Right Brain: Finger-Nose, There and Back Again.

This poetic reflection highlights how routine bedside tests become opportunities for multifaceted learning for trainees. The "finger-to-nose" test used in neurologic examinations not only provides insight into neurologic signs but also teaches about clear communication, patience, and connecting with the patient's way of thinking. Drawing on real-life, day-to-day clinical encounters, the piece explores the patient's often misinterpreted instructions and the unexpected responses they provoke as the trainee learns to administer the neurologic examination.I say it gently, as a part of my examinati…

Ear, nose and throat disorders and international travel.

BACKGROUND: Disorders of the ear, nose and throat may be the most commonly occurring pre-existing health condition in international travellers. Despite their high incidence, there is limited guidance for travellers and their clinicians on their prevention and management. This narrative review addresses this deficit by compiling and discussing available evidence on this neglected subject. METHODS: A comprehensive review of the literature was undertaken using Medine and Scopus databases and multiple combinations of relevant MeSH search terms. Further references were obtained from focused search…

Saddle Nose Deformity Reconstruction with a Allograft Bone.

OBJECTIVE: Providing lasting cosmetic and functional results for patients with saddle nose deformity with allograft. METHODS: This report describes experience with using a of freeze-dried allograft bone allograft in 58 patients who underwent dorsal augmentation over 5-year period (2018-2023). Thirty-eight patients had saddle-shaped deformity of the nose, and 16 patients had post-traumatic cases with saddle nose deformities. All patients underwent a clinical examination using computed tomography. Before surgery using a 3D model, the graft was contoured according to the shape of the nasal defec…

Diagnostic Performance of Electronic Nose Technology in Sarcoidosis.

BACKGROUND: Diagnosing sarcoidosis can be challenging, and a noninvasive diagnostic method is lacking. The electronic nose (eNose) technology profiles volatile organic compounds in exhaled breath and has potential as a point-of-care diagnostic tool. RESEARCH QUESTION: Can eNose technology be used to distinguish accurately between sarcoidosis, interstitial lung disease (ILD), and healthy control subjects, and between sarcoidosis subgroups? STUDY DESIGN AND METHODS: In this cross-sectional study, exhaled breath of patients with sarcoidosis and ILD and healthy control subjects was analyzed by u…

Counter Autografting of Dorsal Septum in Crooked Nose.

INTRODUCTION: The nasal septum plays an important role in nasal form and function. In this study, we describe a novel, alternative septoplasty approach that results in an aesthetically acceptable dorsal profile and improves airway function in patients with a crooked nose. PATIENTS AND METHODS: This study enrolled 26 patients who presented with crooked noses between 2012 and 2017. All patients underwent open rhinoplasty under general anesthesia. During correction of the cartilaginous part, a dorsal strip from the deviated septum was prepared and inverted in a 180° fashion to exert a counter-fo…

Nose defects reconstruction with the Zitelli bilobed flap.

The skin cancers of the head and neck district are localized mostly on the nose, which, by being in the center of the mid-face, has an important aesthetic role. Therefore, many reconstructive techniques have been studied; among them regional skin flaps appeared the most suitable since they guaranteed the oncological radicality as well as a good aesthetic outcome. In this paper, we present our experience with the use of the Zitelli bilobed flap, which is defined as a double transposition flap for the reconstruction of the nose defects. From 2008 to 2012, we have treated 86 patients (56 men and …

Asymptomatic flesh-colored lobular nodule on the nose.

Chondroid syringoma is a rare benign tumor of the skin appendages. Chondroid syringoma is mostly manifested by a slow-growing, painless, well defined subcutaneous or intradermal nodule. A 43-year-old man presented to our clinic with the complaint of an asymptomatic nodule on the dorsum of the nose for 8 months. The lesion was totally excised. Histopathological examination was notable for epithelial islets embedded in the chondroid matrix in the dermis. The patient was diagnosed with chondroid syringoma in light of the clinical and histopathological findings.

Semilunar conchal cartilage graft in saddle nose reconstruction.

INTRODUCTION: The saddle nose deformity is easily recognized by the loss of septal support and nasal dorsal height with adverse functional and aesthetic consequences. TECHNIQUE: We treated a 50-year-old woman and a 54-year old man that presented with a moderate saddle nose deformity following a previous septorhinoplasty (female patient) and a posttraumatic severe saddle nose deformity (male patient). The patients were treated by open approach rhinoplasty under general anesthesia, and the saddle nose deformity was reconstructed with a semilunar conchal cartilage graft. A semilunar part of the …

Nose-To-Brain Delivery of PLGA-Diazepam Nanoparticles.

The objective of the present investigation was to optimize diazepam (Dzp)-loaded poly(lactic-co-glycolic acid) nanoparticles (NP) to achieve … d on Sprague-Dawley rats using technetium-99m-labeled ((99m)Tc) Dzp formulations to investigate the nose-to-brain drug delivery pathway. Brain/blood uptake ratios, drug targeting efficiency, and direct nose-to-brain transport were found to be 1.23-1.45, 258, and 61% for (99m)Tc-DNP (i.n) compared to (99m)Tc-DS (i.n) (0.38-1.06, 125, and 1%). Inside the brachycephalic nose: intranasal mucosal contact points.

The purpose of this study was to evaluate the prevalence of intranasal mucosal contact points in brachycephalic and normocephalic dogs. In total, 82 brachycephalic dogs (42 pugs and 40 French bulldogs) were evaluated by rhinoscopy for their intranasal mucosal contact and 25 normocephalic dogs were evaluated as a control group. Of those, 162 brachycephalic nasal cavities were evaluable and 140 had contact between intranasal structures (87%). Intraconchal and septoconchal mucosal contact points were the most commonly detected sites of contact. French bulldogs had a significantly higher prevalenc
