Proximal femur fractures (PFF) pose a major challenge in elderly patients with severe comorbidities and receiving antithrombotic therapy since according to the latest guidelines the surgery should be performed as soon as possible, preferably within 24 hours, to reduce mortality and morbidity. This review outlines the practical approach to surgical management of PFF that relies on increasing evidence of safety of early surgery in patients with PFF receiving antiplatelet and anticoagulant therapy. We have also used information from the existing evidence-based guidelines for elective/planned surg

Fractures of the femoral shaft are combined in 2-5% of all cases with an ipsilateral fracture of the proximal femur. These double fractures are mostly caused by high energy trauma. An analysis of 9 selected papers points to the problem. In more than 10% the proximal fracture is not diagnosed initially. In the literature nearly 30 different operative treatments are described and underline the demands on proper operative techniques and implants for ipsilateral femoral shaft and hip fractures. In this publication the new unreamed femoral nail system (UFN) with a special proximal interlocking opti

CASE: This report presents an extremely rare case of a femoral neck fracture dislocation with associated complete hip abductor avulsion. The patient was treated with open reduction of the dislocation, open reduction internal fixation (ORIF) of the femoral neck, and repair of the hip abductors. The patient initially healed but developed avascular necrosis (AVN) requiring conversion to total hip arthroplasty at 3 years. A Harris Hip Score was 92 eleven years after injury.
CONCLUSION: We recommend prompt, anatomic ORIF of the femoral neck fracture dislocation, direct repair of the hip abductor c

OBJECTIVE: Femoral neck fracture (FNF) is a common type of hip fracture, for which cannulated screw fixation is a primary internal fixation method. The optimal configuration for cannulated screw fixation remains controversial, with debate primarily focused on two approaches: parallel fixation configuration (PFC) versus non-parallel fixation configuration (NFC). The objective of this study was to compare the efficacy and safety of two configurations in the treatment of femoral neck fractures.
METHODS: Following PRISMA guidelines, we systematically searched PubMed, Embase, the Cochrane Library,

BACKGROUND: Fragility hip fractures are associated with high morbidity, mortality, and complex care needs that extend beyond acute management. Despite this, palliative care remains underutilised in this population. Consequently, palliative care needs in people with fragility hip fractures remain insufficiently recognised, assessed and integrated into care pathways.
OBJECTIVE: To identify, synthesise, and analyse key concepts emerging from the literature on palliative care in fragility hip fractures, with a focus on their interrelationships and implications for care.
METHODS: An empirical nar

BACKGROUND: With the aging of the population, the incidence of medial femoral neck fractures is likely to increase, and along them the need for total hip replacement. The present study aimed to analyze whether the use of the direct anterior hip approach, compared with posterolateral approach in medial proximal femur fracture patients, results in any advantage in terms of complications rate and functional recovery.
METHODS: A total of 162 patients were included in the study, and divided by approach: 100 performed with direct anterior approach (group A) and 62 with posterolateral approach (grou

INTRODUCTION: Cephalomedullary nails (CMN) have proven to be effective for the fixation of unstable pertrochanteric hip fractures, but limited data exists regarding the degree of shortening that occurs with nailing systems that offer "dynamically" versus "statistically" locked helical blades. This study aimed to determine the incidence, timing, and amount of postoperative fracture shortening in patients with pertrochanteric hip fractures treated with "dynamically" versus "statistically" locked helical blades.
METHODS: A retrospective review was performed on patients requiring operative fixati

BACKGROUND: Stress fractures of the femoral neck are not common in clinical practice, and simultaneous stress fractures of the femoral neck and proximal femur of the unilateral femur are even more rare. We introduce a case of this type of fracture that was treated in our department, analyze the causes, and review similar stress fractures reported in the literature to provide references for the diagnosis and treatment of such conditions.
CASE SUMMARY: A 62-year-old female, with a free medical history, was admitted to the hospital mainly due to pain in the right hip and worsening pain in the ri

BACKGROUND: Fractures through the proximal femur are broadly grouped into intertrochanteric fractures and intracapsular fractures. It is not clear why a patient may sustain an intertrochanteric fracture as compared with an intracapsular fracture. There is an established relationship between relative hip retroversion and the development of osteoarthritis. We postulate retroversion also may be a risk factor for having intracapsular fractures develop.
QUESTIONS/PURPOSES: We looked specifically at the geometry of the hip to analyze the possibility of a relationship between acetabular version, fem

Fractures of the head and neck of the femur in children are very rare, occurring only after a high-velocity trauma, e.g. a fall from a height. This analysis shows the clinical course of traumatic femoral head and neck fractures in paediatric patients. Predisposing factors for poor outcome or fracture complications, such as non-union or femoral head necrosis, are described. Between 1993 and 2006, 16 paediatric patients with proximal femoral fractures were treated at the Level One Trauma Centre of the Medical University of Vienna. The minimum follow-up was two years. Inclusion criteria were age …

Thirty-seven patients with 37 proximal femoral fractures were treated with a reconstruction locked femoral nail. There were four ipsilateral intracapsular femoral neck and shaft fractures, two intertrochanteric fractures, 18 intertrochanteric fractures with diaphyseal extension, eight subtrochanteric fractures with involvement of the lesser trochanter, and five subtrochanteric fractures without involvement of the lesser trochanter. The overall union rate was 92%. Twenty-one complications developed in 13 patients (35%) which included three of the four femoral neck and shaft fractures, and six o…

Proximal femoral fractures in the skeletally immature patient can be challenging for the orthopaedic surgeon to manage. This type of injury includes the femoral head/neck, intertrochanteric, and subtrochanteric fractures. The Delbet classification system historically describes all injuries in the pediatric proximal femur, except subtrochanteric fractures. Understanding the possible complications (coxa vara, osteonecrosis, premature physeal closure, nonunion) when managing these injuries can help with preoperative and intraoperative decision making. Although the understanding of proximal femur …

This study compared the biomechanical characteristics of proximal femur bionic nail (PFBN) and proximal femoral nail antirotation (PFNA) in treating osteoporotic femoral intertrochanteric fractures using finite element analysis. Under similar bone density, the PFBN outperforms the PFNA in maximum femoral displacement, internal fixation displacement, stress distribution in the femoral head and internal fixation components, and femoral neck varus angle. As the bone density decreases, the PFBN's biomechanical advantages over PFNA become more pronounced. This finding suggests that the PFBN is supe

OBJECTIVES: In this meta-analysis, we discuss the clinical efficacy of closed reduction with in situ fixation for valgus-impacted femoral neck fractures (VIFNFs) by contrasting postoperative functional recovery and complication profiles.
MATERIALS AND METHODS: We performed a systematic literature search in PubMed, Embase, Web of Science, the Cochrane Library, and ScienceDirect for publications up to October 2025. The search targeted original studies which directly compared the surgical outcomes of closed reduction and in situ internal fixation for valgus-impacted femoral neck fractures. Searc

OBJECTIVE: Child abuse has been recognized to be a common cause of femur fractures in infants. Fractures of the proximal femoral physis in abused infants have been less emphasized. Our report seeks to highlight this infrequent but clinically important inflicted injury.
METHODS: Report of 2 cases and a literature review of fractures of the proximal femoral physis in infants, including the role of abuse in this injury.
CONCLUSION: Proximal femoral physeal injuries occur infrequently in infants but often result from abuse. Diagnosis may be difficult due to lack of femoral head ossification befo

Periprosthetic femoral fractures can be a difficult management problem. Proximal femoral fractures with a loose component are managed best with revision arthroplasty. We reviewed the midterm follow-up of 14 proximal femoral fractures managed with a long-stem extensively porous-coated femoral component. The average follow-up in this series was 8.2 years (minimum, 5.3 years). Fractures were treated with open reduction and internal fixation, supplemental cortical strut grafting when required, and a canal-filling implant. All fractures achieved union with an average time to union of 4 months. Ther…

Intramedullary (IM) nail fixation is widely used for the treatment of atypical fractures of the femoral shaft. The configuration and location of proximal interlocking screws are unique to each nailing system and maybe transverse or oblique in direction. The authors experienced two cases of incomplete secondary fractures at the subtrochanteric region after IM nail fixation for atypical femoral shaft fractures. The proximal screw fixation of the two cases was different from one another. One was fixed with a spiral blade plus transverse screw and the other was fixed using an oblique direction scr
