We experienced two cases of priapism; one, idiopathic and low flow type, the other post-traumatic and high flow type. We tried caverno-spongio-anastomosis (CSA) as an operative treatment in both cases. Postoperative courses were uneventful. We suggest that CSA is a simple and reliable treatment of priapism.

Numerous reports have emphasized the association between priapism and the ingestion of psychotropic medication. Clinicians are becoming increasingly aware of this association and its subsequent severe morbidity. Review of the literature reveals that medications possessing alpha-adrenergic blocking properties are most frequently associated with priapism. A case is reported of a man 60 years of age with degenerative stenosis of the lumbar canal at the L3-4 level and lumbar arachnoiditis, whose symptoms of claudication of the cauda equina were accompanied by uncomfortable involuntary erections. All symptoms were relieved by surgical decompression.

Priapism is exceptional before puberty and a new case is here related. Facing the failure of the medical treatment, a caverno-saphenous shunt is realized at the 72e hour. In connection to this case, we present a review of etiological and pathological knowledges applying to this anomaly of erection. An early surgical treatment is obviously necessary to prevent fibrosis, ground for later impotence. Likewise, we must insist on the usually temporary practice of this accident which may lead to discuss about the suppression of the shunt in some secondary deficiency of erection.

The treatment of priapism has changed significantly because of better understanding of the physiology of erection and of the pathophysiology of the disease. Several operative procedures have been advised to provide better venous drainage to the corpora. Herein we describe our experience with 20 patients. In 7 cases a modification of the cavernospongiosum shunt was used. This shunt is done under direct vision at the level of the proximal glans, thus, providing a better cavernosum-spongiosum shunt.

A new surgical procedure for idiopathic priapism has been used successfully in patients. In 2 of these patients the procedure was done after 5 days of persistent or recurrent erections. The method consists of a combination of external polyethylene shunts between the corpora cavernosa and the venous systems of the underarms drip infusion of a heparin-saline solution into the corpora and prompt systematic anticoagulant therapy. All 3 patients were cured of the priapism and recovered normal sexual powers.
