Surgery or gamma -knife for the treatment of arteriovenous malformations?

Decision making for either surgery or gamma-knife for the treatment of arteriovenous malformations (AVMs) cannot be uniform. The skill of the neurosurgeon in operating on AVMs is now being compared with that of the gamma-knife. The decision varies from case to case and is to be taken by the neurosurgeon. This report presents three cases in which such decision making was not easy. Case 1 was a non-ruptured cingulate AVM of 2.5 cm diameter in the cingulate cortex. The operative field was anticipated to be very narrow between the parietal bridging veins. Case 2 was a tiny ruptured AVM in the spee

[Management of craniofacial fractures with titanium mini-plate osteosynthesis and primary bone transplantation].

The frontobasal traumatic lesions are often combined with several type of midface fractures therefore the authors adopted the craniofacial injury definition. In the years 1995-1997, 534 patients with head trauma were operated on by maxillofacial surgeon and neurosurgeon of which 35 (6.5%) cases showed concomitant combined frontonaso-orbital-skull base involvement. In the complex management of the severe craniofacial lesions the cooperative team-work of maxillofacial surgeon, neurosurgeon, otolaryngologist and ophthalmologist is required. Stabilized midface and the "watertight" dura closure or

Risk of infection after cerebrospinal fluid shunt: an analysis of 884 first-time shunts.

Postoperative infections are major complications of cerebrospinal fluid (CSF) shunting in the treatment of hydrocephalus and other conditions with obstructed CSF circulation. In a retrospective study 884 first-time shunted patients with hydrocephalus operated on in the years 1958-1989 were investigated with special reference to the infection rate and to the influence of the following variables: time period, age of the patient, education of the neurosurgeon, length and time of the operation and the exact placement of the distal drain. The overall infection rate for all implanted CSF shunts was

New approaches in the treatment of hypertensive intracerebral hemorrhage.

A comparison of the long-term outcome after surgical and nonsurgical treatment of hypertensive putaminal hemorrhage was performed in Japan over the last 15 years to determine the appropriate role of surgery. The overall results show a poor response to surgical treatment, but neurosurgeons also know that hematoma evacuation may bring about a dramatic result in some cases. In addition, experimental studies have shown that hematoma evacuation improves neuronal function at the penumbra. The discrepancy between the results of this study and the neurosurgeon's clinical impression is probably a refle…

Strabismus after endoscopic third ventriculostomy.

Until recently, the treatment of choice for noncommunicating hydrocephalus included the use of valve-regulated shunts such as the ventriculoperitoneal shunt. High failure rates and multiple complications have led neurosurgeons to use other techniques. Endoscopic third ventriculostomy (ETV) is becoming popular as treatment for noncommunicating hydrocephalus when standard shunts have failed.(1) We present 2 adolescents who developed exotropia, hypertropia, and diplopia after endoscopic third ventricle surgery.

Surgical treatment of congenital fronto-orbitomalar asymmetries.

Congenital fronto-orbital asymmetries can be of various origin. Correction of these malformations, whether due to maldevelopment of the foetus or to a congenital tumour, generally require a combined intracranial and extracranial approach. In recent years, a working group of plastic surgeons and neurosurgeons in the French-speaking part of Switzerland have been particularly interested in these problems. Some of their results are presented here.

Intracranial arachnoid cysts: Review of natural history and proposed treatment algorithm.

BACKGROUND: With a prevalence of 1.4%, intracranial arachnoid cysts are a frequent incidental finding on MRI and CT. Whilst most cysts are benign in the long-term, clinical practice, and imaging frequency does not necessarily reflect this. METHODS: A literature review was conducted searching the Medline database with MESH terms. This literature was condensed into an article, edited by a consultant neurosurgeon. This was further condensed, presented to the neurosurgery department at Princess Alexandra Hospital for final feedback and editing. RESULTS: This review advises that asymptomatic pati

The treatment of cavernous sinus meningiomas: evolution of a modern approach.

Cavernous sinus meningiomas (CSMs) are challenging lesions for the skull base neurosurgeon to manage given their close association with cranial nerves II-VI and the internal carotid artery. In the 1980s and early 1990s, with advancements in microsurgical techniques, increasing knowledge of the relevant microsurgical neuroanatomy, and the advent of advanced skull base surgical approaches, the treatment of CSMs involved attempts at gross-total resection (GTR). Initial fervor for a surgical cure waned, however, as skull base neurosurgeons demonstrated the limits of complete resection in this regi…

Practical and technical aspects of trans-sphenoidal surgery.

Trans-sphenoidal surgery was first described more than a century ago. Today, this approach occupies a crucial place in the armamentarium of the neurosurgeon for the management of sellar, suprasellar, and parasellar pathological conditions. Over the years, the procedure has witnessed multiple modifications, benefitting from technological advances and from innovative ideas of pioneering neurosurgeons and otolaryngologists. The introduction of the microscope and then the endoscope allowed progressive improvement of visualization, illumination, and magnification in this restricted surgical corrido…

Planning for better standards in the care of the neurological sequelae of trauma.

Injuries of the nervous system are common, and cause many deaths and much permanent disability. They inflict an enormous financial burden on … a small minority of such injuries receive neurosurgical care. This is chiefly because the number of neurosurgeons at present available is small in relation to the tasks. Many cases must therefore be in the care of general or orthopaedic surgeons, especially in country centres, and it is important that these surgeons should have some basic neurosurgical training …

A semantic segmentation model for automatic precise identification of pituitary microadenomas with preoperative MRI.

PURPOSE: Magnetic resonance imaging (MRI) is an essential technique for diagnosing pituitary adenomas; however, it is also challenging for neurosurgeons to use it to precisely identify some types of microadenomas. A novel neural network model was developed using preoperative MRI to assist clinicians in diagnosing pituitary microadenomas. METHOD: Sixty patients with pathologically diagnosed pituitary microadenomas, including hyperprolactinemia (n = 19), growth hormone microadenomas (n = 17), and adrenocorticotropin microadenomas (n = 24), were enrolled. An image edge-supervised same receptive …

Simultaneous Combined Endonasal and Transorbital Endoscopic Approach for a Large Orbital Cavernous Venous Malformation: A Technical Case Report.

Orbital cavernous venous malformation (OCVM) is among the most common benign intraconal orbital lesions. Traditionally, ophthalmologists have treated these lesions through a transorbital approach (TOA), while neurosurgeons have favored transcranial access. More recently, the endoscopic endonasal approach (EEA) has become preferred for lesions located inferomedial to the optic nerve. In selected complex cases of orbital lesions, combined approaches integrating EEA and TOA have been employed to overcome the limitations of each method. We report the case of a woman in her 50s with a prior history…

The impact of arachnoid structures on skull-base meningioma surgical management: a radiological analysis and narrative review.

The presence of intact arachnoid membranes between skull base meningiomas and critical neurovascular structures is crucial for predicting surgical outcomes, understanding tumor development and growth, and planning the feasibility of tumor resection or the need for adjuvant treatments. While neurosurgeons often utilize the subarachnoid cisterns to enhance access to these tumors and facilitate their removal, a comprehensive review aimed at health professionals involved in the diagnosis and treatment of this complex pathology, including radiologists, neurologists, oncologists, ophthalmologists, a…

A standardised protocol for neuro-endoscopic lavage for post-haemorrhagic ventricular dilatation: A Delphi consensus approach.

Neuro-endoscopic lavage (NEL) has shown promise as an emerging procedure for intraventricular haemorrhage (IVH) and post-haemorrhagic ventricular dilatation (PHVD). However, there is considerable variation with regard to the indications, objectives, and surgical technique in NEL. There is currently no randomised trial evidence that supports the use of NEL in the context of PHVD. This study aims to form a consensus on technical variations in the indications and procedural steps of NEL. A mixed-methods modified Delphi consensus process was conducted between consultant paediatric neurosurgeons ac…

[Computational Fluid Dynamics(CFD)].

Computational fluid dynamics(CFD)is a useful tool for simulating blood flow and has been applied to hemodynamic analysis in cerebrovascular disease. Although CFD requires an engineering approach, it can potentially contribute to preoperative surgical simulation as an intraoperative aid. In this study, we describe the basic hemodynamic parameters for CFD analysis and demonstrate their effective practical use by focusing on intracranial aneurysms. A thinning cerebral aneurysmal wall indicates a rupture risk, and it cautions neurosurgeons of an intraoperative rupture. High pressure and low wall s…
