Economic costs of obesity and inactivity.

PURPOSE: The purpose of this paper is to assess the economic costs of inactivity (including those attributable to obesity). These costs represent one summary of the public health impact of increasingly sedentary populations in countries with established market economies. Components of the costs of illness include direct costs resulting from treatment of morbidity and indirect costs caused by lost productivity (work days lost) and forgone earnings caused by premature mortality. METHODS: We searched the Medline database for studies reporting the economic costs of obesity or inactivity, or cost

Prevalence of cholinergic urticaria in young adults.

BACKGROUND: Cholinergic urticaria is primarily seen in young adults, but little is known about its prevalence. OBJECTIVE: A prospective study was designed to investigate the frequency and severity of cholinergic urticaria in this age group. METHODS: Four hundred ninety-three high school and university students (15 to 35 years of age) were examined for cholinergic urticaria by means of a questionnaire and partly by exercise provocation tests. RESULTS: The overall prevalence of cholinergic whealing was 11.2%, with most of the affected persons being older than 20 years. The highest prevalence

Comparison of aerobic fitness and space motion sickness during the shuttle program.

Space motion sickness (SMS) is an important problem for short duration space flight and 71% of STS crewmembers develop symptoms. The search for effective countermeasures and factors that correlate with sensitivity has been extensive. Recently, several investigators have linked aerobic fitness with motion sickness sensitivity in the 1-G or high-G environment. This paper compares the aerobic fitness of 125 Shuttle crewmembers with their SMS symptom category. Aerobic fitness data were obtained from the exercise tolerance test (ETT) conducted nearest the time of launch. SMS data were derived from

Carbamates, atropine, and diazepam: effects on performance in the running rat.

We have reported that when rats (500 g, male) are exercised to exhaustion on a treadmill, pretreatment with the centrally acting carbamate physostigmine reduced endurance (run time, RT) and increased the rate of rise of core temperature (Tc+). Both RT and Tc+ were restored to control levels by pretreatment with either or a combination of atropine (A), and diazepam (D). Our objective in the present work was to determine whether A+D could also restore the performance and thermoregulatory decrements induced by the peripherally acting carbamate pyridostigmine (PY). After drug administration, rats

Heat stroke in endurance exercise.

Six athletes were examined immediately after collapsing from heat stroke during exercise, and then followed for several weeks. At the time of collapse most of the patients were sweating profusely, their rectal temperatures being more than 42 degrees C. All recovered within a few hours. The renal function was not disturbed more than expected during heavy exercise, serum levels of liver enzymes were, however, increased for several weeks. Electrolyte homeostasis was undisturbed but for a transient hypercalcemia that can not be fully explained. The marked increments in plasma levels of catecholami

Effects of endurance exercise on metabolic water production and plasma volume.

Six endurance-trained and heat-acclimatized adult males ran for 1 h (or until exhaustion) at room temperature (23.8 degrees C) on three occasions. The work loads approximated 37, 56, and 74% of the subjects' aerobic capacities. Venous blood samples were drawn, and urine was collected before and immediately after each exercise bout. Metabolic cost was partitioned by energy substrate, and metabolic water production was quantified from urinary nitrogen, oxygen, and carbon dioxide production. Total body water loss was recorded as the decrease in body weight during the exercise. All subjects comple

Influence of naloxone on antidepressant drug effects in the forced swimming test in mice.

The influence of naloxone on the effects of several antidepressant drugs, atropine and caffeine was studied in the forced swimming test in mice. Naloxone itself has no effect in this test, but significantly reduces that of two tricyclic antidepressants, clomipramine (20 and 30 mg/kg) and desipramine (20 and 30 mg/kg). Except for clorgyline at the high dose of 60 mg/kg, no significant reduction of activity by naloxone was observed with other antidepressants (pargyline, nomifensine and mianserin), nor with caffeine and atropine. These results are discussed in terms of the pharmacological charact

Red cell hemoglobin, hydrogen ion and electrolyte concentrations during exercise in trained and untrained subjects.

Red cell concentrations of hemoglobin (MCHC), H+, Na+, K+, Mg++, cl- were measured in femoral venous blood of six untrained (UT), six endurance trained (TR) and three semitrained (ST) subjects during graded increasing work (4, 8, 12, 18 and 24 mkp/s, 10-15 min on each step) on a bicycle ergometer. Before exercise no significant differences were detected for the measured variables when comparing UT and TR. During exercise MCHC, [Na+], [K+] and [Mg++] remained constant indicating lack of water shift into the erythrocytes in spite of a marked acidosis (lowest pH Blood value 7.225). This lack resu

Evaluation of physical performance by rectangular-triangular bicycle ergometry and computer-assisted ergospirometry.

A great deal of information about physical adjustment to work can be obtained from quantitative stress testing. Maximal stress limited by symptoms of exertional intolerance is the concept of the 2 min duration work increment test (rectangular-triangular exercise test). Compared to steady-state work tests strict observation of the standardized procedure- and computer assisted evaluation of ergospirometric parameters offer innovatory opportunities: (1) the test is of short duration (8-14 min), (2) the subjects recover rapidly, even from an exhausting test, (3) one is more likely to be able to ob

Replacing sitting with light-intensity physical activity throughout the day versus 1 bout of vigorous-intensity exercise: similar cardiometabolic health effects in multiple sclerosis. A randomised cross-over study.

PURPOSE: Persons with Multiple Sclerosis (PwMS) are physically inactive and spend more time in sedentary behaviours than healthy persons, which increases the risk of developing cardiometabolic diseases. In this randomised crossover study, the cardiometabolic health effects of replacing sitting with light-intensity physical activity (LIPA) and exercise (EX) were investigated. MATERIALS AND METHODS: Twenty-eight mildly disabled PwMS performed four 4-day activity regimens in free-living conditions; CONTROL (habitual activity), SIT, LIPA, and EX. Plasma glucose and insulin (oral glucose tolerance

Heart Rate Variability as a Reliable Biomarker Following Concussion: A Critically Appraised Topic.

CLINICAL SCENARIO: Recent systematic reviews show conflicting information regarding the effect of concussion on cardiac autonomic function. Controlled aerobic exercise is the most popular intervention for those recovering from a concussion. There is a gap in the literature supporting the utility of objective metrics during exertional return to play protocols and rehabilitation. CLINICAL QUESTION: Can heart rate variability (HRV) during physical exertion be a reliable biomarker over time for those who suffered a sport-related concussion? SUMMARY OF KEY FINDINGS: A literature search produced 3…

Haemochromatosis, sinus node dysfunction and Brugada syndrome--a ménage a trois of findings in one and the same patient: coincidence or causality?

We describe the case of a 52-year-old male patient with a recently diagnosed hereditary haemochromatosis who was referred to our electrophysiology laboratory due to presyncopal spells during physical exertion. The electrophysiological study unexpectedly revealed a sinus node dysfunction as well as a Brugada syndrome--both diagnosed on the grounds of an ajmaline test.

Chronic idiopathic anhydrosis--a rare cause of heat stroke.

A 27 year old man presented with heat stroke following exposure to a humid, hot environment in the absence of physical exertion. Investigation revealed the presence of generalized anhydrosis without evidence of an associated disease. Although chronic idiopathic anhydrosis is rare, this entity should be considered in cases of unexplained heat intolerance and heat stroke.

Injuries, Medical Conditions, and Changes in Blood Levels in German Special Operations Forces Selection.

BACKGROUND: Medical conditions often develop during military training. The aim of this study was to compile medical conditions and injuries sustained during a 5-day military exercise, compare them with incidences at similar civilian events, and subsequently identify differences between those who finished the exercise (Finishers) and those who did not (Nonfinishers) to identify preventable causes for not finishing and to reduce unnecessary health risks. METHODS: Fifty-one soldiers had their blood parameters (creatine kinase [CK], aspartate transaminase [AST], alanine transaminase [ALT], gamma-

Women's Surgical Preferences in Stress Urinary Incontinence: Sling or Bulking Agent?

INTRODUCTION: The aim of the study was to evaluate which treatment women with mild-to-moderate stress urinary incontinence (SUI) chose after … home, do jobs from which they cannot be absent for a long time, and also are exposed to unavoidable physical exertion. Women who choose to perform a sling instead do jobs that do not involve physical exertion and thus can start again quickly, are married, and have help with household chores. Sixty-five percent choose a bulking agent to avoid complications. A total …

Cycling Induced Spontaneous Coronary Artery Dissection in a Healthy Male.

INTRODUCTION: Spontaneous coronary artery dissection (SCAD) is a rare but important cause of acute coronary syndrome with a spectrum of disease that can include unstable angina, acute myocardial infarction, or sudden cardiac death. It has also been found in case reports to be caused by shear stress from physical exertion. We present a rare cycling induced SCAD that occurred in our institution in an otherwise healthy male with no cardiac risk factors. CASE PRESENTATION: A 36-year-old male presented to the emergency department with complaints of lightheadedness and diaphoresis after a bicycle f…
