Various defence mechanisms are found in the oesophagus which can be elicited by reflux damage. Premucosal defence includes bicarbonate ions and epidermal growth factor (EGF) secreted by salivary and oesophageal glands. The mucosa can respond by increasing epithelial cell turnover and upregulating EGF receptor and endocytosis. The intercellular barrier can be increased by the contents of membrane-coating granules. Local pH can be regulated by carbonic anhydrase. The whole viscus can exhibit peristalsis to effect a mechanical clearance of the refluxed gastric and duodenal material.

Enterobacterial and pseudomonal carriage in the oropharynx, stomach and gut is uncommon in healthy individuals (apart from E. coli in the gut). Underlying intrinsic disease is the main cause of oropharyngeal and gastrointestinal carriage of 'abnormal' flora. Interventions including intubation, catheterisation and drugs impairing peristalsis may promote this abnormal carriage in the critically ill. The oropharyngeal and gastrointestinal carrier state is the first stage in the development of nosocomial infections. Procedures aimed at the prevention of hospital infections via the abolition of the…

Recurrent vomiting is common in children with severe mental retardation and leads to significant morbidity with malnutrition, anemia, and aspiration pneumonitis. Spasms of the abdominal muscles and diaphragm, uncoordinated peristalsis, and central nervous system disorders are causes of dysphagia and continuous gastroesophageal reflux. It is desirable that mentally retarded children with vomiting have a barium swallow and esophagoscopy as early as possible. Fundoplication should be performed before complications develop. Spasms with aspiration followed by apnea, in particular, are life-threaten…

The influence of Pentothal anesthesia in blocking the mechanism intimating ureteral peristalsis in the dog is examined. Pressure measurements of pacemaker rhythmicity were made through renal cortical catheterization of the renal pelvis. Simultaneously, ureteral recordings were made by extracellular electromyography of the ureteral smooth muscle wall. It is shown that Pentothal anesthesia transiently blocks the spontaneous and rhythmic contractions of the renal pelvis and inhibits the generation of ureteral peristaltic contractions. It is demonstrated that repeated or prolonged pacemaker blocka…

Meckel diverticulum is the most common congenital anomaly of the gastrointestinal tract which is located in small bowel within 2 feet of the ileocecal valve. Nevertheless, an inverted Meckel's diverticulum is an uncommon condition believed to result from aberrant peristalsis in that specific area. This article showed signs, symptoms, and possible clinical presentations using CARE guidelines in a case of inverted Meckel's diverticulum and reviews other possible features lastly, definitive treatment, results, and case follow-up were shown to refresh, and raise surgeons' awareness of this rare di…

In this case report, we present the rare complication of proximal migration of ureteral stent after kidney transplant, and we describe our approach for its diagnosis and treatment. Although endoscopic management is the treatment of choice for retrieval of migrated stents, we reexplored the transplant in response to erroneous reporting of ureteral stent lying in an extravesicular position on a pelvic computed tomography scan. Subsequent exploration disproved the original interpretation of the image. Proximal stent migration may result from retrograde peristalsis and is generally related to subo…

Esophageal achalasia is a primary esophageal motility disorder defined by the lack of esophageal peristalsis, and by a lower esophageal sphincter that fails to relax in response to swallowing. Patients' symptoms include dysphagia, regurgitation, aspiration, heartburn, and chest pain. Achalasia is a chronic condition without cure, and treatment options are aimed at providing symptomatic relief, improving esophageal emptying, and preventing the development of megaesophagus. Presently, a laparoscopic Heller myotomy with a partial fundoplication is considered the best treatment modality. A properl…

There is no consensus on the ideal treatment for esophageal perforation and on the maximal extent of esophageal disruption amenable to primary repair. The effect of extensive esophageal injury on postoperative esophageal motility is also unknown. We report the case of a longitudinal iatrogenic esophageal laceration extending from the hypopharynx to the cardia in a morbidly obese patient treated with primary repair. The patient exhibited no postoperative esophageal leak or stricture and maintained a preserved esophageal peristalsis on manometry at 3 months. An extensively lacerated esophagus ca…

Megaesophagus is a disorder of the esophagus characterized by diffuse dilation and decreased peristalsis. It is classified into congenital and acquired forms. Gastrointestinal, endocrine, immune-mediated, neuromuscular, paraneoplastic, and toxic disorders have been associated with acquired megaesophagus. Common clinical signs of megaesophagus are regurgitation, weight loss, coughing, and halitosis. Most cases of megaesophagus can be diagnosed using thoracic radiography; however, diagnosing the underlying cause requires a thorough history and additional diagnostics. The treatment, management, a…

We present the case of a patient who was referred to our hospital after she reported having passed a long, whitish object per rectum. Accordingly, capsule endoscopy was performed using the PillCam(®) SB video capsule. A tapeworm of the species Diphyllobothrium nihonkaiense was detected; it appeared to be freely floating and unfolded in the jejunum and sometimes tangled or irregularly folded in the ileum. The stretching of the strobila by strong peristalsis in the ileum may have resulted in the separation of the caudal portion of the strobila, which descended into the colon and was eventually p…

This review focuses on the pathophysiology of gastroesophageal reflux disease (GERD) and its implications for treatment. The role of the natural anti-reflux mechanism (lower esophageal sphincter, esophageal peristalsis, diaphragm, and trans-diaphragmatic pressure gradient), mucosal damage, type of refluxate, presence and size of hiatal hernia, Helicobacter pylori infection, and Barrett's esophagus are reviewed. The conclusions drawn from this review are: (1) the pathophysiology of GERD is multifactorial; (2) because of the pathophysiology of the disease, surgical therapy for GERD is the most a…

We report two new cases of gallbladder volvulus. This rare condition principally affects elderly patients but is due to a congenital malformation related to the presence of a long mesocyst. Various factors such as kyphoscoliosis, multiparity and peristalsis of neighbouring organs favorise the occurrence of volvulus, the role of lithiasis is debatable. The diagnosis may be suggested on ultrasound but is most often confirmed at laparotomy. Diagnosis should be made rapidly in order to avoid progression towards a potentially fatal secondary peritonitis. Certain clinical elements such as the presen…

A morphologic study of the duodenal papilla was performed by laminations in three planes. The muscle system reveals that there are no autonomic sphincters, but muscle bundles deriving from duodenal longitudinal and circular muscle layers, interdigitating and acting for opening of the papillary mouth. Activity results from myogenous rhythmic contractions of smooth muscle cells, stimulated by the ductal hydrostatic pressure, and from the duodenal peristalsis. The closure of the papilla is weak, incomplete and achieved by the oblique entrance of the duct. Interpretation of functional analysis and…

A gastroduodenostomy as part of partial gastric resection may change function and morphology of the duodenum in a typical fashion. End to side anastomosis may produce a blind pouch. An end to end anastomosis may lead to postanastomotic dilatation, which may mimick a duodenal bulb (a so called "neo -- or pseudobulb"). Abnormal duodenal peristalsis with atony may follow vagotomy, pyloroplasty, gastroduodenostomy or excisional duodenal biopsy and may thus result in functional obstruction. A blind loop syndrome may develop after bypass procedures for duodenal stenosis.

Esophageal diverticulum (ED) is a rare condition with a clinical presentation that can be variable. Esophageal diverticulum has been associated with motility disorders; however, the association with mid-ED is less clear. Hypercontractile esophagus, also known as jackhammer esophagus, is a rare motility disorder of peristalsis diagnosed by esophageal high-resolution manometry after exclusion of mechanical obstruction. We describe the second reported case of mid-ED secondary to hypercontractile esophagus successfully treated through robotic diverticulectomy with long myotomy. KEYWORDS: esophagus…

Epidermolysis bullosa encompasses a group of rare disorders typified by blister formation following minor trauma to the skin. Gastrointestinal tract involvement may occur in the extremely rare recessive dystrophic variants. The gastrointestinal manifestations present in 25 patients with the Hallopeau-Siemens variant of recessive dystrophic epidermolysis bullosa are reviewed. In the oesophagus both anatomical and motility abnormalities were observed. Features seen included a generally shortened oesophagus, strictures including those resembling webs, hiatus herniae, decreased peristalsis, oesoph…

Esophagogastric junction outflow obstruction (EGJOO) is an abnormal finding on high-resolution manometry (HRM) characterized by an elevated median integrated relaxation pressure with some intact peristalsis.1 EGJOO is associated with heterogeneous symptoms, disease course, and response to treatment.1-4 It can be idiopathic or secondary with causes including malignancy, infiltrative disease, or structural etiology.1,3 Therefore, Chicago Classification of Esophageal Motility Disorders version 3.0 (CC v3.0) states a finding of EGJOO should prompt further investigation with cross-sectional imaging…

Multipeaked waves (MPW) in the distal esophagus occur frequently in patients with esophageal spastic motor disorders and diabetes mellitus and are thought to represent repetitive esophageal contractions. We aimed to investigate whether the relative motion between a stationary pressure sensor and contracted peristaltic esophageal segment that moves with respiration leads to the formation of MPW. We mathematically modeled the effect of relative movement between a moving pressure segment and a fixed pressure sensor on the pressure waveform morphology. We conducted retrospective analysis of 100 sw
