BACKGROUND: Cervical cancer (CC) is the most common and second-most deadly cancer among Peruvian women, despite national authorities' sustained political and budgetary efforts to improve Human Papillomavirus (HPV) vaccination, screening for early detection of cervical cancer, and making treatment more accessible. Considering the importance of the health system in CC prevention and control, this article examines the role of the health system as a whole in the CC screening in Peru, a dimension that previous studies have not comprehensively covered. To this end, this article utilizes the WHO's bu

BACKGROUND: Tajikistan has accomplished reductions in maternal, newborn, and child mortality over the past decades through targeted policies and interventions. Challenges remain in providing quality healthcare due to limited resources, geographic barriers, and inadequate infrastructure. We aimed to evaluate the impact of a quality improvement (QI) initiative implemented in ten district hospitals from 2021 to 2024 to improve maternal, newborn, and childcare to accelerate progress towards achieving the Sustainable Development Goals.
METHODS: A baseline assessment was conducted in 2021, with an

Environmental disasters are inherently political phenomena, shaped by systemic inequalities and entrenched power dynamics. This article analyzes how the securitization paradigm and slow violence frameworks explain the disproportionate impact of Hurricane Katrina on marginalized communities. While the territorial securitization paradigm focuses on reactionary military preparedness, it fails to address the long-term systemic issues-such as racism, socioeconomic inequality, and inadequate -infrastructure-that exacerbate vulnerability to environmental events. Using qualitative case study analysis,

Including local populations in energy transition planning can increase its legitimacy, socio-political feasibility, and, in turn, climate mitigation efficacy. Comprehensive stakeholder involvement in participatory modeling facilitates learning; however, the choice of options and scenarios in the model can be opaque to participants. Here, we present a framework in which stakeholders engage with near-optimal modeling results holistically and showcase our methodology in a case study of the remote Arctic settlement of Longyearbyen, Svalbard. Participants choose across a continuum of modeling-based

Herbert Dorken was born in Montreal, Canada, on July 7, 1926, and died in Grass Valley, California, on February 18, 2012, at the age of 85. Dorken was a prolific writer, with nearly 100 publications to his credit. He was particularly adept at taking raw health care data that the government had assembled with no apparent interpretation and translating it so that its meaning and implications for health care practice were clear. He strongly believed that the health care system in America, and particularly its mental health system, was in need of extensive reform. At the invitation in the mid- 198

Clozapine is known as the first 'atypical' medication and is effective in people who have treatment-resistant schizophrenia. Its 1990 emergence in the USA was marked by considerable controversy over its high cost, due in large part to having been both the first new antipsychotic medication to come to market in over a decade and the need for comprehensive safety monitoring within a decentralized health system. This paper traces the history of clozapine's discovery and development in Europe, its part in the 1975 Finnish agranulocytosis scare, and its subsequent volatile emergence in the USA. Ana

Marriage and family therapy emphasizes systemic responsiveness, yet integrating advocacy into clinical training remains limited. This pilot study examines a trauma-informed service-learning partnership with a community-based organization, where graduate students conducted supervised forensic evaluations for asylum seekers. Using a phenomenological case study design with mixed-methods follow-up, we explored how direct engagement with survivors of politically motivated persecution shaped students' professional development, cultural humility, and advocacy skills. Upon the 1-year follow-up partici

Climate change reflects social, political, and ecological harms that shape clinical care and health inequities. This article offers a justice-oriented, public health-informed framework for clinical nursing practice, showing how climate-related exposures and structural vulnerability appear in patient presentations, care transitions, and recovery. Drawing on structural competency, Cultural Safety, planetary health ethics, and the Global Nurse Agenda for Climate Justice, the article illustrates how nurses can integrate environmental assessment, advocacy, and system stewardship into routine care.

Cognitive behavioral therapy (CBT) rests on the premise that individuals possess personal agency to change their life circumstances. At the same time, given growing political homogeneity within psychology, views of individual agency are increasingly being displaced by greater emphasis on conceptualizing distress as reflecting systemic oppression. Drawing on research concerning these frameworks, this article briefly examines the implications of these trends for CBT in the United States. Specifically, we outline three potential concerns, including declining interest in traditional CBT, reduced v

The COVID-19 pandemic continues to affect most aspects of daily life, and looking for ways to cope and adapt in this altered state is a priority. Days of unsettling changes have turned into weeks, months, and, most likely, at least a year or more until an effective vaccine is distributed worldwide. COVID-19 has disrupted societies across the world, with a global scope that is unprecedented, ongoing, and without a demarcated end. Combined with the political turmoil related to the presidential election in the United States, environmental turmoil including widespread fires, and ongoing structural

The history of mental disorders occasioned by World War I is a complex and important history, indelibly linked with social, political and cultural circumstances, and the history of the war itself. The Richmond War Hospital was a 32-bed establishment on the grounds of the large Richmond District Asylum in Dublin which, from 16 June 1916 until 23 December 1919, treated 362 soldiers with shell shock and other mental disorders, of whom more than half were considered to have recovered. Despite the limitations of the Richmond War Hospital, it was a generally forward-looking institution that pointed

Underreporting of infectious diseases is a pervasive challenge in public health that has emerged as a central issue in characterizing the dynamics of the COVID-19 pandemic. Infectious diseases are underreported for a range of reasons, including mild or asymptomatic infections, weak public health infrastructure, and government censorship. In this study, we investigated factors associated with cross-country and cross-pathogen variation in reporting. We performed a literature search to collect estimates of empirical reporting rates, calculated as the number of cases reported divided by the estima

BACKGROUNDS AND AIMS: Iran has 2.1 and 1.8% of its 15-64-year-old population living with illicit substance and opioid use disorders, respectively. To address these problems, Iran has been developing a large and multi-modality addiction treatment system, spanning the time before and after the Islamic Revolution.
METHODS: Iran's current drug treatment scene is a combination of services, ranging from medical/harm reduction services to punitive/criminal justice programs. Included in this array of services are drop-in centers providing low-threshold harm reduction services, such as distribution of

Patio 29 lies in the northern sector of Santiago's General Cemetery. To the naked eye, it is a grim unweeded field of some twelve hundred rusted tin crosses. But to the families of the 1,197 detained-disappeared during Augusto Pinochet's brutal dictatorship, Patio 29 is both a site of horror and a site of hope. Its story begins in September-December 1973 when 320 early victims of the repression were brought there in makeshift wooden crates that held as many as three bodies each, and buried in unmarked graves. A few years later, two hundred of those graves were exhumed by the military, and the

In the United States and Germany before World War II, physicians participated in state-authorized eugenic sterilization programs in an attempt to prevent persons deemed to possess undesirable heritable characteristics from propagating. A comparison of U.S. and German histories reveals similarities that argue against easy dismissal of a Nazi analogy. On the basis of a review of editorials in New England Journal of Medicine and Journal of the American Medical Association from 1930 to 1945 it is difficult to accept the suggestion that the alliance between the medical profession and the eugenics m

BACKGROUND: Compulsory service programs are used to improve access to health professionals in underserved areas. Their implementation depends on workforce conditions, organizational support, and the wider policy environment. Specialist services such as ophthalmology likewise rely on functioning clinical infrastructure, trained support staff, and referral capacity. This study aimed to identify the main factors and driving forces shaping the future of Iran's compulsory service program for healthcare workers in underserved areas, drawing on ophthalmologists' experiences and an expert-panel assess
