Comparison of HRV analysis methods during orthostatic challenge: HRV with respiration or without?

Heart Rate Variability (HRV) analysis has become an important tool in assessing human Autonomic Nervous System (ANS) activity in recent years. Orthostatic challenge is one of the most common tests to detect ANS dysfunction. In this study we looked at the changes in ANS activity of normal subjects to orthostatic challenge and compared the results of 3 different HRV analysis methods: Time-Domain Methods, HRV spectral analysis without respiratory analysis (RA) and with RA. Although all three methods have indicated an increase in sympathetic activity and a decrease in parasympathetic activity from

Effect of parasympathetic blockade on ventilatory and cardiac depression induced by opioids.

We have previously shown that delta-opioid agonists decrease ventilation and heart rate. Because of these results and the known interactions between opioid and acetylcholine metabolism, we hypothesized that opioids induce cardiorespiratory changes via the parasympathetic nervous system. To test this hypothesis, we administered atropine sulfate (systemically) at maximal effect of D-Ala-D-Leu-enkephalin (DADLE; a preferential delta-opioid agonist), injected intracisternally, and examined its effect on cardiorespiratory function. All experiments were performed on chronically instrumented and cons…

Autonomic neurotransmission in cardiovascular regulation and pathophysiology.

The autonomic nervous system (ANS) is a central regulator of cardiovascular function, coordinating involuntary control of heart rate, vascular tone, and blood pressure through its sympathetic (SNS) and parasympathetic (PNS) subdivisions. The SNS mediates the "fight or flight" response via catecholamines, increasing heart rate, contractility, and vasoconstriction, whereas the PNS promotes restorative processes through acetylcholine, decreasing heart rate and enhancing vasodilation. Nitric oxide further modulates vascular tone and autonomic balance, serving as a key neuromodulator. Assessment of

Region-specific parasympathetic nerve remodeling in the left atrium contributes to creation of a vulnerable substrate for atrial fibrillation.

Atrial fibrillation (AF) is the most common heart rhythm disorder and a major cause of stroke. Unfortunately, current therapies for AF are suboptimal, largely because the molecular mechanisms underlying AF are poorly understood. Since the autonomic nervous system is thought to increase vulnerability to AF, we used a rapid atrial pacing (RAP) canine model to investigate the anatomic and electrophysiological characteristics of autonomic remodeling in different regions of the left atrium. RAP led to marked hypertrophy of parent nerve bundles in the posterior left atrium (PLA), resulting in a glob

Central control of cardiorespiratory interactions in fish.

Fish control the relative flow rates of water and blood over the gills in order to optimise respiratory gas exchange. As both flows are markedly pulsatile, close beat-to-beat relationships can be predicted. Cardiorespiratory interactions in fish are controlled primarily by activity in the parasympathetic nervous system that has its origin in cardiac vagal preganglionic neurons. Recordings of efferent activity in the cardiac vagus include units firing in respiration-related bursts. Bursts of electrical stimuli delivered peripherally to the cardiac vagus or centrally to respiratory branches of c…

Cardiac morphogenesis--recent research advances.

It has been demonstrated recently that a specific region of neural crest contributes cells to the septa of the outflow tract of the heart. Removal of this region of cardiac neural crest prior to migration from the neural fold results in persistent truncus arteriosus in chick embryos. Removal of other regions of cranial neural crest results in double outlet right ventricle. Since double outlet right ventricle is produced by manipulation of noncardiac neural crest, this malformation is thought to be an indirect rather than direct effect of neural crest ablation. The cranial neural crest forms th

Plethysmographic Peripheral Perfusion Index: Could It Be a New Vital Sign?

The plethysmographic peripheral perfusion index (PPI) is a very useful parameter with various emerging utilities in medical practice. The PPI represents the ratio between pulsatile and non-pulsatile portions in peripheral circulation and is mainly affected by two main determinants: cardiac output and balance between sympathetic and parasympathetic nervous systems. The PPI decreases in cases of sympathetic predominance and/or low cardiac output states; therefore, it is a useful predictor of patient outcomes in critical care units. The PPI could be a surrogate for cardiac output in tests for flu…

[The intermediate nerve and its place in the system of cranial nerves].

The intermediate nerve (IN) in embryogenesis of man, cat and white rat is formed similarly. From the common with the VIII cranial nerve anlage the geniculum node, the vestibular and cochlear ganglia are emerged. A separated root of the IN connects the geniculum node with the nuclei, situating in the CNS. From the geniculum node main branches of the IN get off: the greater petrosal nerve and the cord of the tympanum, its fibers in the temporal bone canal run in the facial nerve trunk. In the periphery the IN branches unite with other cranial nerves, make connections with branches of vegetative

[Autonomic neuropathy in reduced glucose tolerance].

33 subjects belonging to I.G.T. class according to N.D.D.G. criteria and controlled in our ambulatory, have been studied for the response to the tests usually employed for the investigation of parasympathetic and sympathetic cardiovascular innervation. We have found pathological values for the SL1 test and borderline values for SL2 and LS tests; on the contrary all the other tests presented normal values. The Authors conclude that signs of deterioration of cardiac and vascular innervation, especially regarding the parasympathetic nervous system, can be present also in the subjects showing a sm…

Paroxysmal atrial fibrillation developed during incomplete epidural anesthesia -A case report-.

Atrial fibrillation (AF) is the most common sustained tachyarrhythmia, and occurs in organic heart disease such as rheumatic, atherosclerotic and hypertensive heart disease. In recent studies, the sympathetic and parasympathetic nervous systems have been shown to have important roles in initiating paroxysmal AF. We report here a patient who developed paroxysmal AF that might be a result of an imbalance of the sympathetic-parasympathetic systems due to epidural anesthesia, and that was potentiated by pain with inadequate analgesia. A 69-year-old woman was scheduled for operation of a right-side…

Autonomic reactivity in relation to attachment and early adversity among foster children.

This study examined whether the quality of relationships with foster caregivers was associated with autonomic nervous system reactivity of children during separation and reunion with their foster caregiver. Moreover, effects of early adversity were examined in relation to attachment and autonomic nervous system reactivity. The sample included 60 children between 26 and 88 months of age, who participated with their primary foster caregivers in the Strange Situation. Respiratory sinus arrhythmia and preejection period were measured as indicators of parasympathetic and sympathetic nervous system

Effects of a facial nerve lesion on responses in forehead microvessels to conjunctival irritation and paced breathing.

To investigate parasympathetic influences on the forehead microvasculature, blood flow was monitored bilaterally in seven participants with a unilateral facial nerve lesion during conjunctival irritation with Schirmer's strips and while breathing at 0.15 Hz. Blood flow and slow-wave frequency increased on the intact side of the forehead during Schirmer's test but did not change on the denervated side. However, a 0.15 Hz vascular wave strengthened during paced breathing, particularly on the denervated side. These findings indicate that parasympathetic activity in the facial nerve increases fore

The cooperative phenomenon of autonomic nervous system in urine storage for Wistar rats.

The fractal dimension (FD) and spectral frequencies of physiological signals are two important indices in the study of physiological functions and dynamical diseases. The first index can be used to characterize the intensity and the second the rhythms of signals embedded in seemingly random data. Recent studies using both indices verified that synergic co-activations of bladder and external urethral sphincter (EUS) of Wistar rats were present during the voiding of urine. In this study, the primary aims were to (1) examine if the involved muscles in the lower urinary tract would be under simila

Behavioral-independent features of complex heartbeat dynamics.

We test whether the complexity of the cardiac interbeat interval time series is simply a consequence of the wide range of scales characterizing human behavior, especially physical activity, by analyzing data taken from healthy adult subjects under three conditions with controls: (i) a "constant routine" protocol where physical activity and postural changes are kept to a minimum, (ii) sympathetic blockade, and (iii) parasympathetic blockade. We find that when fluctuations in physical activity and other behavioral modifiers are minimized, a remarkable level of complexity of heartbeat dynamics re

Salivary total protein and experimental Coriolis sickness.

Symptomatic reporting has been used in quantifying the severity of motion sickness including Coriolis sickness. This study was designed to objectively examine if the dynamic changes of salivary protein concentration relate to the severity of Coriolis sickness. Healthy adults with normal vestibular function underwent a modified Coriolis Sickness Susceptibility Index (CSSI) test, utilizing a staircase profile. Salivary samples were obtained prior to stimulation, 15, 30, and 45 min following stimulus onset, and/or upon reaching "nausea-II" endpoint. Total protein concentration tended to rise over

Parasympathetic hyperresponsiveness and bradyarrhythmias during apnoea in hypertension.

Voluntary end-expiratory apnoea in a 23-year-old asymptomatic mild hypertensive patient consistently elicited bradyarrhythmias (complete heart block and sinus pause) and sympathetic activation to muscle blood vessels, indicating simultaneous sympathetic and parasympathetic activation during apnoea. The sympathetic bradyarrhythmic response to apnoea was potentiated by hypoxia and eliminated by atropine. Baroreflex activation also attenuated the bradycardic response to apnoea. A 43-year-old hypertensive patient with sleep apnoea also exhibited bradyarrhythmias (sinus arrest for up to 10 s) and a

[Electromyographic and manometric studies of stomach motility after vagotomy].

Vagotomy coincides not only with a reduction of the cephalic phase of secretion but also with a significant reduction of the motor activity of the stomach. Electrical and mechanical activity of the gastric wall after truncal vagotomy were verified in animal experiments (dog n = 5) by means of implanted electrodes and pressure transducers. Longlasting motility disturbances expressed as a disorganisation of the basic electrical rhythm occurred in the gastric wall. According to the pattern periods of tachygastria and tachyarrhythmia were identified. Those periods interrupted the regular fasted mo

[Evaluation of the autonomic innervation of the pupil].

The purpose of the study was to examine vegetative innervation of the pupil in different diseases associated and not associated with vegetative disorders. In addition to the ++clinico-neurological method, the main method of examination was to study the pupillary cycle (determination of the time of the pupillary cycle (TPC) with the aid of a slit lamp). Fifty healthy test subjects and 262 patients suffering from different somatic and neurologic diseases were examined. The patients were divided in 9 clinical groups: spinal amyotrophy, neural Charcot-Marie amyotrophy, Guillain-Barre syndrome, dia
