Endodontic microsurgery (EMS) is often utilized to surgically treat teeth with persistent endodontic lesions when conventional nonsurgical endodontic treatments have been unsuccessful. However, EMS with conventional techniques may result in esthetic concerns, such as gingival recession, papilla loss, and soft tissue scarring, especially in patients with high esthetic demands. This manu-script describes a dual-layer flap preparation approach that utilizes an external coronally advanced flap (CAF) and an internal trapdoor periosteal flap (TPF) for EMS in the esthetic zone. The external CAF is a

BACKGROUND: Periosteal distraction (PD) has been used as a salvage strategy for diabetic foot (DF) and diabetic foot ulcer (DFU), yet its relative effectiveness compared with other approaches is still unclear.
METHODS: Up to March 17, 2026, seven databases were searched for comparative studies assessing PD-related treatments in DF/DFU. Twenty-six studies were included for qualitative synthesis, and 14 contributed to the primary comparative evidence set. The studies were organized into three categories: PD-based interventions versus conventional treatment, pure PD versus transverse tibial tran

OBJECTIVE: To investigate the anatomic basis for transposition of the distal dorsal ulna bone flap pedicled with dorsal metacarpal artery to repair the defect of the 3rd or 4th and 5th metacarpal bone head.
METHODS: In 30 adult cadaveric upper limbs, the branches and constitutions of the dorsal carpal arterial networks were observed.
RESULTS: The dorsal carpal arterial networks were consisted of the dorsal carpal branches of ulnar and radial arteries, the terminal branches of posterior interosseous artery and the dorsal carpal branch of anterior interosseous artery, and then the 2nd, 3rd, 4t

We describe a new technique for extensive retropositioning of the soft palate for the treatment of velopharyngeal insufficiency. This technique is identified as a nasopharyngeal pushback, and has been used repeatedly in conjunction with both a Cronin nasal flap and a superiorly based pharyngeal flap when maximum retropositioning was needed. This procedure has been used for over ten years, each time obtaining an additional pushback distance equal to or greater than the distance achieved by freeling the soft palate from the posterior border of the hard palate.

BACKGROUND: The purpose of the present case report is to describe the potential of the subperiosteal peri-implant augmented layer (SPAL) tec … if not properly managed. This case report presents a surgical approach that uses the patient's own periosteum, a thin tissue layer covering the bone, to help reconstruct buccal bone defects at the time of immediate implant placement. The periosteum was carefully separated and repositioned to act as a natural biological membrane, creating a protected space t …

Populations of bone marrow stromal cells (BMSCs, also known as bone marrow-derived "mesenchymal stem cells") contain a subset of cells that are able to recapitulate the formation of a bone/marrow organ (skeletal stem cells, SSCs). It is now apparent that cells with similar but not identical properties can be isolated from other skeletal compartments (growth plate, periosteum). The biological properties of BMSCs, and these related stem/progenitor cells, are assessed by a variety of assays, both in vitro and in vivo. Application of these assays in an appropriate fashion provide a great deal of i…

An 8 year-old female with esophageal atresia and a type IV laryngotracheoesophageal cleft underwent tracheostomy, gastric pull-up and fundoplication with G-tube insertion at birth. She remained nil per os due to aspiration. The trachealis was separated from the esophagus that was reapproximated and clavicular periosteum was placed. A long posterior costal cartilage graft that engaged behind the cricoid plate and tapered inferiorly was inserted. A second thin cartilage graft was sutured to it distally to extend the length of the repair. This allowed for removal of the tracheostomy and oral feed…

The authors describe a case of distal femoral physeal injury with disruption of the perichondrium in a 9-year-old girl after a sledding accident. The patient presented with knee pain, limited range of motion, and inability to bear weight. Initial radiographs were normal. A magnetic resonance imaging of the knee demonstrated abnormal signal and widening of the distal femoral physis with elevation of the posterior distal femoral periosteum. This case illustrates the main magnetic resonance imaging findings in an occult Salter Harris type I injury: increased physeal thickness and signal intensity…

A speculative suggestion for a curative approach to osteogenesis imperfecta is based on the experimental observation that a pluripotent stem cell for mesenchymatous tissues can be isolated from marrow or periosteum. This cell, referred to as a Mesenchymal Stem Cell (MSC) could provide a vehicle for gene therapy based on that fact that many mesenchymal tissues have reasonably high cellular turnover rates. If autologous MSCs with the "proper" gene construct can be transplanted into affected patients, it may be that the progeny of the gene-corrected MSC can replace the defective mature phenotypes…

An unusual case of the ossifying fibroma of a 54-year-old woman is reported. Clinically the lesion appeared as an exophytic mass on the hard palate and apparently originated outside of bone. Histologically the resected tumor is well encapsulated and made up of a richly cellular fibroblastic stroma containing foci of mature bone with a predominantly lamellar structure. While the possibility exists that the lesion is reactive, it appears to be a true neoplastic growth and a purely soft tissue process arising from the periosteum of the palate. Thus, we proposed the term periosteal ossifying fibro…

UNLABELLED: The medial femoral condyle free flap serves as an attractive reconstructive option for small- to intermediate-sized bony defects … d novel reconstructive sites and surgical indications. Flap components were described in 157 cases; periosteum was used only in four cases (2.5%), whereas all others are composed of cortical bone combined with periosteum, cancellous bone, and/or cartilage (97.5%). Additionally, a skin island was used in 43 cases (25.9%). Flap measurements were reported in 51 …

AIMS: Transient ischemia of osteoporotic bones during elective orthopedic surgery or fracture repair carries risks for serious complications … ischemia-reperfusion elevated the neutrophil adherence ~3-fold in the postcapillary venules of the periosteum (intravital microscopy), with an ~50-60% increase in intravascular neutrophil activation (CD11b; FACS analysis), an enhanced TNF-α release (ELISA) and periosteal expression of ICAM-1 (the endothelial ligand of CD11b; immunohistochemistry). INTRODUCTION: An involucrum refers to the formation of new bone by the periosteum surrounding a bony sequestrum. Over time, the sequestrum undergoes resorption via creeping substitution, ultimately leading to complete bone healing. This phenomenon is well-documented in the context of long bone osteomyelitis.
METHOD: We report a case of an 82-year-old woman who healed spontaneously from stage 3 medication-related osteonecrosis of the jaw following dentoalveolar surgery while on denosumab.
AIM: The aim of this article is to highlight a rare instance of resolution through the process of involuc…

Pediatric ankle injuries are common; ankle epiphyseal fractures are also common in children. But isolated distal epiphyseal fibular fractures of the distal fibula are clinically rare. We describe one unusual case of an adolescent with a completely displaced Salter-Harris type II distal fibular epiphyseal fracture. The attempt of closed reduction failed, and the patient required open reduction and internal fixation. The localized periosteum and the superior peroneal retinaculum were avulsed from the distal fibular metaphysis, with the peroneal tendons underneath exposed but no obvious subluxati…
