins of cutaneous branches from the pudendal plexus. The CA of the proximal and distal cutaneous branches of the pudendal nerve (or plexus) overlapped craniocaudally by approximately one-ha

Rebound insomnia in neuroleptic drug withdrawal neurophysiologic characteristics.

Rebound insomnia is one of the medical effects of reduction in dosage or discontinuation of neuroleptic drugs. The electrophysiologic features of sleep dysfunction are reported and discussed in 3 patients manifesting withdrawal-related DIMS. Electrographic anachronism and cyclic alternating pattern are signs of N-Rem sleep dysfunction. Clinical and neurophysiologic data suggest that rebound insomnia in neuroleptic withdrawal is due to an enhancement of physiologic mechanisms and rebound supersensitivity of cholinergic transmission in the ARAS.

Friedreich's ataxia. A clinical review with neurophysiological and echocardiographic findings.

In a review of twelve cases of Friedreich's ataxia, the important criteria for early diagnosis were identified. Clinical signs included absent tendon reflexes, ataxic gait, and a positive Romberg test. Nerve conduction velocities showed absent or severely slowed sensory conduction, with normal motor conduction. In addition, echocardiographic changes of symmetrical hypertrophic cardiomyopathy were found.

Neurophysiological findings among workers exposed to organic solvents.

Neurophysiological findings among patients with solvent poisoning and among groups with long-term occupational exposure to various solvents are reviewed. Hydrocarbons with six carbon atoms have been shown to cause peripheral neuropathy, which can be revealed with electroneurography and electromyography. Various mixtures of solvents and carbon disulfide have caused similar types of abnormalities. Abnormal electroencephalograms have been reported for patients with solvent poisoning and also connected to occupational exposure. Visual evoked potentials have rarely been applied to study of solvent
