Perinatal mortality in term and post-term births.

The hospital records were reviewed of 64 infants born at or after 37 weeks of amenorrhea who suffered perinatal mortality during a 3-year period at Boston Hospital for Women. The most frequent cause of death of a term or post-term infant was extrinsic perinatal hypoxia, and the second most common was lethal malformation. Half the deaths of term infants occurred ante partum. Further reduction in perinatal mortality for this group may require extension of antepartum fetal monitoring techniques to all pregnancies. Intrapartum loss was rare, occurring at a rate of 0.43 per 1000 for infants deliver

Twin pairs, smoking in pregnancy and perinatal mortality.

Twins born in the province of Quebec during the first six months of 1972 were ascertained by reviewing all live births and perinatal death certificates. A total of 852 twins, constituting 426 pairs, were included in the study. Smoking habits of the mother during pregnancy were recorded at the hospital of birth for 88.5% of the pairs. The risk of at least one perinatal death was significantly higher for twin pairs born to smoking mothers. The Mantel-Haenszel test statistic controlling for sex of pairs, parity (0,1+), maternal age (< 25,25+ years) and years of schooling was 4.224 (p = 0.04). The…

Maternal, prenatal and traditional practice factors associated with perinatal mortality in Yemen.

BACKGROUND: Perinatal mortality remains a major international problem responsible for nearly six million stillbirths and neonatal deaths. OBJECTIVES: To estimate the perinatal mortality rate in Sana'a, Yemen and to identify risk factors for perinatal deaths. METHODS: A community-based prospective cohort study was carried out between 2015 and 2016. Nine-hundred and eighty pregnant women were identified and followed up to 7 days following birth. A multi-stage cluster sampling was used to select participants from community households', residing in the five districts of the Sana'a City, Yemen. …

The major determinants of perinatal mortality in a large metropolitan hospital. Results of a retrospective study.

We reviewed the records of all perinatal deaths that occurred at Mount Sinai Hospital, New York, from 1975 to 1978. The perinatal mortality rate was 20 per 1,000 births. Most potentially preventable perinatal deaths occurred in association with low birth weight. We noted that fetal deaths that occurred prior to the onset of labor were usually associated with small-for-gestational-age (SGA) or growth-retarded fetuses, whereas deaths occurring during labor were usually the result of trauma to the very small third-trimester fetus, one weighing less than 1,500 gm. Neonatal deaths most commonly res…

Use of refined perinatal mortality rate trends to evaluate the effect of a confidential inquiry.

Standardised perinatal mortality rates and cause-specific rates were used to evaluate the effect of a confidential inquiry into perinatal deaths in West Glamorgan obstetric units. Cause of death was classified by means of 5 pathological subgroups, and the denominator data were obtained from the computerised standard child-health system. The standardised perinatal mortality rate fell from 103 in 1981 to 85 in 1983, and the perinatal mortality rate fell from 15.9 to 9.4 per 1000 births in that period; 3 of the cause-specific rates declined considerably. The trend indicates an improvement in ante…

Association between fetal sex, birthweight percentile and adverse pregnancy outcome.

INTRODUCTION: The objective was to evaluate the association between fetal sex and adverse pregnancy outcome, while correcting for fetal growth and gestational age at delivery. MATERIAL AND METHODS: Data from the Netherlands Perinatal Registry (1999-2010) were used. The study population comprised all white European women with a singleton delivery between 25+0 and 42+6  weeks of gestation. Fetuses with structural or chromosomal abnormalities were excluded. Outcomes were antepartum death, intrapartum/neonatal death (from onset of labor until 28 days after birth), perinatal death (antepartum deat…

Adverse perinatal events and maternal interpregnancy weight change: A population-based observational study.

OBJECTIVE: Mothers whose newborn experiences adversity may neglect their own health to care for their affected infant or following a perinatal death. Weight gain after pregnancy is one measure of maternal self-care. We measured interpregnancy weight gain among women whose child had an adverse perinatal event. METHODS: This population-based observational study included 192 154 primigravid women with two consecutive singleton births in Ontario, Canada. Outcomes included net weight gain, and adjusted odds ratios (aOR) of moving to a higher body mass index (BMI) category between pregnancies, comp…

[Maternal and perinatal health status in the State of São Paulo, Brazil].

The assistance offered during pregnancy and labour as also to the newborn child, and its relationship to maternal and perinatal mortality in the State of S. Paulo in 1984, is analysed on the basis of official available data. With respect to prenatal care the number of visits per woman was considered to be "sufficient" though of doubtful quality. The proportion of cesarean sections was very high (46.2%). Maternal mortality was found to be 4.86 deaths per 10,000 live births, but despite its being high, this figure is certainly too low and the correct figure is probably twice as high. The princip

The status of infant mortality in Sweden and the United States.

A study was made of live birth rates, infant, neonatal, and postneonatal death rates, late fetal death ratios, and perinatal death ratios for Sweden and the United States for selected years, 1950-1973. Sweden has lower live birth rates, infant, neonatal, and postneonatal death rates, as well as lower perinatal death ratios. Data available on low-birth-weight live births for selected years, 1959-1970, for both countries indicated that rates in the United States were almost twice those in Sweden. Implications of these findings for the United States are discussed in light of the quantity, quality…

Examining adverse fetal/neonatal outcomes associated with severe maternal morbidity.

BACKGROUND: While there is a global focus on severe maternal morbidity (SMM), less is known about the impact of SMM on fetal and neonatal outcomes. AIMS: To examine fetal/neonatal outcomes associated with SMM. MATERIALS AND METHODS: A national New Zealand (NZ) retrospective cohort study describing fetal/neonatal outcomes of all women with SMM admitted to a NZ Intensive Care Unit (ICU) or High Dependency Unit (HDU) in 2014. Adverse fetal/neonatal outcomes were defined as one or more of the following: fetal or early neonatal death, hypoxic ischaemic encephalopathy, Apgar score less than seven

A health system framework for perinatal care in South African district hospitals: a Delphi technique.

BACKGROUND: The majority of perinatal deaths occurring in low- and middle- income countries are preventable. South Africa is a middle-income country with consistently high perinatal mortality rates and most factors contributing to preventable deaths are linked to the functioning of the health system. Particularly of concern in South Africa is the high perinatal mortality in district hospitals, where most births occur and where intrapartum care is provided to women of low and intermediate risk. Therefore, it is crucial to strengthen the health system for perinatal care in district hospitals. Th…

Perinatal mortality trends in a referral hospital.

A comparative study of perinatal mortality patterns over a period was conducted at a teaching hospital of South India. Among the 6,048 babies born from January 1984 to December 1985 (Group A), there were 265 (43.8/1000) still births and 127 (22.0/1000) early neonatal deaths. Three hundred and thirty seven (41/1000) babies were still born and 235 (29.8/1000) early neonatal deaths out of 8,215 deliveries during 1992-93 (Group B). The perinatal mortality rate (PMR) in Groups A and B were 57.9/1000 and 57.7/1000 respectively. Unbooked cases accounted for the majority (> 75%) of perinatal deaths du…

Primary causes of total perinatally related wastage at Tygerberg Hospital.

The primary obstetric cause of total perinatally related wastage (TPRW) (i.e. all antepartum or postpartum deaths of infants greater than or equal to 500 g and who died before hospital discharge) was studied in a clearly defined population in the western Cape over a 1-year period. There were 302 deaths from 7,923 singletons and 31 deaths from 65 pairs of twins delivered from patients cared for by Tygerberg Hospital maternity services. Thirty per cent of the deaths were late abortions, 42% stillbirths, 18% early neonatal deaths, 7% late neonatal deaths and 4% perinatally related infant deaths.

Maternal anaemia and its impact on perinatal outcome in a tertiary care hospital in Pakistan.

Maternal anaemia is a common problem in pregnancy, particularly in developing countries. We investigated the relationship between maternal anaemia and perinatal outcome in a cohort of 629 pregnant women from October 2001 to 2002. Of these, 313 were anaemic (haemoglobin < 11 g/L). Perinatal outcomes included preterm delivery, low birth weight, intrauterine growth retardation, perinatal death, low Apgar scores and intrauterine fetal death. Univariate and multivariate analyses were performed. The risk of preterm delivery and low birth weight among the anaemic women was 4 and 1.9 times more respec…

Prediction of Perinatal Mortality in Ebstein's Anomaly Diagnosed in the Second Trimester of Pregnancy.

OBJECTIVES: Firstly, to describe the outcome of a series of fetuses with Ebstein's anomaly (EA) and, secondly, to study the utility of different second-trimester echocardiographic parameters to predict fetal and neonatal mortality. METHODS: 39 fetuses with EA diagnosed between 18 and 28 weeks of gestation were included. Fetal echocardiography included the cardiothoracic ratio (CTR); right atrial (RA) area index; displacement of the tricuspid valve (TV); tricuspid regurgitation; pulmonary artery; and ductus arteriosus flow characteristics. Additionally, 2 novel parameters were obtained: the re
