[Atypical optic neuritis in systemic lupus erythematosus (SLE)].

HISTORY AND CLINICAL DATA: A 67-year-old woman experienced acute unilateral visual loss accompanied by pain with eye movements. There was a marked relative afferent pupillary defect and a nerve fiber bundle defect in the upper half of the visual field. Optic discs were normal. After 4 days vision worsened to motion detection and only a temporal island was left in the visual field. The optic disc margin was blurred. Since thirty years she had been suffering from renal insufficiency. Immunoserologic examination revealed elevated ANA and DS-DNA antibody titers. An optic neuritis in systemic lupus…

[The multi-channel VEPs topography in optic neuritis].

The multi-channel VEPs topographies of 20 normal persons and 22 patients suffered from optic neuritis were recorded. In normal subjects, the topography showed symmetric distribution by full-field stimulation and paradoxical lateralization by half-field stimulation. In patients with optic neuritis, it showed asymmetric distribution most on the temperal side, some on the nasal side and occasionally in the middle by full-field stimulation. The result suggests that the optic nerve may be damaged on either temperal or nasal. The effect of the early diagnosis and evaluating treatment using topograph…

[Clinical and morphological picture of retrobulbar optic neuritis].

A case of unilateral retrobulbar neuritis was described. The patient, 43-year-old man died because of the extensive postinfarctional changes on the second day of the treatment. Retrobulbar part of the optic nerve and chiasma opticum were investigated. The microscopic pictures represent the correlations between morphological changes and their pressure on the neural fibres connected with visual disturbances.

[A case of optic neuritis following rubella encephalitis].

A 5-year-old boy with left side optic neuritis, which manifested itself 21 days after the onset of rubella eruption, is reported. Seven days after the onset of rash which persisted for 3 days, he was found to be excessively somnolent and disoriented, and a diagnosis of rubella encephalitis was made at our clinic. Clinical symptoms spontaneously subsided 3 days later. Twenty-one days after the appearance of rash (16 days after the onset of encephalitis) he complained of left frontal headache and blurred vision of left eye. The funduscopic examination revealed the left papillitis. Pattern revers…

[Atypical forms of optic neuritis of multiple sclerosis].

The authors report seven cases of optic neuritis in patients with a diagnosis of multiple sclerosis. All patients had severe bilateral eye disease with no or incomplete recovery. Clinical signs and symptoms of multiples sclerosis were observed in only some patients. IRM was remarkably sensitive for detecting cerebral lesions and confirming the diagnosis. In every case visual field abnormalities consisted of a diffuse defect of sensitivity with pericentral scotoma. The authors emphasize on two atypical forms, total acute neuropathy and chronic primary neuropathy.

Clinical and magnetic resonance imaging in optic neuritis.

We found 23 of 48 patients (48%) with isolated monosymptomatic optic neuritis (ON) to have 1 to several brain lesions by MRI. All the brain lesions were clinically silent and had characteristics consistent with multiple sclerosis (MS). During 4 years of follow-up, 9 patients (19%) developed definite MS on clinical grounds. Six of the converting patients had abnormal MRIs; the other 3 had MRIs that were normal both initially (when they had ON only) and when repeated after they had developed MS. The other 17 patients with abnormal MRIs have not developed symptoms or signs of MS during follow-up.…

Demyelinating optic neuritis: clinical features and differential diagnosis.

Optic neuritis is a common cause of sudden visual impairment in young adults. Early recognition by the clinician would alleviate much of the anxiety experienced by sufferers, but other conditions that may cause permanent blindness must be excluded. These can usually be discerned from the history and confirmed by a brief neuro-ophthalmological examination.

T-lymphocyte subpopulations in acute unilateral optic neuritis.

The authors prospectively analyzed T-lymphocyte subpopulations in the peripheral blood of nine patients with acute unilateral optic neuritis and compared them with 25 controls without neurologic disease. The presence or absence of alterations in circulating T-cell subsets has not been examined previously in patients with isolated optic neuritis. The authors found the mean ratio of inducer (CD4) to suppressor (CD8) T-lymphocytes was 2.07 +/- 0.51 for the group with optic neuritis, statistically indistinguishable from a value of 1.78 +/- 1.04 for the control group. Multiple sclerosis (MS) subseq…

Colour and luminance vision in human optic neuritis.

A comparison of sensitivities to chromatic and luminance stimuli has been carried out in patients with a past history of optic neuritis. Patients were selected with differing degrees of stable residual visual deficits, and with marked interocular differences in sensitivity. Threshold contrast sensitivity was measured to sinusoidal luminance gratings and to chromatic red/green and blue/yellow gratings, all with the same spatial frequency of 1 cycle per degree. A psychophysical criterion was used to ensure that detection of the chromatic grating was determined only by its colour differences. Whe…

Optic neuritis and myelopathy in systemic lupus erythematosus.

The optic neuritis of systemic lupus erythematosus (S.L.E.) more frequently results in the persistence of a central scotoma or complete blindness after a single attack than demyelinating optic neuritis, although the initial clinical presentations may be identical. A significant number of patients, however, recover normal vision. Optic neuritis may be the presenting symptom of S.L.E. and as myelopathy may also occur in the course of the disease, confusion with multiple sclerosis may result, especially if there are no arthritic, cutaneous nor visceral manifestations. We report a case of lupus op…

Electrophysiological disorders in multiple sclerosis and optic neuritis.

Visual evoked response (VER), auditory brainstem evoked response (ABER), somatosensory evoked response (SSER), blink reflex and electronystagmographic (ENG) investigative methods were applied to a group of 89 patients with Multiple Sclerosis (MS) and Optic Neuritis (ON). The MS patients were classified as definite (n = 31), probable (n = 31) and possible (n = 27). The aim of this study was to determine the diagnostic value of the five electrophysiological tests in MS. VER and ABER recordings were found to reveal the highest number of asymptomatic abnormalities (33 and 31 percent respectively).…
