INTRODUCTION: Fluid overload is a major challenge in hemodialysis patients and might cause hypervolemia. We speculated that hemodialysis patients reaching dry weight could have undetected hypervolemia and low hemoglobin (Hb) concentration (g/dL) due to hemodilution.

The pattern of dilution and re-concentration of the blood in connection with intravenous infusion of saline-alone or in combination with bleeding-was studied in volunteers. In these we followed the immediate and delayed changes in 16 common haematological and clinical chemical variables in relation to such infusions. In one group 15 ml/kg was given in an hour. Rapid administration of intravascular volume expanders is often necessary during anesthesia. Significant controversy still exists on the relative values of different volume expanders. Fifteen hypoxemic patients (Pao2 less than 70 torr on room air) were studied preoperatively. They were randomized into three groups. We briefly review 20 years of clinical experience with 120.531 units of a 4% modified fluid gelatin as a plasma substitute. Its Mn is 22.500. The initial intravascular volume effect equals that of 4% albumin; the t0,5 of this effect is about 4 hours. It has no dosage-related side effects and specifically does not impair surgical hemostasis in volumes up to 10-15 litres within 24 hours. The effects of dextran 1.8% in Ringer's lactate solution were compared with infusion of Ringer's lactate alone in two groups of 12 patients after minor surgical interventions. Plasma volume increased significantly only in the dextran treatment patients. We observed a marked hemodilution with both infusions, but all changes had disappeared after 24 hours. No adverse effects on the coagulation process, hepatic and renal function were noted. These results suggest that dextran 1.8% in Ringer's lactate is an effective and safe solution in plasma volume expansion for this type of patient.
