Primary peritonitis due to group A streptococcus.

Primary peritonitis is a rare condition occurring, by definition, in patients without underlying causes, such as perforated viscus, pre-existing ascites, or nephrosis. We report a case of primary peritonitis and shock due to group A beta-hemolytic streptococcus, a rare etiology. A review of the world's literature shows a predilection for women to have this condition. The entry site is obscure in most cases. Asymptomatic genital tract colonization may be a portal of entry in some women. Shock or toxic shock syndrome often accompany the abdominal findings. Laparotomy to exclude a perforated visc…

[The treatment of appendicular peritonitis in children].

The experience of treatment of 513 children with an acute destructive appendicitis, complicated by peritonitis, was presented. The guided laparostomy conduction in patient in terminal state had promoted the frequency lowering of the postoperative complications occurrence and of the patients deaths. In 11 patients, whose state was estimated as extremely severe, laparostomy with double bottom according to classical method was performed. Of them 8 had recovered, 3 died, in 7 patients the complications occurred: the intestinal loops eventration--in 2 and postoperative infiltrate--in 5. The prolong…

Possible development of idiopathic sclerosing encapsulating peritonitis.

We report a rare case of idiopathic sclerosing encapsulating peritonitis (SEP). During a laparotomy before undergoing a distal gastrectomy with Billroth II reconstruction for early gastric cancer, the patient was found to have a membranous encapsulation wrapping each small bowel loop, unlike peritoneal encapsulation or typical SEP. He had complained of persistent heartburn, distension and diarrhea for 2 months in the post-operative course. The second laparotomy, which was performed to improve prolonged transit, revealed typical SEP with a thick and fibrotic membrane that encased the small bowe…

Spontaneous bacterial peritonitis in fulminant hepatic failure.

Ascites may be associated with fulminant hepatic failure (FHF), but spontaneous bacterial peritonitis (SBP) is an extremely rare complication. We report on two patients with FHF who developed SBP. One patient died and the other recovered.

Fetal parvovirus B19 infection and meconium peritonitis.

A 33-year-old primigravida at 26 weeks gestation presented with fetal hydrops and fetal anemia following prior parvovirus B19 infection. The fetus required two intrauterine transfusions of packed red cells. At 35 weeks gestation, a cesarean section was performed for obstetric reasons. As a consequence of a prenatal bowel perforation, the neonate developed meconium peritonitis, for which she needed laparotomy. This case demonstrates that there may be an association between intrauterine parvovirus infection and meconium peritonitis, the latter possibly caused by vascular injury in fetal life.

[Spontaneous bacterial peritonitis caused by Listeria monocytogenes].

We present a 68 year old male with alcoholic cirrhosis that was admitted with abdominal pain and fever. Hepatocarcinoma and spontaneous bacterial peritonitis by Listeria monocytogenes was diagnosed. The patient was treated with ampicillin and tobramycin during 25 days following a favorable course although ascitic fluid remained abnormal during 21 days. It is noted the rarity of Listeria as a cause of bacterial peritonitis in cirrhotic patients although they are immunodeficient. It is also important to establish the etiological origin because standard treatment of spontaneous bacterial peritoni…

The management of colorectal perforation and peritonitis.

BACKGROUND: Surgical outcomes in patients presenting with colonic perforation or peritonitis tend to be poor. This study was undertaken to determine outcomes in such patients at a time before multiple re-laparotomies were performed. METHODS: Retrospective analysis of computer records of all patients presenting acutely to the University Surgical Unit (Wellington School of Medicine) with colonic perforation or peritonitis over a 15-year period. RESULTS: Seventy-three patients, 33 males and 40 females were admitted with either perforation or localized peritonitis of colorectal origin. Of these,…

[Laparoscopic treatment of appendiceal peritonitis in adults].

UNLABELLED: The aim of this study was to evaluate the results of laparoscopic treatment of appendicular peritonitis. PATIENTS AND METHODS: From January 1991 to December 1994, 32 patients (16 men and 16 women with a mean age of 43 years) underwent emergency laparoscopy for a clinical diagnosis of localized or generalized appendicular peritonitis. All patients had double antibiotic therapy for at least 7 days. The laparoscopic appendectomy technique consisted of:insufflation to 12 mmHg, introduction of 3 trocars, first peritoneal lavage, coagulation of the mesoappendix, ligature of the base of …

[Local treatment of peritonitis by aspiration drainage].

Aspiration pelvic drainage in combination with naso-intestinal drainage were used in 783 patients with various forms of peritonitis. 489 patients had local peritonitis. There was no mortality in this group of patients. 294 patients had general peritonitis, the mortality rate in this group 6,5% (19 patients). In 34 cases the re-do surgery has been performed (13 patients died). Aspiration drainage is recommended for the treatment of various forms of peritonitis.

Cirrhosis: a risk factor for cryptococcal peritonitis.

Cryptococcal peritonitis is usually associated with end-stage renal disease and peritoneal dialysis. Significant liver disease has not been well recognized as a risk factor for its development. We report two patients with cirrhosis who developed peritoneal infections with Cryptococcus neoformans. We also retrospectively review all cases of cryptococcal illness at the Ohio State University Medical Center from October 1990 to January 1994 and present a review of the literature regarding cryptococcal peritonitis associated with hepatic dysfunction. Cirrhotic patients with this entity present with…

[Meconium peritonitis: intrauterine follow-up--postnatal outcome].

In nine patients with meconium peritonitis prenatal ultrasonographic findings were correlated with the clinical course and outcome. Ultrasound findings included polyhydramnion (n = 4), ascites (n = 4), disseminated (n = 3) and solitary echogenic areas (n = 1), echopoor cystic areas (n = 3) and echogenic-echopoor solitary areas (n = 1). Intra-abdominal calcifications were found in five patients before delivery. Eight neonates survived and were subsequently followed up, one fetus died in utero. Four of the eight survivors required surgery, namely for meconium ileus (n = 1), perforation secondary…

[Spontaneous bacterial peritonitis due to Listeria monocytogenes].

We present a case of spontaneous bacterial peritonitis (SBP) caused by Listeria monocytogenes in a patient previously diagnosed as alcoholic liver cirrhosis. The clinical presentation, biochemical data and outcome of the patient are compared with those of cases of SBP caused by Listeria monocytogenes in patients with cirrhosis published in the Spanish and English literature. Twelve out of 20 cases described in the literature were published by Spanish authors. This greater proportion could be related to dietary habits (greater consumption of fruits and vegetables), climatic or demographic facto…

[Spontaneous bacterial peritonitis caused by Listeria monocytogenes].

Listeria monocytogenes is a gram-positive coccobacillus that produces infections in both the normal and the compromised host. Symptomatic bacteremia and pulmonary infection or meningitis are the most common clinical presentations in adults. According to a current review of the literature, Listeria is a rare bacteria that may produce spontaneous bacterial peritonitis (23 cases reported). Listeria peritonitis occurs in more than two-thirds of the cases in patients with chronic liver disease, but may also occur in patients with malignancy or undergoing peritoneal dialysis. We describe two cases o…

[Intensive care management of diffuse septic peritonitis].

The function of the peritoneum in severe abdominal sepsis involves specific properties and defence mechanisms: large surface, efficient barrier, continuous mobility, capacity of migration, multiplication, secretion and absorption. The development of peritonitis supposes an initial lesion of the peritoneal surface by septic or biochemical mechanism. Peritoneal response consists of: septation of the abdominal wall, adhesion of the omentum to damaged surfaces or visceral perforation, massive stepping up of cellular and humoral defence mechanism. In our opinion therapeutic procedures consist of: e…

[Various aspects of hormonal disorders in peritonitis].

Impairment of hemodynamics and metabolism, which is determined by the condition of the hypothalamo--hypophyseal-adrenal system, is very important in the pathogenesis of peritonitis. The cortisol content, the activity of plasma renin, aldosterone, T3 and T4 were studied in 50 patients with diffuse and generalized peritonitis. The regional control group was formed of 50 practically healthy individuals. The patients were examined on the 1st, 3rd, 7th, and 14th postoperative days. The "low T3" syndrome in considerable increase of the content of cortisol, which inhibits the production of triiodothy…

[Peritonitis and cerclage of the cervix uteri].

Authors report a case of peritonitis following uterine cervical cerclage in a patient with twins. This observation leads to two problems: complications of cerclage and its real indications. Two recent controlled studies conclude that in a singleton pregnancy, there is no advantage in cervical cerclage in prolonging the gestation period and in decreasing the perinatal mortality. However, the last MRC/RCOG study, still in progress, shows that in a preliminary study, there is a small advantage in cervical cerclage in high-risk-pregnancy with two previous mid trimester spontaneous abortions and/or…
