OBJECTIVE: ObjectiveThis study aimed to explore the role of the miR-21/Mef2c/Sost pathway in the pathogenesis of diabetic osteoporosis (DOP) and to investigate whether metformin ameliorates DOP by regulating this signaling pathway.
METHODS: We compared bone mineral density (BMD), bone turnover biomarkers, and bone miR-21 expression levels between elderly female patients with and without diabetes. Diabetic mice and high-glucose-treated MLO-Y4 osteocytes were used to explore the involvement of the miR-21/Mef2c/Sost pathway in DOP. miR-21 mice and MLO-Y4 osteocytes transfected with miR-21 mimics…

Notch signaling plays a fundamental role in skeletal physiology, and activation of NOTCH3 in osteoblasts/osteocytes impairs corticalization, causes cortical osteopenia and suppresses cancellous bone remodeling. To address possible mechanisms responsible, we explored transcriptome profiles in osteocytes and femoral bone from 1-month-old male C57BL/6 mice following activation of NOTCH3 in Dmp1-expressing cells. We used a NOTCH3 gain-of-function mouse line expressing the NOTCH3 intracellular domain (N3ICD) from the Rosa26 locus (R26-N3ICD mice) following the deletion of a loxP flanked STOP casset…

The present study was designed to investigate mRNA expression of matrix metalloproteinase-8 (MMP-8) and MMP-13 in forming periodontium during tooth eruption in the rat. RT-PCR for the decalcified paraffin sections indicated expression of MMP-8 and MMP-13 in the periodontal tissues. In situ hydridization demonstrated expression of MMP-8 in osteoblasts, osteocytes, periodontal ligament cells, cementoblasts, and cementocytes along with collagen types I and III. In contrast, transcripts of MMP-13 were confined to a small population of osteoblasts and osteocytes in alveolar bone. The results sugges…

Inactivating mutations that lead to loss of heterozygosity within the HRPT2/Cdc73 gene are directly linked to the development of primary hyperparathyroidism, parathyroid adenomas, and ossifying fibromas of the jaw (HPT-JT). The protein product of the Cdc73 gene, parafibromin, is a core member of the polymerase-associated factors (PAF) complex, which coordinates epigenetic modifiers and transcriptional machinery to control gene expression. We conditionally deleted Cdc73 within mesenchymal progenitors or within mature osteoblasts and osteocytes to determine the consequences of parafibromin loss …

Emerging studies are showing the effect of osteocytes on orthodontic tooth movement. Nitric oxide (NO), produced by osteocytes, is an important regulator of bone response to loading and has been shown to mediate osteoclast activity. iNOS (which produces NO) has been shown to mediate inflammation-induce …

Hypophosphatemic rickets is caused by renal phosphate wasting that is most commonly due to X-linked dominant mutations in PHEX. PHEX mutations cause hypophosphatemia indirectly, through the increased expression of fibroblast growth factor 23 (FGF23) by osteocytes. FGF23 decreases renal phosphate reabsorption and thereby increases phosphate excretion. The lack of phosphate leads to a mineralization defect at the level of growth plates (rickets), bone tissue (osteomalacia), and teeth, where the defect facilitates the formation of abscesses. The bone tissue immediately adjacent to osteocytes ofte…

Implant-associated bone infection involves a complex interplay between pathogenic stimuli and host cell responses, yet analysis in preclinical models has typically relied on qualitative or semi-quantitative measures. We aimed to establish a quantified evaluation framework to define host-pathogen relationships in a preclinical implant infection model. Staphylococcus aureus-coated stainless-steel implants were inserted trans-cortically in mouse tibiae and bone changes recorded longitudinally by in vivo micro-CT. An automated segmentation task list was developed to independently isolate and quant

Osteogenesis imperfecta (OI) is associated with an increased risk of early-onset osteoarthritis (OA); however, it remains unclear whether this risk is driven by underlying osteochondral alterations. Here, we aimed to characterize the osteochondral phenotype in OI to identify osteochondral features that may be relevant to increased OA susceptibility. We collected bilateral tibial plateaus from one adult with classical OI (COL1A2) who underwent bilateral total knee arthroplasty, and a femoral head from a second adult (COL1A1) who underwent total hip arthroplasty. The specimens were analyzed usin

Bone renews itself and changes shape throughout life to account for the changing needs of the body; this requires co-ordinated activities of bone resorbing cells (osteoclasts), bone forming cells (osteoblasts) and bone's internal cellular network (osteocytes). This review focuses on paracrine signaling by the IL-6 family of cytokines between bone cells, bone marrow, and skeletal muscle in normal physiology and in pathological states where their levels may be locally or systemically elevated. These functions include the support of osteoclast formation by osteoblast lineage cells in response to …

Bone tissue is comprised of three primary cell types: osteoclasts, the bone resorbing cells; osteoblasts, the bone forming cells; and osteocytes, the cell type involved in mechanotransduction. Osteoclasts are multinucleated cells of hematopoietic origin that degrade bone, while osteoblasts arise from mesenchymal stem cells to synthesize bone matrix. Osteocytes, differentiated from osteoblasts, sense mechanical strain, coordinate with osteoclasts and osteoblasts, and regulate bone remodeling. Cytokines produced during immune responses to infections and autoimmune diseases significantly influenc…

Thirteen patients with scaphoid fractures and four patients with Kienbock's disease of the lunate underwent magnetic resonance imaging (MRI) prior to surgery. A total of 28 specimens had MR-histologic correlation. Biopsy specimens obtained by curettage provided pathologic correlation. MRI proved accurate in prospective evaluation of bone viability for both scaphoid fractures and Kienbock's disease. Normal marrow signal was shown to correlate with the presence of osteoid and osteocytes on light microscopy and a surface layer of fluorescence reflecting tetracycline uptake in viable bone. Decreas…

Sclerostin, mainly produced by osteocytes, is now considered a major regulator of bone formation. Identified from patients with a low bone mass, sclerostin inhibits the Wnt pathway by binding to LRP5/6 and subsequently increases bone formation. Sclerostin may also play a role in the mediation of systemic and local factors such as calcitriol, PTH, glucocorticoids and tumor necrosis factor-alpha. Circulating sclerostin levels increase with age and with the decline of kidney function. However, they are surprisingly higher in patients with a high bone mineral density, suggesting that sclerostin ma…

Studies during the last decade demonstrated that apoptosis is as important as mitosis for the growth and maintenance of the skeleton and provided information on the significance and molecular regulation of apoptosis of bone cells. It is now known that: (1) all osteoclasts die by apoptosis after completing a bone resorption cycle; (2) the majority of osteoblasts also die, whereas the remainder become lining cells or osteocytes; and (3) osteocytes, although long-living cells, also can die prematurely. Furthermore, mounting evidence indicates that systemic hormones, local growth factors, cytokine…

Estrogen is the major hormonal regulator of bone metabolism in women and men. Therefore, there is considerable interest in unraveling the pathways by which estrogen exerts its protective effects on bone. Although the major consequence of the loss of estrogen is an increase in bone resorption, estrogen deficiency is associated with a gap between bone resorption and formation, indicating that estrogen is also important for maintaining bone formation at the cellular level. Direct estrogen effects on osteocytes, osteoclasts, and osteoblasts lead to inhibition of bone remodeling, decreased bone res…

Case reports since 1957 implicate corticosteroids in excess of physiologic requirements as a cause of nontraumatic osteonecrosis. Both laboratory and clinical studies demonstrate marked alterations in lipid metabolism with hyperlipemia, fatty liver and systemic fat embolism. Intra-arterial infusion of fat produces embolic vascular obstruction, focal marrow necrosis and osteocytic death in the femoral head of the rabbit. Induced hypercortisonism in rabbits produces severe hyperlipemia, fatty liver, systemic fat emboli, terminal vascular obstruction in bone and associated areas of osteocytic dea

Increased bone turnover may produce a disturbance in bone structure which may result in fracture. In cortical bone, both reduction in turnover and increase in hip bone mineral density (BMD) may be necessary to decrease hip fracture risk and may require relatively greater proportionate changes than for trabecular bone. It should also be noted that increased porosity produces disproportionate reduction in bone strength, and studies have shown that increased cortical porosity and decreased cortical thickness are associated with hip fracture. Continued studies for determining the causes of bone st

BACKGROUND: Aseptic osteonecrosis is characterized by a complete death of the tissue (necrosis), more specifically, an ischemic necrosis of the lamellar bone tissue. The denotation aseptic refers to causal pathogenesis; therefore, it is not a matter of an infectious, septic-induced bone necrosis as in the case of acute infectious osteomyelitis. Formal pathogenesis leads to either (1) a hypoperfusion of the lamellar bone in the sense of an ischemic necrosis or (2) to directly induced damage of osteocytes and osteoblasts, which causes aseptic osteonecrosis.
CAUSE: The causes of hypoperfusion/is…
