Impact of pneumococcal vaccine response on asthma exacerbation frequency in young children.

INTRODUCTION: Chronic asthma is a heterogeneous disease, and increased eosinophils have been shown to predict increased asthma exacerbations, especially in adults. Recent recommendations suggest the need for supplemental PPV-23 vaccination in older children with chronic asthma. METHODS: To investigate differences in preschool asthma, our case-cohort study comprised of 127 children, mean age 47 months (32-65), with a history of asthma exacerbations requiring more than three courses of systemic steroid bursts and more than six antibiotics courses in the previous year. RESULTS: At baseline, mea

Effects of 10-valent pneumococcal conjugate (PCV10) vaccination on the nasopharyngeal microbiome.

Pathogenic bacteria, such as Streptococcus pneumoniae, Staphylococcus aureus, Haemophilus influenzae and Moraxella catarrhalis, are important vaccine targets. The 10-valent pneumococcal conjugate vaccine (PCV10) acts on 10 differents S. pneumoniae serovars. However, this vaccine could also act on other bacteria genera, leading to dysbiosis. Moreover, the vaccination has also been associated with imbalances in the ratio between commensal and potentially pathogenic bacteria. Despite the wealth of studies assessing the influence of the microbiome on vaccine effects, how vaccination can influence

The association between pneumococcal vaccination, ethnicity, and the nasopharyngeal microbiota of children in Fiji.

BACKGROUND: Streptococcus pneumoniae is a significant global pathogen that colonises the nasopharynx of healthy children. Pneumococcal conjugate vaccines, which reduce nasopharyngeal colonisation of vaccine-type S. pneumoniae, may have broader effects on the nasopharyngeal microbiota; however, data are limited. In Fiji, nasopharyngeal carriage prevalence of S. pneumoniae and other colonising species differ between the two main ethnic groups. Here, we examined the association between the 7-valent pneumococcal conjugate vaccine (PCV7) and the nasopharyngeal microbiota of children in Fiji, includ

Acute respiratory failure due to eosinophilic pneumonia following pneumococcal vaccination.

A 68-year-old woman under maintenance hemodialysis was admitted to our hospital with fever and dyspnea that had developed two days after the second vaccination with the 23-valent pneumococcal polysaccharide vaccine (PPV23). She had received the first vaccination with PPV23 five years earlier without any complications. Chest X-ray and computed tomography (CT) showed bilateral diffuse infiltrative and ground glass opacities. Bronchoalveolar lavage (BAL) revealed an abundance of eosinophils. A positive test result for PPV23 was obtained in the drug lymphocyte stimulation test. The patient was dia

Seven-valent pneumococcal conjugate vaccine and nasopharyngeal microbiota in healthy children.

Seven-valent pneumococcal conjugate vaccine (PCV-7) is effective against vaccine serotype disease and carriage. Nevertheless, shifts in colonization and disease toward nonvaccine serotypes and other potential pathogens have been described. To understand the extent of these shifts, we analyzed nasopharyngeal microbial profiles of 97 PCV-7-vaccinated infants and 103 control infants participating in a randomized controlled trial in the Netherlands. PCV-7 immunization resulted in a temporary shift in microbial community composition and increased bacterial diversity. Immunization also resulted in d

Orbital complications of acute sinusitis: changes in the post-pneumococcal vaccine era.

IMPORTANCE: The widespread use of the 7-valent pneumococcal conjugate vaccine (PVC7), developed to combat invasive Streptococcus pneumoniae infections, has the potential to influence the prevalence and antibiotic resistance patterns of pathogens associated with orbital complications from acute sinusitis. Given the significant morbidity that may result from inadequate treatment of orbital infections related to acute sinusitis, determining the impact of PCV7 on the bacteriology and drug resistance of the pathogens associated with these infections may provide critical information needed to accura

Protracted fever with cellulitis-like reaction in pneumococcal polysaccharide-vaccinated children.

Side effects of the 23-valent pneumococcal polysaccharide vaccine (PPV23) are regarded as mild and uncommon in adults. In our clinical experience, however, protracted fever with a cellulitis-like reaction after PPV23 vaccination is not rare in pediatric patients. We reviewed the records of 17 children with those clinical findings after PPV23 vaccination.

Using registry data to evaluate the 2004 pneumococcal conjugate vaccine shortage.

BACKGROUND: The most recent pneumococcal conjugate vaccine (PCV7) shortage occurred between December 2003 and September 2004. To ensure vaccination of the highest-risk children, the Centers for Disease Control and Prevention recommended that providers delay administration of the third and fourth doses of vaccine to healthy children. We used Michigan Child Immunization Registry (MCIR) data collected from September 1, 2001 to November 30, 2004 to evaluate changes in PCV7 coverage. METHODS: Vaccination and demographic data from MCIR were reviewed for 420,733 children born between September 2001

Effect of introduction of the pneumococcal conjugate vaccine on drug-resistant Streptococcus pneumoniae.

BACKGROUND: Five of seven serotypes in the pneumococcal conjugate vaccine, introduced for infants in the United States in 2000, are responsible for most penicillin-resistant infections. We examined the effect of this vaccine on invasive disease caused by resistant strains. METHODS: We used laboratory-based data from Active Bacterial Core surveillance to measure disease caused by antibiotic-nonsusceptible pneumococci from 1996 through 2004. Cases of invasive disease, defined as disease caused by pneumococci isolated from a normally sterile site, were identified in eight surveillance areas. Iso
