The bibliometric analysis aimed to assess the bibliometric characteristics, current status and temporal trends in diagnostic evolutions related to infectious diseases caused by the genus Orientia and Rickettsia. Scrub typhus is the most common endemic vector-borne disease in Taiwan. We identified a total of 4,857 laboratory-confirmed cases during 2006-2016 with hyperendemic foci on offshore islands, including Penghu (778 cases, 16.0%) and Kinmen (716 cases, 14.7%), and eastern Taiwan, including Taitung (628 cases, 12.9%) and Hualien (508 cases, 10.5%). Tsutsugamushi disease is an acute infectious rickettsial disease caused by the intracellular parasite Orientia tsutsugamushi. Due to its variety of clinical signs, this disease is often misdiagnosed. Transmission of Rickettsia tsutsugamushi (Rt) from a rodent to trombiculid mites was studied. Wild rodents, Apodemus speciosus, were subcutaneously inoculated with Karp strain of Rt, and larval trombiculid mites were released on the ear lobes for feeding, 10 and 20 days after infection. Scrub typhus is a serious public health problem in the Asia Pacific region. Early diagnosis and treatment can prevent complications and mortality. Splenomegaly in isolated scrub typhus is mostly mild or subclinical and rarely massive. We report an adolescent boy with scrub typhus presenting with fever, massive splenomegaly and severe anemia. Clinicians should be aware of atypical presentations of scrub typhus, which is easily treatable with minimal investigations.

OBJECTIVE: We studied the clinical phenotypes and prevalent genotypes of Orientia tsutsugamushi in our area using indirect immunofluorescence assay (IFA).
METHODS: We prospectively screened all febrile children presenting to our hospital over three years.
