Beyond Strength: Participant Perspectives on the Benefits of an Older Adult Exercise Program.

This study examines the expected and experienced benefits among participants in Enhance®Fitness (EF), an evidence-based group physical activity program for older adults. We also describe the implications for program dissemination (reach, implementation, and maintenance) within the RE-AIM (reach, effectiveness, adoption, implementation, and maintenance) framework. Twenty semistructured interviews were conducted with EF participants enrolled from 2005 to 2012. Interviews were digitally recorded, professionally transcribed, and analyzed using a deductive approach. Participants were motivated to j

Beyond burnout: professional grief and moral distress in pediatric neuro-oncological neurosurgery.

OBJECTIVE: Pediatric neuro-oncology exposes healthcare professionals to high technical complexity, ethical uncertainty, and sustained emotional stress. Professional grief, moral distress, and second victim experiences are increasingly recognized in pediatric oncology, yet they remain insufficiently characterized in pediatric neuro-oncological neurosurgery. METHODS: The authors conducted a cross-sectional, anonymous survey among healthcare professionals involved in pediatric neuro-oncology at a tertiary children's hospital. A 27-item questionnaire assessed 5 domains: professional grief, moral

Organisation of care for pregnancy in patients with congenital heart disease.

Improvements in surgery have resulted in more women with repaired congenital heart disease (CHD) surviving to adulthood. Women with CHD, who wish to embark on pregnancy require prepregnancy counselling. This consultation should cover several issues such as the long-term prognosis of the mother, fertility and miscarriage rates, recurrence risk of CHD in the baby, drug therapy during pregnancy, estimated maternal risk and outcome, expected fetal outcomes and plans for pregnancy. Prenatal genetic testing is available for those patients with an identified genetic defect using pregestational diagno

Federal Support for Family Planning Clinics Associated with Dramatic Gains in Cervical Cancer Screening.

BACKGROUND: Since the 1990s, policymakers have successfully increased cervical cancer screening through federal and state public policies. However, the most dramatic gains in Pap smear use occurred in the 1960's and 70's, during the establishment of federal support for family planning clinics through the War on Poverty and Title X. This study estimated the effect of this support on cervical cancer screening, and quantified its role in dramatic increases in Pap smear use. METHODS: Using a natural experiment in the timing and receipt of federal family planning grants, the screening behavior of

Improved access to early infant diagnosis is a critical part of a child-centric prevention of mother-to-child transmission agenda.

BACKGROUND: Prevention-of-mother-to-child-transmission (PMTCT) programs have made it possible to achieve dramatic reductions in the rate of vertical HIV transmission. However, high attrition, particularly after delivery, has limited the impact of these interventions for HIV-exposed infants who remain at risk through the end of breastfeeding. DESIGN AND METHODS: A review of current literature on early infant diagnosis (EID) testing and country experience in low-and middle-income countries. RESULTS: While PMTCT programs report reduced rates of infection among infants tested at 2 months of age,

The Interplay of Perceived Organizational Support and Perceived Professional Benefits on Nurses' Flourishing: A Network Analysis.

AIM: Using network analysis to examine the relationships between nurses' perceived organizational support, perceived professional benefits, and flourishing. BACKGROUND: Nurses work in high-pressure and high-risk environments over the long term, making their mental health of paramount importance. Flourishing, as a more comprehensive and objective new indicator for assessing mental health, has not been fully explored in terms of its complex interactions with perceived organizational support and perceived professional benefits. METHODS: A total of 586 nurses were recruited for this study. An on

Organization Communication Factors and Abnormal Mammogram Follow-up: a Qualitative Study Among Ethnically Diverse Women Across Three Healthcare Systems.

BACKGROUND: Regular mammogram screening for eligible average risk women has been associated with early detection and reduction of cancer morbidity and mortality. Delayed follow-up and resolution of abnormal mammograms limit early detection efforts and can cause psychological distress and anxiety. OBJECTIVE: The goal of this study was to gain insight from women's narratives into how organizational factors related to communication and coordination of care facilitate or hinder timely follow-up for abnormal mammogram results. DESIGN: We conducted 61 qualitative in-person interviews with women fr

A Cluster Analysis of University Commuters: Attitudes, Personal Norms and Constraints, and Travel Satisfaction.

Higher education institutions are recognised as settings where the community's awareness of sustainable mobility can be strengthened or reshaped. The first objective of the present study was to identify groups of commuters based on their modal choice in a large higher education institution in Italy. The second objective was to compare the groups on socio-demographic and psychosocial variables, specifically attitudes, personal norms, personal constraints, and travel satisfaction. The cluster analysis revealed five different types of commuters: car-oriented, two-wheeled urban users, pedestrians,

Avoiding "shiny objects" and heeding "red flags": a sensemaking framework for emerging technology procurement.

OBJECTIVES: Health systems can be vulnerable to "shiny object syndrome," adopting unneeded emerging technologies (ETs), or may ignore "red flags" if they rely on linear health technology procurement processes that exclude the influence of information exchanges between the health system, vendors, and industry partners. METHODS: We conduct a post-hoc comparison of 2 procured communication-related clinical workflow ETs and examine ET-related IT Committee meetings within an academic medical center. We apply sensemaking theory, highlighting examples where sense-giving, sense-demanding, and sense-b

Variability and Innovation in Faculty Development Models in Emergency Medicine.

BACKGROUND: Ongoing faculty development is essential in academic emergency medicine (EM) to ensure physicians remain clinically competent, educationally effective, and aligned with evolving healthcare demands. Despite this necessity, approaches to faculty development vary widely in structure, participation expectations, and underlying educational philosophy. OBJECTIVES: This paper explores the spectrum of faculty development models, including voluntary participation, asynchronous learning, and institutional mandates, examining how each aligns with learning theory and motivational frameworks s

Caregiving and developmental factors differentiating young at-risk urban children showing resilient versus stress-affected outcomes: a replication and extension.

This study tested hypotheses from an organizational-developmental model for childhood resilience. In this model resilience reflects a child's mastery of age-salient objectives, in the face of substantial adversity, by drawing on internal and external resources that enhance processes of adaptation specific to each developmental stage. Interviews were conducted with parents of 122 7- to 9-year-old urban children exposed to multiple risk factors, 69 classified as resilient and 53 as maladjusted. Consistent with predictions generated by the model: (1) characteristics of a child's caregiving system

[Supportive care for survivors].

Advances in early detection and treatment have pushed in a few decades the management of cancers from a management model of the end of life to the management of a chronic disease. This evolution has accelerated the development of supportive care in two directions, firstly towards the best possible support to the end of life in advanced cancer patients (palliative care) and secondly to the limitation of treatment toxicities, the prevention of relapse and the return to life as "normal" as possible (care for after cancer). If palliative care now has a legitimacy and a solid regulatory base, this
