Opening base wedge osteotomy of the first metatarsal utilizing rigid external fixation.

The authors describe their operative approach to metatarsus primus adductus deformity when present in conjunction with a congenitally short first metatarsal. Hallux abducto valgus correction in this clinical setting has traditionally advocated use of a crescentic or opening wedge osteotomy in order to prevent further shortening of the first metatarsal. Fixation of opening base wedge osteotomies has been fraught with problems in the past, with possibilities for displacement or eventual loss of initial correction. The authors have found the mini-Hoffmann external fixation device useful for this

Follow-up study of medial facetectomies and posterolateral fusion with instrumentation in unstable degenerative spondylolisthesis.

From 1978 to 1983, 54 consecutive patients with unstable degenerative spondylolisthesis were treated by medial facetectomies and posterolateral fusion with combined distraction and compression rod instrumentation. The average period of follow-up observation was 30 months. Twenty-five patients had a combination of disc herniation or instability, excluding olisthesis. Single-level fusion was performed in 29 patients, two-level in 23, and three-level in two. Preoperative low-back pain in 87.0% and sciatica in 66.7% were reduced to 7.5% and 5.6%, respectively, at follow-up treatment. Preoperative

Lumbosacral spinal fusion with Knodt distraction rods.

Knodt distraction rods have been used in conjunction with lumbar spinal decompression and/or fusion to open the intervertebral foramen. A review was undertaken to evaluate the efficacy of the Knodt Rods in 24 patients with multiple level foramenal canal stenosis. All 24 patients were treated by wide decompressive laminectomy, foramenotomies and bilateral-lateral fusion. One half of those 24 patients were treated with supplemental Knodt Rods and the other half was treated without Knodt Rods. Pre- and post-operative radiologic and clinical results were compared between the two groups. Although t

Acrylic stabilization of the cervical spine for neoplastic disease: evolution of a technique for vertebral body replacement.

The resection of collapsed cervical vertebral bodies affected by malignant lytic lesions often requires innovative methods for stabilization. Over the years, immediate stabilization of the cervical spine has been achieved with acrylic bridges, preformed metallic placement bodies, fibular strut grafts, and other aids. Because acrylic bonds poorly to adjacent bony structures, techniques to assure the adequate fixation of an acrylic bridge between resected vertebral bodies are needed. The possible progression of disease in adjacent bone prompts us to maximize the amount of internal fixation with

Two-stage resection and spinal stabilization for aneurysmal bone cyst. A report of two cases.

Two cases of aneurysmal bone cyst of the dorsal lumbar spine are presented with four- and seven-year follow-up periods. The lesion in each case involved the vertebral bodies and posterior elements and extended to contiguous bony structures. Both anterior and posterior staged procedures were required for maximal excision, rendering the spine extremely unstable at the involved levels. Anterior strut grafting with autogenous iliac bone was done during the first stage after the vertebral bodies had been curetted. Three weeks later the remaining dorsal elements and associated tumor were removed thr

Severe lumbosacral spondylolisthesis in adolescents and children. Reduction and staged circumferential fusion.

Using a new surgical regimen, fourteen patients with lumbosacral spondylolisthesis and more than 50 per cent slipping were treated by reduction of the slip with two Harrington distraction rods extending from the first lumbar laminae to the sacral alae and bilateral posterolateral fusion from the fourth lumbar to the second sacral segment. Then, at a second procedure, thirteen had an anterior lumbosacral fusion using two bicortical wedge-shaped iliac grafts. The distraction rods were removed six to twelve months later. At follow-up, correction of the slips ranged from 70 to 100 per cent. In fou

Rotatory subluxation: experience from the Hospital for Sick Children.

OBJECT: Diagnosis and management of atlantoaxial rotatory subluxation (AARS) is challenging because of its variability in clinical presentation. Although several treatment modalities have been employed, there remains no consensus on the most appropriate therapy. The authors explore this issue in their 9-year series on AARS. METHODS: Records of patients diagnosed radiologically and clinically with AARS between May 2001 and March 2010 were retrospectively reviewed. Of 40 patients identified, 24 were male and were on average 8.5 years of age (range 15 months-16 years). Causes of AARS included tr

[Progress in diagnosis and treatment of distal tibiofibular syndesmosis injury].

OBJECTIVE: To review the progress in the diagnosis and treatment of distal tibiofibular syndesmosis injury. METHODS: Different kinds of documents were widely collected, current developments of the diagnosis and treatment of distal tibiofibular syndesmosis injury were summarized. RESULTS: The disease history (damage mechanism), clinical examination, and imaging examination (X-ray, CT scan, and MRI) can assist the diagnosis of distal tibiofibular syndesmosis injury. Patients with unstable distal tibiofibular syndesmosis injury needs active surgery treatment, and the principle is anatomical red

[Design and experiment of the pedicle aiming device for the cervical spine].

OBJECTIVE: To probe the accuracy and safety of using an aiming device in the transpedicular fixation of cervical spine. METHODS: Eight cervical specimens were obtained. We used the computed tomography (CT) to scan C3 to C7, measured the medial angle of the pedicle, and determined the location of the pedicle projecting on the articular process. Then we took the oblique X-ray film, measured the cephalic/caudal angle of the pedicle, and determined the location of the pedicle projecting on the articular process. All the specimens were equally divided into two groups. Screws of 2.8 mm x 30 mm, wer

Four-part displaced proximal humeral fractures: operative treatment using Kirschner wires and a tension band.

Thirty-five patients with four-part displaced proximal humeral fractures and fracture-dislocations were retrospectively reviewed from the years 1977-1990. Thirty-three patients were available for study. All fractures were treated by open reduction and internal fixation allowing early motion. Surgical technique was as atraumatic as possible with the goal of restoring normal anatomy of the proximal humerus with a minimal fixation system. The fixation device consisted of two modified Kirschner wires introduced through the tuberosities and reinforced by a tension band wiring. Neer's criteria were

Open fixation of flail chest after blunt trauma.

Progressive left chest volume loss developed in a patient with severe flail chest despite reasonable oxygenation without intubation. Because of this chest volume loss, pain, and shortness of breath, she underwent open chest wall repair using multiple metallic struts. Rapid recovery ensued, despite a perforated duodenal ulcer on postoperative day 1. Benefits of open fixation of severe flail chest are clearly demonstrated and should be considered instead of prolonged ventilation or supportive care alone for select patients.

[Surgical treatment of avulsion fractures of the intercondylar eminence using ridged crossed trans-epiphyseal Kirschner wires].

Four patients with avulsion fractures of the intercondylar eminence were treated osteosynthetically by means of trans-epiphysial ridged crossed Kirschner wires at the emergency surgical clinic of the Solingen City Hospital in the period May 1984 to August 1985. In the case of two patients, who could be follow-up examinated, we found excellent results. The described procedure has proven to be technically simple and lends itself among other things to the secure feastening of flat, predominantly cartilaginous bearing fragments.

Anterior spine stabilization and decompression for thoracolumbar injuries.

In a series of patients with thoracolumbar spine injuries, anterior spinal canal decompression resulted in better neurologic recovery than did previously reported posterior instrumentation or nonoperative treatment. The technique allows stabilization over a much shorter segment of the spine than posterior instrumentation and therefore is indicated for fractures at L2 and below and in all patients with burst fractures and neurologic compromise.
