SAPHO syndrome is a disorder characterized by Synovitis, Acne, Pustulosis, Hyperostosis, and Osteitis. As the osteoarticular and skin manifestations often do not occur simultaneously and there are no validated diagnostic criteria, the diagnosis can be difficult. Clinical and imaging investigation is necessary to establish the many differential diagnoses of SAPHO syndrome. The etiopathogenesis involves infectious (probably Propionibacterium acnes), immunologic, and genetic factors. Benzoyl peroxide is one of the most widely used topical agents for acne. It has potent antibacterial and mild anti-inflammatory and comedolytic effects. To treat mild to moderate acne, it can be used alone or in combination with topical antibiotics and topical retinoids. The combination of benzoyl peroxide with either erythromycin or clindamycin is synergistic and well-tolerated. In more severe acne, when oral antibiotics are required, benzoyl peroxide can contribute to suppressing the emergence of resistant strains of Propionibacterium acnes.

The respiration of different species of propionic acid bacteria was found to be resistant to the inhibiting action of cyanide. Propionibacterium globosum, P. globosum (emm), P. vannielii, P. shermanii, P. petersonii, P. pentosaceum and P. raffinosaceum were shown to contain superoxide dismutase, catalase and peroxidase. The highest activity of catalase and superoxide dismutase was found in P. globosum and P. vannielii, the lowest one in P. shermanii and P. raffinosaceum. Peroxidase was detected in propionic bacteria for the first time.

An investigation was done to determine the incidence of postdefecation bacteremia. A study of 82 healthy volunteers was done. Among 164 predefecation cultures, only one (0.6%) from an anaerobic flask was positive for microorganism, which was identified as Staphylococcus epidermidis. In only two of 328 (0.6%) postdefecation cultures did microorganisms grow, which were shown to be Propionibacterium acnes. The results of this study suggest that either bacteremia after defecation does not occur or is a rare event in healthy individuals.

Two rapidly growing propionibacteria that could reductively dechlorinate tetrachloroethylene (PCE) and cis-1,2-dichloroethylene (cis-DCE) to ethylene were isolated from environmental sediments. Lipophilic propionibacterium acnes stores large amounts of porphyrins. This peculiarity can be used for photodynamic destruction of propionibacteria. After repeated irradiations with a blue light-type high pressure lamp (Narva NC 1000-64) both acne and seborrhoea was improved markedly (10 irradiations, each for 10 minutes, cumulative light dose 325 J/cm2). Also porphyrin content inside acne follicles dropped. Serial irradiations with short range visible light (400 to 420 nm) are recommended for acne treatment during the light-poor season of the year.

We present a case of vertebral osteomyelitis following vertebroplasty in a patient with acne of the back. Vertebral biopsy revealed Corynebacterium species and blood cultures were positive for Propionibacterium species. The patient did not respond to long-term intravenous antibiotics and required vertebral corpectomy and debridement with instrumentation. Acne vulgaris is a highly prevalent disease with significant potential for physical and emotional scarring. Acne lesions have long been noted to improve after exposure to sunlight. This improvement may be secondary to activation of endogenous porphyrins produced by Propionibacterium acnes. Recently, several investigators have presented studies in which light of particular wavelengths has been used to treat acne vulgaris. In this article, we review the results of these studies as we look to the future of light-based acne treatment.

The in vitro activity of DMG-Mino and DMG-DM Dot against 350 anaerobic bacterial strains including anaerobic cocci, Propionibacterium acnes, Clostridium perfringens, Clostridium difficile, Bacteroides fragilis, other Bacteroides species and fusobacteria was determined by the agar dilution method. Their activity was compared with that of minocycline, doxycycline, piperacillin, cefoxitin, imipenem, clindamycin and metronidazole. DMG-Mino and DMG-DM Dot and imipenem were the most active agents tested. DMG-Mino and DMG-DM Dot had in vitro activity superior to that of minocycline and doxycycline.

For two centuries, Koch's postulates have set the gold standard for establishing the microbiological etiology of infection and disease. Genomic sequencing now brings finer resolution to both bacterial strain variation and the host genetic state that may predispose to disease. In this issue of the JID, Fitz-Gibbons and colleagues present strain-based resolution of Propionibacterium acnes and its association with the common teenage malady acne vulgaris. Here I examine how Koch's postulates were envisioned and incorporate this finer resolution of both host and microbial states.
