Orbital Anatomy.

This article outlines orbital anatomy and imaging correlates for diagnosis and surgical planning. The bony orbit houses the globe, optic nerve, extraocular muscles, lacrimal apparatus, vessels, and fat, with communications to cavernous sinus and intracranial space through foramina. Bony walls include roof, floor, medial lamina papyracea, and lateral walls; periosteum forms a delimiting potential space. The globe comprises cornea, sclera, iris, lens with zonules, and anterior and posterior chambers, while the posterior segment contains the vitreous, choroid, retina, and optic nerve. Extraocular…

Orbital Hypertelorism.

Orbital hypertelorism may be seen in isolation or more commonly within the context of other congenital craniofacial dyscrasias such as craniosynostosis, midface hypoplasia, exorbitism, atypical orofacial clefting, and malocclusion. Careful consideration of the preoperative cranial, orbital, and midface phenotypes, as well as the time of intervention, is imperative to choosing the optimal surgical approach to orbital repositioning. Virtual surgical planning serves as a powerful tool to increase surgical accuracy, decrease skeletal relapse, and mitigate historical complication profiles.

[Orbital trauma].

Orbital trauma can occur independently or in conjunction with other craniofacial trauma and can cause damage to bony and neurovascular structures as well as soft tissues. Appropriate interdisciplinary treatment of patients is essential to prevent long-term damage such as blindness or muscle dysfunction. Even complex fractures and soft tissue damage can be visualized using modern thin-layer computed tomography (CT), which is necessary for proper treatment.

Orbital Ependymoma.

Orbital ependymomas are rare in the orbit and usually occur secondary to extracerebral extension of an intraventricular ependymoma. The authors present a rare case of orbital ependymoma in a 74-year-old female. The patient was initially diagnosed with intraventricular ependymoma at the age of 13 years that required multiple repeat craniotomies for tumor recurrence. She then developed progressive tumor growth with extension into the bilateral frontal lobes and orbit. The orbital involvement produced binocular diplopia, epiphora, and globe distortion with compressive optic neuropathy. To the aut…

Orbital indications.

GKNS for uveal melanoma has become a recognized valued treatment which avoids enucleation of the eye. In the early days, the eccentric location of these tumors made treatment difficult, but Gamma Knife Perfexion has solved that problem. It is known that larger tumors and tumors with an anterior location have a worse prognosis. GKNS has also been of rather unexpected benefit in optic nerve gliomas that require treatment. Choroidal hemangiomas may benefit from treatment as can secondary glaucoma. It has also been found to be beneficial in the treatment of thyrotoxic ophthalmopathy.

Orbital Trauma.

The orbit is contained within a complex bony architecture with overlying soft tissue that involves many important anatomical structures. Orbital trauma is a frequent cause of damage to these structures. The authors review the literature on reconstructive techniques focusing on fractures of the orbital rim, orbital roof, orbital floor, medial orbital wall, and naso-orbito-ethmoid complex. A thorough literature review was conducted using PubMed analyzing articles relevant to the subject matter. Various search terms were used to identify articles regarding orbital trauma presentation, diagnosis, …

Orbital lymphoma.

Orbital lymphomas constitute 50-60% of ocular adnexal lymphomas. A total of 2211 cases of orbital lymphoma with a known subtype have been reported in the last 24 years (1994-2017). The vast majority of orbital lymphomas are of B-cell origin (97%), of which extranodal marginal zone B-cell lymphoma (EMZL) (59%) is the most common subtype, followed by diffuse large B-cell lymphoma (23%), follicular lymphoma (9%), and mantle cell lymphoma (5%). Orbital lymphoma is primarily a disease of the elderly. Gender distribution varies according to lymphoma subtype. However, extranodal marginal zone B-cell …

[Orbital traumatology].

BACKGROUND: In the case of trauma to the midface, an accompanying fracture of the orbit is common because to the anatomical conditions (bony buttresses of the midface). Etiology varies according to locoregional conditions. MATERIALS AND METHODS: An overview of orbital traumatology including diagnostic procedures, symptoms and therapy is provided. In addition, our own patient collective over one year including a 5-year follow-up was prospectively analyzed. RESULTS: The possible fractures of the midface with involvement of the orbit including their classifications are listed. The key clinical …

Orbital cysticercosis.

A young male patient presented to our ocular emergency department with chief complaints of progressive pain, redness, diplopia and a right-sided face turn. Ocular examination revealed severely restricted extraocular movements along with retinal folds in the left eye. Initial orbital ultrasound and CT findings were equivocal; however, serology favoured an infective cause. Considering the endemicity of the disease and equivocal investigation findings, a diagnosis of orbital cysticercosis with an atypical presentation was made. The patient was managed medically with a combination of oral albendaz…
