Histoplasmosis is a rare, but important, disease process in patients with the acquired immune deficiency syndrome (AIDS). Initially reported in this patient population in 1982, over 100 cases of histoplasmosis have been reported throughout the United States. Most of these patients present with disseminated disease. Untreated, histoplasmosis will be the direct cause of death in over 80% of patients, but treatment with itraconazole can reduce this mortality to less than 25%. We present two cases of histoplasmosis in AIDS patients presenting as pneumoperitoneum.

A new sign of pneumoperitoneum was observed in a patient with perforation of the ascending colon. The presence of free air in the peritoneal cavity allowed identification of the transverse mesocolon and the root of the small bowel mesentery on plain abdominal radiographs obtained in the supine and in the prone position. This sign may be an aid in the diagnosis of pneumoperitoneum.

Postoperative ileus (POI) is a relatively common complication of abdominal surgery that typically resolves with conservative treatment. The current Covid-19 pandemic has been the most discussed topic of the year, mostly about protection and ways to avoid dissemination of the virus. In the healthcare system, especially in the operating rooms, the viability of laparoscopic surgery was questioned, mostly because of the transmission through aerosol. This article tries to suggest a way to minimize risks of laparoscopic surgery, during this situation, by using electrostatic filters, a simple, effective and low cost alternative.
