A case of disturbed vertical gaze.

A 10-month-old boy with vertical ocular motor apraxia (OMA) is presented. An optokinetic projector confirmed absence of downward saccades. No case of idiopathic vertical OMA has previously been described. A review of the MRI, initially reported as normal, showed pathology in areas known to be involved with vertical eye movements: the cerebellum and thalamus.

July 2000: A 70 year old with rigidity, decreased ocular movements, and dementia.

The July Case of the Month (COM): A 70 year old male presented with a four year history of cognitive decline, difficulty expressing himself, and an increasingly unsteady gait with numerous falls. At presentation he was wheel-chair bound. Examination showed some slowing of speech, mild memory impairment, but normal cranial nerves. Spastic weakness and brisk reflexes were also noted, with bilateral ankle clonus. MRI scans were normal. Four years later he was admitted with a urinary tract infection and was mute with severely impaired ocular motility. He died 18 months later and autopsy showed the

Ocular motor abnormalities in Gaucher disease.

Gaucher disease (GD) without primary neurological involvement (GD1) is now treatable with exogenous enzyme replacement therapy (ERT). ERT does not halt the fatal neurological progression of type 2 (infantile) disease (GD2), but in type 3 disease (GD3), neurological progression may be slowed down or possibly halted in some cases by higher doses of ERT. Therefore, it is crucial to distinguish between GD1 and GD3 disease for appropriate treatment. Saccade initiation failure (SIF) (ocular motor apraxia, supranuclear gaze palsy) is often the earliest neurological sign in GD3. This sign can be diffi

Reliability of and comparisons among three variations of the alternating cover test.

One of the most commonly used methods for measuring heterophoria is the alternating cover test. Differences in specific procedures and observational criteria used by examiners may result in different measurements of heterophoria on the same patient. This study evaluated the inter-examiner reliability of three different cover test techniques: the estimated cover test, the prism neutralized objective cover test, and the prism neutralized subjective cover test. Two examiners performed each technique on each of 72 subjects. Reliability was assessed using correlational methods and mean difference c

Correlation of clinical improvement with pontine lesions in opsoclonus-myoclonus syndrome.

In a case of acute disseminated encephalomyelitis displaying opsoclonus-myoclonus syndrome, magnetic resonance images (MRI) disclosed pontine lesions involving part of the pontine paramedian reticular formation and the raphe nucleus. Shrinkage of the lesions correlated with the patient's clinical state. This observation supports the hypothesis that a pontine lesion can cause opsoclonus-myoclonus syndrome.

[A case of opsoclonus-myoclonus syndrome associated with anti-central nervous system antibody].

We reported a case of opsoclonus-myoclonus syndrome. A 63-year-old man was admitted to Kenwakai Hospital with rapidly progressing symptoms, including lumbago, whole body pain, vertigo, nausea, and anorexia. He became bed-ridden because of severe vertigo and truncal ataxia. Five days after admission, he developed opsoclonus followed by myoclonus and mild disturbance of consciousness, but he showed no appendicular ataxia or pyramidal tract sign. He was treated with prednisolone, 40 mg/day, which was effective for disturbance of consciousness, but opsoclonus and myoclonus persisted. He died of li

[Treatment of strabismus with botulinum toxin].

Having used botulinum toxin for four years, we describe our experience in one hundred squinting patients and compare our results with the literature. We have good results in unilateral sixth nerve palsy and small deviations with binocular potential. Botulinum toxin can also be helpful in third and fourth nerve palsy, in Graves' ophthalmopathy, as an adjunct to transposition surgery and in cases of under- or overcorrections after surgery. In cases of muscle fibrosis and wide angle strabismus, the results are more disappointing. We describe an original method of conditioning the toxin in individ

Congenital ocular motor apraxia: sporadic and familial. Support for natural resolution.

Four patients aged between 9 months and 17 years were detected to have congenital ocular motor apraxia (COMA) over a 10-month period. Three of them were siblings. All exhibited the classical signs of a horizontal saccadic palsy. However, the signs were less pronounced with increasing age of the patient. This supports the observations of other authors who have noted an age-related resolution. There was no evidence of other motor developmental delay in any of the patients and computed tomography revealed no abnormalities. Both these findings are in contradistinction to prior reports of central n

Unexpected cyclotropia simulating disruption of fusion.

Fifteen patients with diplopia associated with prior scleral buckling, prior penetrating keratoplasty, severe corneal scarring, monocular aphakia, long-standing strabismus, or prior vertical offsets of the horizontal recti were thought to have disruption of fusion because diplopia could not be eliminated with prisms. They were each found to have a substantial symptomatic cyclotropia unassociated with an obvious dysfunction of an oblique muscle. In 13 patients, diplopia resolved after the cyclotropia was corrected surgically.

Eyelid motor abnormalities in progressive supranuclear palsy.

Eyelid motor abnormalities found in progressive supranuclear palsy are reviewed. Electrophysiological correlates of blepharospasm, levator inhibition (blepharokolysis) and supranuclear paralysis of lid closure are presented. Disorders of eyelid motility are not uncommon in progressive supranuclear palsy (PSP). They may be found in about one third of patients with this syndrome (Jackson et al., 1983; Golbe et al., 1989). This is not surprising since ocular and eyelid movements are highly coordinated, mainly in the vertical plane (Gordon, 1951; Kennard and Smith, 1963; Kennard and Glaser, 1964),

Convergence insufficiency symptom survey (CISS) scores are predictive of severity and number of clinical signs of convergence insufficiency in young adult Africans.

PURPOSE: This study aimed to investigate the correlation between convergence insufficiency symptom survey (CISS) score and the signs of convergence insufficiency (CI) and to evaluate the sensitivity and specificity of the CISS to predict CI and ametropia among young adult university students. METHOD: This prospective cross-sectional clinic-based study included 300 first year university students (mean age = 21.58 (SD ± 2.2) years) who consecutively reported for eye examination. Participants were administered the CISS questionnaire and investigated for the signs of CI. Diagnosis of CI was based

Maldevelopment of convergence eye movements in macaque monkeys with small- and large-angle infantile esotropia.

PURPOSE: To describe symmetric convergence eye movements evoked by disparity and/or accommodative cues in esotropic macaque monkeys, with the goal of determining whether these animals have the vergence deficits found in humans with esotropia. METHODS: Physical far and near targets were used to evoke large (approximately 8 degrees) symmetric convergence eye movements in four adult macaque monkeys (two with strabismus, two normal), using positive-feedback rewards. One strabismic monkey had infantile-onset, small-angle esotropia (small-eso approximately 2 degrees) induced by alternating occlusio

Eye Segmentation Method for Telehealth: Application to the Myasthenia Gravis Physical Examination.

Due to the precautions put in place during the COVID-19 pandemic, utilization of telemedicine has increased quickly for patient care and clinical trials. Unfortunately, teleconsultation is closer to a video conference than a medical consultation, with the current solutions setting the patient and doctor into an evaluation that relies entirely on a two-dimensional view of each other. We are developing a patented telehealth platform that assists with diagnostic testing of ocular manifestations of myasthenia gravis. We present a hybrid algorithm combining deep learning with computer vision to giv

Surgical Treatment of Congenital Brown Syndrome, A Review.

Background and Purpose: Brown syndrome is an ocular motility disorder that was first described by Harold Brown in 1950. The pathology is attributed to the superior oblique tendon and sheath. It creates a vertical deviation with limitation of elevation of the affected eye in adduction. There is no standard management strategy. The purpose of this paper is to summarize the methods introduced so far, along with their advantages and disadvantages.Patients and Method: Related articles from 1950 to 2022 were searched in Google Scholar, PubMed, and Scopus based on the keywords of this article. Results

Patient-Related Outcome Measures for Oculomotor Symptoms in the Cerebellar Ataxias: Insights from Non-Cerebellar Disorders.

In patients with cerebellar ataxia (CA), symptoms related to oculomotor dysfunction significantly affect quality of life (QoL). This study aimed to analyze the literature on patient-related outcome measures (PROMs) assessing QoL impacts of vestibular and cerebellar oculomotor abnormalities in patients with CA to identify the strengths and limitations of existing scales and highlight any areas of unmet need. A systematic review was conducted (Medline, Embase) of English-language original articles reporting on QoL measures in patients with vertigo, dizziness or CA. Pre-specified parameters were
