Impact of Visual Masking on Attentional Bias in Cross-Sectional Clinical and Healthy Populations: Differences in Conscious and Unconscious Processing of Alcohol-Related Cues.

BACKGROUND: Attentional bias (AB) toward substance-related cues is a core cognitive mechanism in alcohol use disorder (AUD) and closely associated with craving and relapse risk. Yet, the extent to which AB reflects automatic bottom-up processes remains unclear. METHODS: This study (N = 107) examined alcohol-related AB in clinical populations (inpatients and outpatients with AUD) and healthy controls (occasional and non-drinkers). Alcohol cues were presented under masked and unmasked conditions in a dot-probe task, and AB was measured via reaction times. Patients also reported alcohol craving

Acoustic Realism of Clinical Speech-in-Noise Testing: Parameter Ranges of Speech-Likeness, Interaural Coherence, and Interaural Differences.

Speech-in-noise testing is a valuable component of audiological examination that can provide estimates of a listener's ability to communicate in their everyday life. It has long been recognized, however, that the acoustics of real-world environments are complex and variable and not well represented by a typical clinical test setup. The first aim of this study was to quantify real-world environments in terms of several acoustic parameters that may be relevant for speech understanding (namely speech-likeness, interaural coherence, and interaural time and level differences). Earlier acoustic anal

The Roles of Selective Attention and Asymmetric Experience in Bilateral Speech Interference for Single-Sided Deafness Cochlear Implant and Vocoder Listeners.

OBJECTIVES: For many (especially older) single-sided-deafness (SSD) cochlear-implant (CI) users (one normal hearing and one CI ear), masking speech in the acoustic ear can interfere with CI-ear speech recognition. This study examined two possible explanations for this "bilateral speech interference." First, it might reflect a general (i.e., not specific to spatial hearing or CI use) age-related "selective-attention" deficit, with some listeners having difficulty attending to target speech while ignoring an interferer. Second, it could be specific to asymmetric-hearing experience, reflecting ma

Comparison of real-ear insertion gains in Japanese-speaking individuals wearing hearing aids with DSLv5 and NAL-NL2.

OBJECTIVE: We used real-ear insertion gain (REIG), with the international speech test signal (ISTS), to evaluate the amplifying characteristics of hearing aids, set for patients who have been wearing such aids for a long time in a stable manner. We further compared this to the target values of the DSLv5 and NAL-NL2 methods. METHODS: The subjects were adults with moderate sensorineural hearing loss. We examined 40 ears in 25 individuals (15 people wearing hearing aids in both ears and ten people wearing aid in only one ear). Fit assessments were performed based on the speech performance-intens

Frontal asymmetry predicts the incentive value of perceptual information.

Information has been suggested to convey incentive value mediated by dopaminergic systems similar to those implicated in extrinsic reward. Although the reward characteristics of information have received preliminary support by behavioral and fMRI findings, EEG correlates and individual differences have not yet been examined. In the current study, a novel perceptual paradigm was developed to probe the associations between anticipation of perceptual information and frontal electroencephalographic alpha asymmetry, i.e., a marker of approach motivation. Assuming individual differences in engaging

Transient Abnormalities in Masking Tuning Curve in Early Progressive Hearing Loss Mouse Model.

Damage to cochlear outer hair cells (OHCs) usually affects frequency selectivity in proportion to hearing threshold increase. However, the current clinical heuristics that attributes poor hearing performance despite near-normal auditory sensitivity to auditory neuropathy or "hidden" synaptopathy overlooks possible underlying OHC impairment. Here, we document the part played by OHCs in influencing suprathreshold auditory performance in the presence of noise in a mouse model of progressive hair cell degeneration, the CD1 strain, at postnatal day 18-30 stages when high-frequency auditory threshol

Olivocochlear efferents: Their action, effects, measurement and uses, and the impact of the new conception of cochlear mechanical responses.

The anatomy and physiology of olivocochlear (OC) efferents are reviewed. To help interpret these, recent advances in cochlear mechanics are also reviewed. Lateral OC (LOC) efferents innervate primary auditory-nerve (AN) fiber dendrites. The most important LOC function may be to reduce auditory neuropathy. Medial OC (MOC) efferents innervate the outer hair cells (OHCs) and act to turn down the gain of cochlear amplification. Cochlear amplification had been thought to act only through basilar membrane (BM) motion, but recent reports show that motion near the reticular lamina (RL) is amplified mo

Single and multiple microphone noise reduction strategies in cochlear implants.

To restore hearing sensation, cochlear implants deliver electrical pulses to the auditory nerve by relying on sophisticated signal processing algorithms that convert acoustic inputs to electrical stimuli. Although individuals fitted with cochlear implants perform well in quiet, in the presence of background noise, the speech intelligibility of cochlear implant listeners is more susceptible to background noise than that of normal hearing listeners. Traditionally, to increase performance in noise, single-microphone noise reduction strategies have been used. More recently, a number of approaches

Age-related benefits of digital noise reduction for short-term word learning in children with hearing loss.

PURPOSE: To determine the rate of word learning for children with hearing loss (HL) in quiet and in noise compared to normal-hearing (NH) peers. The effects of digital noise reduction (DNR) were examined for children with HL. METHOD: Forty-one children with NH and 26 children with HL were grouped by age (8-9 years and 11-12 years). The children learned novel words associated with novel objects through a process of trial and error. Functions relating performance across trials were calculated for each child in each listening condition and were compared. RESULTS: Significant effects were observ

Distortion product otoacoustic emission contralateral suppression functions obtained with ramped stimuli.

The purpose of this research was to investigate the changes that occur in human distortion product otoacoustic emission (DPOAE) level functions over continuous frequency bands in response to activation of the medial olivocochlear (MOC) efferent system by contralateral broadband noise. DPOAEs were obtained using continuous upward ramps of the lower frequency tone (f(1)) while the higher frequency tone (f(2)) was fixed. These ramps were designed to change the stimulus frequency ratio f(2)/f(1) over a fixed range for each fixed f(2) value of 2, 3, and 4 kHz. Contralateral noise was presented on a

Application of the TEN test to hearing-impaired teenagers with severe-to-profound hearing loss.

Hearing impairment is often associated with damage to the hair cells of the cochlea. An area of the cochlea with complete loss of function of inner hair cells is known as a 'dead region'. Dead regions can be identified by measuring detection thresholds for pure tones in quiet and in threshold-equalizing noise (TEN). So far, the TEN test has only been used to identify dead regions in adults with moderate-to-severe hearing impairment. The aim of this study was to assess problems in applying the TEN test to teenagers with longstanding severe-to-profound sensorineural hearing impairment, and to as

[Electrocochleography and phase audiometry in diagnosis of Meniere disease].

The diagnosis of Menière's disease is uncertain when the typical symptoms do not occur completely and definitely. A reliable finding of an endolymphatic hydrops (EH) is the base for a correct prognosis and therapy. Electrocochleography is a proven diagnostic procedure but requires a lot of time and of technical know-how. The mobility of cochlear partition can be tested by low-frequency masking (LFM) recording the phase dependent subjective masked threshold of a short test tone. We performed electrocochleography and LFM in 29 patients with suspected Menière's disease at the same day. Both tests

Role of the efferent medial olivocochlear system in contralateral masking and binaural interactions: an electrophysiological study in guinea pigs.

Contralateral broadband noise (BBN) elevates ipsilateral auditory thresholds (central masking) and reduces the amplitude of ipsilateral brainstem auditory evoked potentials (BAEPs). Binaural interactions are complex psychophysical phenomena, but binaural interaction components are easily extracted from BAEPs to monaural versus binaural click stimulation. However, contralateral, or binaural, acoustical stimulation is known to activate simultaneously the crossed and uncrossed medial olivocochlear (MOC) efferent systems and decrease activity in both cochleas. Particularly, contralateral BBN stimu

Evaluation of the nonorganic hearing loss suspect.

OBJECTIVE: To examine the utility of distortion-product otoacoustic emissions (DPOAEs) in the assessment of the nonorganic suspect because DPOAE analysis constitutes an objective test of hair-cell function that yields audiometriclike data. STUDY DESIGN: Retrospective study of clinical findings. SETTING: Audiology outpatient clinic of our university's medical center. PATIENTS: The study cohort comprised 30 patients who presented with a profile of suspicion for nonorganic hearing loss. Most cases were found, with the aid of DPOAE testing, to be nonorganic or to have nonorganic overlays to org

Paranoid schizophrenia: non-specificity of neuropsychological vulnerability markers.

During stages of remission, patients with paranoid schizophrenia seldom show severe attentional or information-processing dysfunctions, except in cases of long-term chronicity. The diagnostic specificity of four putative psychological vulnerability indicators of schizophrenia - the Span of Apprehension, the degraded stimulus Continuous Performance Test (dsCPT), the degraded stimulus visual backward masking task and the Wisconsin Card Sorting Test (WCST) - was examined in a group of patients with paranoid schizophrenia. Since no single test seems to identify all patients, the use of a combinati

Mismatches between auditory demands and capacities in the industrial work environment.

Exposure limits to noise in the workplace have been set merely to prevent disabling hearing loss. Consequently, even in workplaces where a limit of 85 dBA-8 h is strictly enforced, the sound environment is very poorly matched to human auditory capabilities. In fact, ergonomists have been led to characterize this feature of the industrial work environment as an 'extreme condition' that is being imposed on a major segment of the workforce. This type of sound environment seriously restricts the possibilities of making use of auditory temporal, spatial, and frequency resolution, and often reduces

The effect of otitis media with effusion on the masking-level difference and the auditory brainstem response.

This study investigated the masking-level difference (MLD) and auditory brainstem response (ABR) in a group of children with a history of otitis media with effusion (OME) and a control group of children with no known history of ear disease. All children had normal hearing at the time of testing. The main goal of the study was to determine whether there was an association between a reduced MLD in the OME children and an abnormal ABR (in terms of prolonged absolute or interwave intervals, or interaural differences in the ABR waveforms). The results indicated that the group of children having a h

Using the derived auditory brain stem response to estimate traveling wave velocity.

An estimate of the traveling wave velocity (TWV) was evaluated in 18 young adults to determine the technique's applicability as a possible tool in evaluating Meniere's disease. This TWV estimation uses a derived auditory brain stem response procedure requiring high-intensity masking. The results indicate that on average, TWVs were similar to those obtained via psychophysical measures or electrocochleography. However, instances of undermasking, due to subjects' intolerance, resulted in a number of subjects exhibiting faster than expected TWVs.

The influence of talker differences on vowel identification by normal-hearing and hearing-impaired listeners.

Vowel identification was tested in quiet, noise, and reverberation with 20 normal-hearing subjects and 20 hearing-impaired subjects. Stimuli were 15 English vowels spoken in a /b-t/context by six male talkers. Each talker produced five tokens of each vowel. In quiet, all stimuli were identified by two judges as the intended targets. The stimuli were degraded by reverberation or speech-spectrum noise. Vowel identification scores depended upon talker, listening condition, and subject type. The relationship between identification errors and spectral details of the vowels is discussed.

Evoked-response forward-masking functions in chinchillas with noise-induced permanent hearing loss.

Evoked-response forward-masking functions were measured by chronic electrodes in the inferior colliculus of the chinchilla before and after exposure to an intense tone that produced a permanent hearing loss. Before exposure, the forward-masking time constants ranged from 50 to 90 ms. After exposure, the forward-masking time constants increased significantly in the region of hearing loss, but not in regions where hearing was normal. The effect of the hearing loss on the time course of forward masking was most pronounced once the hearing loss exceeded 20-25 dB. These physiological changes in the
