Successful Kidney Transplantation Under Combined Spinal-Epidural Anesthesia in a Patient With a History of Left Pneumonectomy Due to Bronchiectasis.

BACKGROUND Kidney transplantation offers clear survival and quality-of-life advantages over dialysis, but severe pulmonary disease can complicate perioperative planning. In patients with substantially reduced respiratory reserve after pneumonectomy, general anesthesia may be considered high risk, creating uncertainty about transplant feasibility. CASE REPORT A 33-year-old man with end-stage kidney disease secondary to biopsy-proven AA amyloidosis was evaluated for preemptive living-donor kidney transplantation. He had undergone left pneumonectomy in 2015 because of bronchiectasis. Pulmonary fu…

Indications, contributing factors, and short-term outcomes of pneumonectomy: an 8-year retrospective study in a resource-limited setting.

BACKGROUND: Pneumonectomy, the surgical removal of an entire lung, was first performed in 1933 by Evarts A. Graham for lung carcinoma. Today, pneumonectomy is primarily indicated for non-small cell lung cancer (NSCLC) worldwide. However, it carries a higher risk of morbidity and mortality compared to less extensive lung resections. OBJECTIVES: This study aims to investigate the indications for pneumonectomy and its short-term outcomes among patients who underwent the procedure between 2016 and 2023. METHODS AND MATERIALS: A retrospective study was conducted on 112 patients who underwent pneu…

Successful Endobronchial Valve Placement in the Treatment of Persistent Bronchopleural Fistula and Empyema Allows the Avoidance of Right Completion Pneumonectomy.

INTRODUCTION: This case report addresses the complexity of management of air leak and persisting infection in polymorbid patients. CASE PRE … bronchopleural fistula. The only possible surgery that would cure the patient was right completion pneumonectomy. To avoid this high-risk operation, an endobronchial valve was placed intraoperatively in the posterior segment bronchus, leading to closure of the fistula and resolution of the infection. The patient recovered well and was discharged 10 days later. …

The Influence of Airway Closure Technique for Right Pneumonectomy on Wall Tension During Positive Pressure Ventilation: An Experimental Study.

Bronchopleural fistula (BPF) remains a significant source of morbidity and mortality after right pneumonectomy (RPN). Postoperative mechanical ventilation represents a primary risk factor for BPF. We undertook an experiment to determine the influence of airway diameter on suture line tension during mechanical ventilation after RPN. RPN was performed in 6 fresh human adult cadavers. After initial standard bronchial stump closure (BSC), the airway suture lines were subjected to 5 cm H2O incremental increases in airway pressures beginning at 5-40 cm H2O. To minimize airway diameter, a carinal res…

Successful implantation of the Edwards Sapien THV via direct aortic access in a patient with previous pneumonectomy and no other access.

The Edwards Sapien transcatheter heart valve (THV) is available for treatment of severe symptomatic aortic stenosis via transfemoral access or trans-apical access when ilio-femoral vessels are considered unsuitable. Some patients, however, may be unsuitable for access via either of these routes. We describe successful implantation of an Edwards Sapien THV via direct access of the ascending aorta in a patient with previous right pneumonectomy and pulmonary disease who was considered unsuitable for implantation via femoral vessels or via trans-apical approach. The potential advantages of this no…

A Rare Case of Early-Onset Postpneumonectomy Syndrome After Right Pneumonectomy in Patient with Small Cell Carcinoma Right Lung.

Postpneumonectomy syndrome (PPS) is a rare, life-threatening complication characterized by dynamic airway obstruction due to mediastinal rotation at any time point following pneumonectomy. This can produce life-threatening respiratory and cardiovascular complications. We report a case who developed PPS following right pneumonectomy in a 55-year-old female patient with small cell carcinoma (SCC) right lung.

Quantitation and predictors of short-term mortality following extrapleural pneumonectomy, pleurectomy/decortication, and nonoperative management for malignant pleural mesothelioma.

BACKGROUND: For malignant pleural mesothelioma (MPM), the benefit of resection, as well as the optimal surgical technique, remain controversial. In efforts to better refine patient selection, this retrospective observational cohort study queried the National Cancer Database in an effort to quantify and evaluate predictors of 30- and 90-day mortality between extrapleural pneumonectomy (EPP) and pleurectomy/decortication (P/D), as well as nonoperative management. METHODS: After applying selection criteria, cumulative incidences of mortality by treatment paradigm were graphed for the unadjusted …

Left sleeve pneumonectomy performed through a clamshell incision with extracorporeal membrane oxygenation for bronchogenic carcinoma: report of two cases.

We report herein the cases of two patients with bronchial gland carcinomas in the left main bronchus who were successfully treated by left sleeve pneumonectomy performed through a clamshell incision. Adequate oxygenation during pathological examination of tumor invasion at the bronchial stumps and the tracheobronchial anastomoses was achieved by extracorporeal membrane oxygenation (ECMO) in both patients. Tracheobronchial anastomosis was relatively easy to perform in the excellent operative field achieved by this method. The postoperative courses of both patients were uneventful, although they…

Is pneumonectomy still relevant for non-small cell lung cancer? Long-term overall survival from a 15-year experience.

BACKGROUND: Pneumonectomy has long been shown to be curative for lung cancer but may lead to several acute and chronic adverse physiologic changes. Limited data are available on long-term follow-up of patients who have undergone this procedure. We sought to evaluate whether pneumonectomy is still a valuable therapeutic procedure when it is the only curative approach available and to report on long-term survival after the procedure as well as operative mortality and morbidity. METHODS: The primary outcome of our study was long-term overall survival after pneumonectomy. Secondary outcomes were …

Unforeseen pulmonary destruction following hepatic hydatid cyst fistulization: A case report of emergency pneumonectomy in a critically ill patient.

Direct fistulization of a hepatic hydatid cyst into the lung, causing rapid pulmonary destruction, is exceedingly rare. We report a 41-year-old male presenting with acute asphyxia due to a complicated hepatic hydatid cyst with a giant transdiaphragmatic fistula into the right lung, leading to complete lung destruction and massive tracheobronchial aspiration. Emergency right pneumonectomy was performed for irreversible lung damage. Intraoperative findings confirmed heavy adhesions, lung destruction, and an extensive scolex-laden fistula. The postoperative course was challenging, complicated by …

Patterns of failure, toxicity, and survival after extrapleural pneumonectomy and hemithoracic intensity-modulated radiation therapy for malignant pleural mesothelioma.

INTRODUCTION: We investigated safety, efficacy, and recurrence after postoperative hemithoracic intensity-modulated radiation therapy (IMRT) in patients with malignant pleural mesothelioma treated with extrapleural pneumonectomy (EPP), during the past decade at a single institution. METHODS: In 2001-2011, 136 consecutive patients with malignant pleural mesothelioma underwent EPP with planned adjuvant IMRT. Eighty-six patients (64%) underwent hemithoracic IMRT; the rest were not eligible because of postoperative complications, disease progression, or poor performance status. We assessed toxici…

Heart failure following left-sided pneumonectomy in a patient with known pectus excavatum -- successful treatment using the Ravitch procedure.

A rare case of a patient with progressive dyspnoea due to atrial compression between ascending and descending aorta is demonstrated. After neoadjuvant chemoradiation for a locally advanced nonsmall cell lung cancer stage IIIb, he had a left-sided pneumonectomy. The underlying problem for cardiac compression was the extreme mediastinal shift reinforced by a congenital pectus excavatum. Our treatment was a Ravitch procedure with fair result.
