Perioperative Risk Factors and Long-Term Outcomes Using the Inferior Gluteal Artery Myocutaneous Flap to Reconstruct Complex Perineal Defects.

BACKGROUND: Radical pelvic extirpative surgery is associated with high perioperative morbidity. The inferior gluteal artery myocutaneous (IGAM) flap was introduced and subsequently refined to optimize patient results in this setting. This study evaluated perioperative factors, flap-specific complications, donor-site morbidity and long-term outcomes in patients undergoing IGAM reconstructions following pelvic exenteration or abdominoperineal resection. METHODS: Data were prospectively collected and reviewed for adult patients treated at the Peter MacCallum Cancer Centre, Melbourne, Australia b…

Technical report on the first clinical use of a bioabsorbable PGA spacer in HDR brachytherapy for recurrent cervical cancer.

In recurrent gynecologic malignancies following prior pelvic irradiation, definitive radiation therapy is often precluded by cumulative dose constraints to adjacent organs at risk (OARs), and patients may be left with only highly invasive surgical options such as total pelvic exenteration. While some institutions have explored displacement techniques such as artificial ascites or hyaluronic acid gel injection, these approaches are not widely adopted and frequently fail to ensure consistent and stable separation of OARs. We report the first clinical use of Neskeep®, a bioabsorbable polyglycolic…

Long-term outcomes analysis of flap-based perineal reconstruction.

BACKGROUND: High-risk patients undergoing abdominoperineal resection and pelvic exenteration may benefit from immediate flap reconstruction. However, there is currently no consensus on the ideal flap choice or patient for whom this is necessary. This study aimed to evaluate the long-term outcomes of using pedicled gracilis flaps for pelvic reconstruction and to analyze predictors of postoperative complications. METHODS: This was a retrospective review of a single reconstructive surgeon's cases between January 2012 and June 2021 identifying patients who underwent perineal reconstruction second…

BiCyCLE NMES-neuromuscular electrical stimulation in the perioperative treatment of sarcopenia and myosteatosis in advanced rectal cancer patients: design and methodology of a phase II randomised controlled trial.

BACKGROUND: Colorectal cancer is associated with secondary sarcopenia (muscle loss) and myosteatosis (fatty infiltration of muscle) and patients who exhibit these host characteristics have poorer outcomes following surgery. Furthermore, patients, who undergo curative advanced rectal cancer surgery such as pelvic exenteration, are at risk of skeletal muscle loss due to immobility, malnutrition and a post-surgical catabolic state. Neuromuscular electrical stimulation (NMES) may be a feasible adjunctive treatment to help ameliorate these adverse side-effects. Hence, the purpose of this study is t…

Neoadjuvant chemo-radiotherapy is the standard of care for patients with locally advanced rectal cancer. However, certain patients are ineli … rting colostomy was performed. Short-course radiotherapy led to marked tumor regression. Subsequent pelvic exenteration achieved complete tumor removal. Postoperative wound dehiscence was managed successfully with vacuum-assisted closure therapy. No recurrence was observed on follow-up. A Rare Case of a Non-Invasive Vaginal Urothelial Carcinoma Demonstrating a Clonal Relationship With a Metachronous Non-Invasive Bladder Urothelial Carcinoma.

Non-invasive vaginal urothelial carcinomas (UCs) are extremely rare and are almost always associated with a non-invasive bladder UC. The pathogenesis of these rare vaginal lesions is uncertain. They may represent a primary vaginal tumour or they may arise from a urinary tract UC, in which tumour cells "seed" by non-invasive mechanisms. We describe a case of a non-invasive vaginal UC, occurring 6 years following treatment of a non-invasive bladder UC, managed with an anterior pelvic exenteration and followed up over 5 years. Molecular analysis using a genome-wide single nucleotide polymorphism …

[A Case of Lynch Syndrome with MSH2 Likely Pathogenic Variant Who Developed Rectal Cancer Due to Methylation of MLH1].

The proband is a 47-year-old woman who underwent a posterior total pelvic exenteration for Stage Ⅲb rectal cancer. Microsatellite instability(MSI)test and immunohistochemistry(IHC)for mismatch repair(MMR)proteins using the resected tumor tissue showed MSI-High and loss of MLH1/PMS2 expression. In addition, although BRAF V600E variant was not identified, MLH1 methylation was detected. Therefore, this patient was diagnosed as having sporadic deficient MMR(MSI-High) rectal cancer. However, the possibility of Lynch syndrome could not be ruled out because of her family history and young age of onse…

Scurvy presenting as lower limb ecchymoses in the setting of metastatic colorectal cancer.

A 58-year-old woman presented with a 1-week history of lower limb bruising. She had a medical history of recurrent metastatic colon cancer with a sigmoid colectomy and complete pelvic exenteration leading to colostomy and urostomy formation. She had malignant sacral mass encroaching on the spinal cord. This caused a left-sided foot drop for which she used an ankle-foot orthosis. She was on cetuximab and had received radiotherapy to the sacral mass 1 month ago. On examination, there were macular ecchymoses with petechiae on the lower limbs. There was sparing of areas that had been compressed by…

Aggressive multimodality treatment for advanced rectal cancer.

A case of advanced rectal cancer treated by aggressive local and systemic treatment who has survived more than 7 years from initial recurrence is presented. A 55-year-old woman was diagnosed with advanced lower rectal cancer and underwent a low anterior resection with complete removal of all regional lymph nodes and total mesorectal excision. The tumor was diagnosed as a moderately differentiated adenocarcinoma, pStage IIIB (T3, N2a, M0). Twenty-six months after the initial surgery, local recurrence in the pelvis was detected by computed tomography, and total pelvic exenteration with distal sa…

Hemiresective reconstruction of a redundant ileal conduit with severe bilateral ileal conduit-ureteral re fl ux.

A 58-year-old man was referred to our hospital with high fever and anuria. Since undergoing a total pelvic exenteration due to bladder-invasive sigmoid colon cancer, urinary tract infections had frequently occurred. We treated with the construction of a bilateral percutaneous nephrostomy (PCN), and chemotherapy. Although we replaced the PCN with a single J ureteral catheter after an improvement of infection, urinary infection recurred because of an obstruction of the catheter. Urological examinations showed that an ileal conduit-ureteral reflux caused by kinking of the ileal loop was the reaso…

Urinary tract infection associated with bacteremia caused by vancomycin-resistant enterococcus following continent urinary diversion.

KEY CLINICAL MESSAGE: Even in a country where vancomycin-resistant enterococcus is rare, multidrug-resistant organism precautions are necessary when admitting patients with a history of medical exposure in other countries. On admission, screening is necessary and if infection is confirmed, a multidisciplinary approach involving different specialists is required. ABSTRACT: The patient was a 49-year-old Japanese female living in the United States. Total pelvic exenteration for cervical carcinoma, Miami pouch formation, and ileostomy had been performed in the United States. She returned to Japan…

[Remarkable Local Bleomycin Ointment Therapy for Squamous Cell Carcinoma of the Renal Pelvis in a Hemodialysis Patient with a Long-Term Nephrostomy Catheter--A Case Report].

The patient was a 92-year-old woman. Pelvic exenteration and bilateral ureterocutaneostomy had been performed for rectal carcinoma 29 years earlier. Right nephrostomy had been performed for right ureteral stenosis 20 years earlier. Left nephrectomy had been performed for the left atrophic kidney 12 years earlier. She began receiving hemodialysis for renal failure 10 years earlier. In September 2014, macrohematuria and an abnormal mass at the right nephrostomy were observed. Pathological examination revealed a well-differentiated squamous cell carcinoma. Magnetic resonance imaging revealed that…

Ileal J-pouch vaginoplasty: reconstruction of a physiologic vagina with an ileal J-pouch.

Vaginal reconstruction has been performed for more than a century. Main complications are vaginal stenosis requiring dilatation, dyspareunia, excessive mucus secretion, and poor aesthetic and functional outcome. Here we report a new operation method modified after Baldwin for intestinal vaginoplasty in a patient with pelvic exenteration after spinal cell carcinoma of the vagina. Because of balanced liquid resorption and mucus secretion with sufficient vessel length in the terminal ileum, this intestinal segment was chosen. A J-pouch of distal ileum was constructed pedicled on the ileocolic art…

Myosarcomas of the small and large intestine: a clinicopathologic study.

A retrospective study was made of 38 myosarcomas of the small and large intestines (34 leiomyosarcomas and 4 malignant leiomyoblastomas). Endoenteric, small, and noninvasive tumors were successfully treated by simple wedge excision, cautery snare removal, or segmental bowel resection with a high 5-yr cure rate of 86% (12/14). Exo-enteric tumors often invaded adjacent structures or perforated into the peritoneal cavity. Extensive resection procedures, including pancreaticoduodenectomy, abdominoperineal resection, and pelvic exenteration salvaged only 13% (2/16) of these patients. Three clinicop…

Peri-Operative Systemic Inflammatory Responses Predict Post-Operative Complications in Patients Undergoing Pelvic Exenterative Surgery for Primary Locally Advanced Sigmoid and Rectal Cancer.

BACKGROUND AND OBJECTIVES: The systemic inflammatory response (SIR) is a recognized determinant of outcome in standard colorectal resections, yet its role in exenterative surgery remains unclear. The study aims to evaluate the relationship between peri-operative SIR biomarkers and post-operative complications in pelvic exenterative surgery. METHODS: A retrospective cohort study of consecutive patients undergoing pelvic exenteration (PE) or beyond total mesorectal excision (bTME) for primary locally advanced sigmoid and rectal cancers at a tertiary referral center. Clinicopathological variable…
