Carpal fractures excluding the scaphoid.

Although carpal fractures other than of the scaphoid are uncommon, vigilance in diagnosing these potentially serious fractures is paramount to early and effective treatment. Physical examination and standard plain radiographs may reveal only subtle findings. Use of special radiographic views and computed tomography may help elucidate the diagnosis. Treatment is particular to each fracture. Nondisplaced fractures should be treated nonoperatively. For intra-articular carpal bone fractures, virtually any amount of displacement is unacceptable and requires reduction and fixation. This article orga

Multilevel cervical spondylosis.

Multilevel cervical spondylosis is a common disorder encountered by most actively practicing spine surgeons. Patients can present with a combination of complaints, including mechanical neck pain, radiculopathy, and myelopathy. A comprehensive evaluation is required for appropriate diagnosis and treatment. Accurate interpretation of imaging techniques, including plain x-rays, CT, and MRI, is necessary to formulate the optimal surgical strategy. Key points to address during construction of the operative plan include the direction of approach, the extent of the decompression, and the type of stab

Management of elbow osteoarthritis.

Primary osteoarthritis of the elbow is characterized by painful stiffness, mechanical symptoms, and the presence of hypertrophic osteophytes. Preservation of the joint space is common and may account for the good results that are usually achieved with nonoperative treatment and nonprosthetic arthroplasty. Elbow osteoarthritis typically affects middle-aged men who engage in strenuous manual activity. Open or arthroscopic capsular release and removal of impinging osteophytes are the primary surgical treatment options. The relative sparing of joint cartilage makes elbow osteoarthritis unique in t

Spondylolysis and spondylolisthesis in children and adolescents: II. Surgical management.

Surgical management is indicated for children and adolescents with spondylolysis and low-grade spondylolisthesis (< or =50% slip) who fail to respond to nonsurgical measures. In situ posterolateral L5 to S1 fusion is the best option for those with a low-grade slip secondary to L5 pars defects or dysplastic spondylolisthesis at the lumbosacral junction. Pars repair is reserved for patients with symptomatic spondylolysis and low-grade, mobile spondylolisthesis with pars defects cephalad to L5 and for those with multiple-level defects. Screw repair of the pars defect, wiring transverse process to

Arthrogryposis multiplex congenita.

Since much confusion exists regarding arthrogryposis multiplex congenita (AMC), the President of the IFSSH commissioned the AMC Committee to compile a report on the various aspects of this condition. This report discusses all the facets of AMC, including definition, terminology, dermographics, aetiology, classification, clinical features, management, prognosis and further studies. Manipulation of the deformities starting soon after birth improves the range of motion, which, if surgery needs to be done, makes the operation less extensive. Sometimes surgery may not even be necessary. Early one-s

Surgical management of wrist and finger deformity.

The surgical results of upper extremity intervention have been shown to improve hand function from paperweight/passive assist function to active assist function. Although children with cerebral palsy commonly have a sensibility deficiency in conjunction with their motor deficiency, several recent studies have disproved the previous doctrine that hand surgery should not be performed on children with sensibility deficiencies. The author's report of 134 children treated surgically showed that preoperatively 50% had impaired two-point discrimination and 75% had impaired stereognosis: impaired sens

Spinal tuberculosis.

Spinal tuberculosis is an important and potentially crippling disease, but if recognized early and treated energetically it can be brought under control, often with very gratifying results. Knowledge of the exact bacteriology and sensitivity pattern of the organisms involved can be very helpful indeed, and should be sought where possible.

[From heparin to synthetic antithrombotic drugs].

Low molecular weight heparins (LMWH) have become the reference drugs for the prevention and treatment of venous thromboembolism. Among the newer antithrombotic agents, the pentasaccharide is the most advanced and promising alternative of low molecular weight heparins. The pentasaccharide is a synthetic and specific inhibitor of activated coagulation factor X (factor Xa). The pentasaccharide is identical to the active sequence of heparin which is present in about a third of commercially available heparin mixtures. The pentasaccharide binds specifically to antithrombin III (ATIII) thus potentiat

Surgery should not hurt: A look into the ergonomics of orthopedics.

Orthopedic surgery is among the most physically demanding medical specialties, exposing surgeons to prolonged static postures, repetitive high-force movements, and heavy equipment that significantly increase the risk of musculoskeletal injury. Historical perspectives reveal that ergonomic principles have been recognized since antiquity, yet modern surgical practice continues to neglect them, resulting in high rates of pain, disability, and even premature retirement among orthopedic surgeons. Recent survey data demonstrate that musculoskeletal disorders affect surgeons across all subspecialties

Audit of VTE prophylaxis prescribing preferences among orthopaedic consultants in Irish orthopaedic trauma centres.

BACKGROUND: Thromboembolic complications are common in trauma orthopaedic practise. Despite extensive research, there remain a number of unanswered questions regarding the use of thromboprophylaxis. AIMS: To establish the current practise among Irish consultant orthopaedic surgeons regarding thromboprophylaxis. METHODS: A confidential online questionnaire regarding VTE prophylaxis was circulated to all consultant orthopaedic surgeons in training hospitals in the Republic of Ireland. Questions investigated surgeon awareness of local/national guidelines, inpatient and outpatient prescribing pr

Operative experience in paediatric orthopaedics in UK trainees achieving a Certificate of Completion of Training in trauma and orthopaedic surgery: A descriptive analysis of national eLogbook data.

INTRODUCTION: The Trauma and Orthopaedic curriculum set by the Joint Committee on Surgical Training (JCST) requires that consultant orthopaedic surgeons must have sufficient experience managing children's orthopaedic conditions. In this study, our objective was to describe the paediatric operative exposure of United Kingdom (UK)-trained orthopaedic registrars who obtain a Certificate of Completion of Training (CCT). METHODS: This was a national retrospective cohort study of UK trainees obtaining a CCT between 1 January 2018 and 1 January 2024. ELogbook data for 884 trainees were obtained from

Idiopathic Talipes Equinovarus: Current Concepts.

Idiopathic talipes equinovarus (clubfoot) is a musculoskeletal deformity that affects children worldwide with an incidence of about 0.2% in the United States. It is typically diagnosed at birth or by prenatal testing, and if untreated or undertreated, it can lead to progressive deformity and increasing dysfunction. Ideal treatment involves largely nonsurgical means shortly after birth, with surgery reserved for late-presenting or recurrent cases. As such, all orthopaedic surgeons should be able to recognize the clinical manifestations and general treatment pathway for clubfoot. This review aim

Past, present, and future of orthopedic surgery in hemophilia: looking to a world without bleeding and arthropathy in the near future.

INTRODUCTION: The purpose of this article is to review the literature on orthopedic surgery in people with hemophilia (PWH), to better understand its past and present and attempt to predict the future. AREAS COVERED: A Cochrane Library and PubMed (MEDLINE) search of studies related to orthopedic surgery in PWH was analyzed. A downward trend in orthopedic surgery in PWH has been observed. Patients on continuous prophylaxis are less likely to require orthopedic surgery. In patients with hemophilia, the risk of major bleeding remains high after total joint arthroplasty, even with factor replacem

An Approach to the Management of Severe Clubfoot Deformities on Global Humanitarian Programs: The Role of Talectomy.

The approach to treatment of severe untreated or recurrent congenital talipes equinovarus deformities is very different in the world where patients are mobile, have access to repeated return visits for follow-up treatment, and where more sophisticated options for gradual correction with external fixation are available. For treatment, talectomy may be the only option to treat certain neglected clubfoot deformities during humanitarian programs and it may still have to be used as a salvage procedure used in modern foot surgery. Our extensive experience with these deformities has been on global hu
