Construction and Immune Strategy Optimization of a Vaccine Strain for Influenza A (H5N8) Subtype.

Multiple subtypes of avian influenza virus (AIV), including H5N1, H5N6, and H5N8 viruses, are currently co-circulating in wild birds and poultry and causing sporadic human infections. Vaccine development is essential for pandemic preparedness. In this study, we constructed a candidate vaccine virus (CVV) using reverse genetics (RG) based on the sequence of the first human-infected H5N8 subtype AIV, A/Astrakhan/3212/2020 (H5N8). We evaluated the immunogenicity of the rH5N8/PR8 vaccine strain in combination with Alum, ISA51, and MF59 adjuvants, and we optimized immunization strategies including …

Lessons Learned From Physiotherapists Working During the COVID-19 Pandemic: A Qualitative Approach.

This qualitative study aimed to explore the perceptions and experiences of physiotherapists related to their working situations amidst the COVID-19 pandemic in Spain. Between April and June 2020, we conducted 16 semi-structured personal interviews with physiotherapists from the public, private, and insurance sectors. Using thematic analysis, we identified three themes: legislative and regulatory policies, working conditions, and employment conditions. Physiotherapists raised concerns about COVID-19 policies, confusion, and inconsistency between regional and national guidelines. Private-sector

Computationally designed haemagglutinin with nanocage plug-and-display elicits pan-H5 influenza vaccine responses.

The increasing spread of highly pathogenic avian influenza (HPAI) A/H5 viruses poses a pandemic threat. Circulating clade 2.3.4.4b viruses have demonstrated rapid transcontinental dissemination, extensive reassortment, epizootic spread and potential sustained mammal-to-mammal transmission, signifying a heightened risk of becoming a human pathogen of high consequence. A broadly protective, future-proof vaccine against multiple clades of H5 influenza is urgently needed for pandemic preparedness. Here, we combine two novel vaccine technologies to generate a Digitally Immune Optimised and Selected…

Serological evidence of pandemic (H1N1) 2009 virus in pigs, West and Central Africa.

Besides birds, pigs are another important reservoir of influenza A viruses that can be transmitted to human, as highlighted by the emergence and spread of the pandemic (H1N1) virus (pdm/09) in 2009. Surveillance in pigs is therefore necessary for public health and influenza pandemic preparedness. Nevertheless, there is a serious lack of data on influenza in Africa, especially in swine. We therefore collected serum samples from pigs in Nigeria (2009, 2012) and Cameroon (2011) in which the presence of anti-influenza A neutralizing antibodies was investigated. Our serological survey suggests that…

Influenza virus-like particles as pandemic vaccines.

There is an urgent need to develop novel approaches for vaccination against emerging pathogenic avian influenza viruses as a priority for pandemic preparedness. Influenza virus-like particles (VLPs) have been suggested and developed as a new generation of non-egg-based cell culture-derived vaccine candidates against influenza infection. Influenza VLPs are formed by a self-assembly process incorporating structural proteins into budding particles composed of the hemagglutinin (HA), neuraminidase (NA) and M1 proteins, and may include additional influenza proteins such as M2. Animals vaccinated wi…

Avian influenza: a new pandemic threat?

In December 2003, the largest outbreak of highly pathogenic avian influenza H5N1 occurred among poultry in 8 Asian countries. A limited number of human H5N1 infections have been reported from Vietnam and Thailand, with a mortality rate approaching 70%. Deaths have occurred in otherwise healthy young individuals, which is reminiscent of the 1918 Spanish influenza pandemic. The main presenting features were fever, pneumonitis, lymphopenia, and diarrhea. Notably, sore throat, conjunctivitis, and coryza were absent. The H5N1 strains are resistant to amantadine and rimantadine but are susceptible t

Origins of pandemics: Why ecological disruption is central to global health security and the imperative for proactive prevention.

Pandemics are increasingly recognised as complex ecological and socio-economic phenomena rather than isolated biological events. The emergence of infectious diseases reflects dynamic interactions among humans, animals, and environmental systems, shaped by factors including biodiversity loss, land-use change, climate variability, urbanisation, and intensified human-wildlife contact. This viewpoint examines the evolving understanding of pandemic origins, highlighting the central role of ecological disruption and zoonotic spillover in shaping global health risks. The article further discusses the

WHO Headquarters Risk Communication and Community Engagement (RCCE) Response to COVID-19 - Leadership.

This article examines the Risk Communication and Community Engagement (RCCE) response from the World Health Organization headquarters in concert with its six regional offices and partners in response to the COVID-19 pandemic through two lenses. First, using the findings from the 2017 WHO Guideline for Emergency Risk Communication Policy and Practice, we compared the pre-pandemic recommendations against the realities of response. Second, because WHO was seen as the global leader for RCCE response, we also viewed the RCCE actions through six key crisis leadership functions. As with every major p

Roadmap: one anesthesiology department's strategic approach to COVID-19.

PURPOSE OF REVIEW: The COVID-19 pandemic has driven transformation in every aspect of the healthcare delivery system. The unpredictable onset and magnitude of COVID-19 infections resulted in wide gaps in preparedness for healthcare systems. The development of protocols to address both scarcity of resources and staff protection continues to be essential for risk mitigation. RECENT FINDINGS: The northeast region of the United States had a rapid early surge of COVID-19 infections leading to the exhaustion of critical care capacity. In addition, northeastern hospitals experienced decrease in elec

COVID-19 preparedness at health facilities and community service points serving people living with HIV in Sierra Leone.

After a decade of civil war and the 2014-2016 West African Ebola outbreak, Sierra Leone now faces the COVID-19 pandemic with a fragile health system. As was demonstrated during Ebola, preparedness is key to limiting a health crisis' spread and impact on health systems and ensuring continued care for vulnerable populations including people living with HIV (PLHIV). To assess COVID-19 preparedness and inform interventions to ensure continuity of HIV services at health facilities (HFs) and community service points (CSPs), we conducted site readiness assessments in Freetown, the epicenter of COVID-

Living Rapid Research Needs Appraisal (RRNA) for Priority Diseases Protocol.

Systematic reviews require time and resources to conduct that might not be available in a rapidly developing context such as during new and (re-) emerging infectious disease outbreaks. As part of the Pandemic-PACT program at the Pandemic Sciences Institute at the University of Oxford we have, in collaboration with Cochrane Response and Cochrane South Africa, developed a "living" Rapid Research Needs Appraisal (RRNA) methodology. The aim is to develop a protocol for rapidly and robustly identify key evidence gaps across pre-defined research domains to inform research prioritisation and coordina

The Implementation of Infection Prevention and Control Procedures in Primary Care During the COVID-19 Pandemic: A Qualitative Study of Nursing Roles.

Introduction: During the COVID-19 pandemic, primary care practices felt poorly supported by existing infection prevention and control (IPAC) guidelines, which focused primarily on acute care facilities. This issue was further complicated by insufficient provision of personal protective equipment in primary care settings, which limited clinic capacity and the ability of primary care to provide in-person services. Nurses play an integral role in the implementation of IPAC procedures and the provision of ongoing primary care during a health crisis; however there is limited literature related to n

Surveillance without silos: a One Health data imperative for avian influenza in West Africa.

Highly pathogenic avian influenza (HPAI) has circulated in West Africa since Nigeria's first human case in 2006, making the region a key entry point and dispersal hub for multiple virus lineages in sub-Saharan Africa. The 2.3.4.4b virus clade driving the global spread of avian influenza is now infecting mammals more often, a troubling sign that raises the risk of a future pandemic. At the same time, West Africa's ability to control HPAI is held back by fragmented surveillance systems. Animal, human and environmental health services operate in isolation, rely on incompatible data formats and ar
