Tibial sesamoid fracture in a softball player.

A single case of a tibial sesamoid fracture in a softball player is reported here. A review of the literature confirms that this is an unusual and difficult problem to treat in the athletic population given the significant loads placed on the sesamoids during athletic activity. In the case presented, conservative care was not effective, and the athlete underwent surgical excision of the fractured sesamoid. With use of a postoperative orthosis and cleat modification, surgical management was successful and allowed the athlete to return to her athletic endeavors without restrictions in 8 weeks.

Adult cavovarus foot.

Cavovarus foot deformity, which often results from an imbalance of muscle forces, is commonly caused by hereditary motor sensory neuropathies. Other causes are cerebral palsy, cerebral injury (stroke), anterior horn cell disease (spinal root injury), talar neck injury, and residual clubfoot. In cavovarus foot deformity, the relatively strong peroneus longus and tibialis posterior muscles cause a hindfoot varus and forefoot valgus (pronated) position. Hindfoot varus causes overload of the lateral border of the foot, resulting in ankle instability, peroneal tendinitis, and stress fracture. Degen

Management of disabilities associated with achondroplasia.

Achondroplasia is a skeletal dysplasia that manifests as short stature. Impairment and complications range over many disciplines including orthopedics, pediatrics, neurology, and otolaryngology. The major impairments of the extremities are short limbs, limited elbow and hip extension, and knee and leg deformities that can cause disabilities in arm function and locomotion. Hydrocephalus, a narrow foramen magnum, spinal deformity, and spinal canal stenosis can cause neurological problems, leading to disabilities in locomotion, communication, and learning. Malfunctions of the otolaryngeal system

The effect of a slider shoe on hemiplegic gait.

OBJECTIVE: To assess the effect of a slider shoe on the gait speed and energy efficiency of hemiplegic gait. DESIGN: A-B-A single-case design to compare walking with and without the slider shoe. Results were assessed graphically using the 2SD method. SETTING: Stroke unit of an NHS general hospital in the UK. SUBJECTS: Four acute stroke patients undergoing gait rehabilitation. INTERVENTION: Walking practice with and without a slider shoe worn over the real shoe of the weak leg. MAIN OUTCOME MEASURES: Gait speed (10-m walk test) and energy efficiency (Physiological Cost Index). RESULTS: Al

Over-the-counter foot remedies.

Several effective and inexpensive over-the-counter treatments are available for minor but troubling foot problems. In most cases, one week of therapy with topical terbinafine is effective for interdigital tinea pedis. Treatment of plantar warts with 17 percent salicylic acid with lactic acid in a collodion base is as effective as cryotherapy, but treatment must be sustained for several months. Toe sleeves and toe spacers can relieve pain from hard or soft corns. Metatarsal pads can relieve the pressure associated with plantar keratoses. Heel cups often can relieve pain caused by age-related th

Workplace disability management in postpolio syndrome.

Postpolio syndrome (PPS) is generally defined as a clinical syndrome of new weakness, fatigue, and pain in individuals who have previously recovered from acute paralytic poliomyelitis. These new problems may lead to loss of employment as well as new deficits in instrumental activities in daily living (cleaning, washing, shopping, transportation, etc.), walking, climbing stairs, and personal assistance. We presented three cases of PPS with working disabilities in Japan, and stated the issues confronted with. Particularly at the workplace, PPS individuals need special supports from both rehabili

Posterior tibial tendon dysfunction.

Posterior tibial tendon dysfunction is the most common cause of acquired flatfoot deformity in adults. Although this term suggests pathology involving only the posterior tibial tendon, the disorder includes a spectrum of pathologic changes involving associated tendon, ligament, and joint structures of the ankle, hindfoot, and midfoot. Early recognition and treatment is the key to prevention of the debilitating, long-term consequences of this disorder. Conservative care is possible in the earliest stages, whereas surgical reconstruction and eventually arthrodeses become necessary in the latter

Biomechanical analysis of four supports for the subluxed hemiparetic shoulder.

External support systems, such as slings and lapboards, may reduce shoulder subluxation in individuals with hemiplegia. However there is controversy among occupational therapists concerning the most appropriate method to support the affected arm. The purpose of the present paper is to report the biomechanical analysis of four support systems; two shoulder support systems, a Bobath axillary roll, and a laptray. Two dimensional static biomechanical analyses determine the mechanical characteristics of each of these four support systems. The results of the analyses demonstrate the magnitude of the

Ambulation with the reciprocating-gait orthosis. Experience in 15 children with myelomeningocele or paraplegia.

We reviewed 15 children with spina bifida or paraplegia who have used a reciprocating-gait orthosis between 1985 and 1995. All were nonfunctional ambulators. The level of the spinal lesions ranged from Th10 to L3. The mean age of fitting the orthosis was 5 years. 8 children have stopped using it at an average age of 10 years. The maximum ambulation level with the orthosis was a community ambulator in 4 children, household ambulator in 9 children and 2 remained nonfunctional ambulators. The average daily use was 6 hours by community ambulators to 0.5 hours nonfunctional ambulators. Bilateral di

Reciprocating orthoses for children with myelomeningocele. A comparison of two types.

Since 1987, 22 children with myelomeningocele have been fitted with reciprocating orthoses. The level of the spinal lesions ranged from T10 to L4 and 13 had associated spinal deformities. Twelve of the patients currently use a Reciprocating Gait Orthosis, seven use a Hip Guidance Orthosis or Parawalker, one has progressed to a Knee Ankle Foot Orthosis, one has died and one has been lost to follow-up. The reciprocating orthoses are worn for a mean of 3.5 hours per day (1 to 6.5); daily usage by girls is almost twice that by boys. The mean daily usage by community walkers is 4.2 hours (13 childr

Blount disease (idiopathic tibia vara).

Blount disease or idiopathic tibia vara is a developmental deformity of the proximal tibial medial physis that produces a varus deformity of the knee. It is classified by early or late onset. The etiology is not entirely clear but is presumed to be related to an abnormal compression on the medial aspect of the proximal tibial physis. Early diagnosis is important for a better prognosis. In general, the later the onset and the greater the progression of the deformity, the increased likelihood of articular changes. Orthotics are recommended as the first type of treatment. If progression occurs or

Painful spondylolisthesis in a two-year-old child.

Spondylolisthesis is considered one of the common causes of back pain in children. There are far fewer cases reported in children under the age of 4 years. However, all patients described were diagnosed by radiographic examination in a workup for other congenital or musculoskeletal abnormalities. This report describes a 2-year-old girl who presented with low back pain and was subsequently diagnosed with spondylolisthesis.

The Scottish Rite abduction orthosis for the treatment of Legg-Perthes disease. A radiographic analysis.

We evaluated a consecutive series of thirty-four patients who had had Legg-Perthes disease to determine the results of treatment with the Scottish Rite weight-bearing abduction orthosis. All patients had been older than six years at the time of the onset of the symptoms, had been in the fragmentation phase of the disease when they were first seen, and had had Catterall group-III or IV involvement. Although comparison of this series with those reported previously is difficult due to the variability of the presentation of data in the literature, this orthosis does not appear to offer an advantag

Conservative treatment of genu valgus and varum with medial/lateral heel wedges.

Symptomatic bowlegs and knock-knees are common in the aging person and aging athlete secondary to knee trauma. Diagnosis is made by observation and in mild degrees by a standing alignment x-ray that shows narrowing of the joint surface on the appropriate side. The patients complained of pain on the convexity side of the deformity. Treatment consisted of a simple exercise program, non-steroidal anti-inflammatory drugs and appropriate 1/8-inch wedges on the lateral side of the heel for bowlegs or on the medial side of the heel for knock-knees. The effect of a simple heel wedge formed the basis f
