Mediastinal and subcutaneous emphysema associated with childhood measles.

An analysis of the clinical records and chest radiographs of seventeen children with subcutaneous and mediastinal emphysema (SCME) complicating childhood measles was made. The commonest sites of extra-alveolar air were the retrosternal space (16/17) and paraspinal (12/17). Other areas were within the inferior pulmonary ligament (7/17) and subpulmonary areas (6/17). All patients had pulmonary infiltrates. There was pneumothorax in 30% of cases. Eighty per cent of children showed subcutaneous emphysema in addition to mediastinal emphysema. There was a mortality of 12%. SCME completely resolved w

[Physiopathology of diffuse and bullous emphysema (author's transl)].

In emphysema, structural modifications (alveolar distension with destruction of the walls and of the pulmonary capillary bed) contribute to the understanding of pathophysiological disorders, such as increase of the static pulmonary volumes, loss of elastic recoil, impairment in gas transfer. In panlobular emphysema, the simultaneous destruction of ventilatory and circulatory structures explains the absence of shunt effect. The loss of elastic recoil is the main cause of maximal expiratory flow limitation and of bronchial obstruction which is qualified as "extrinsic". For a same degree of bronc

[The role of pulmonary function and computer-assisted tomography imaging in COPD clinics].

Chronic obstructive pulmonary disease (COPD) is characterized by the presence of airflow limitation caused by a loss of elastic recoil of the lung and/or small airway remodeling. Emphysema is the major pathological lesion in COPD, defined as the abnormal and permanent enlargement of distal airspaces and destruction of alveolar walls. Emphysema correlates most closely with a loss of elastic recoil of the lung and is associated with both airflow limitation and loss of diffusion capacity, while the airway component contributes mainly to airflow limitation. Pulmonary functions reflect these pathol

Emphysema formation in a never-smoker with scleroderma-related interstitial pneumonia.

We report a case of pathological emphysema formation in a never-smoker with scleroderma-related interstitial pneumonia. We realized the importance of pulmonary vasculopathy causing the coexistence of emphysema and fibrosis in scleroderma. Our discovery provides novel information that emphysema can occur in patients with scleroderma-related interstitial pneumonia who have never smoked.

Emphysema and extrapulmonary tissue loss in COPD: a multi-organ loss of tissue phenotype.

We tested whether emphysema progression accompanies enhanced tissue loss in other body compartments in 1817 patients from the ECLIPSE chronic obstructive pulmonary disease (COPD) cohort.Clinical and selected systemic biomarker measurements were compared in subjects grouped by quantitative tomography scan emphysema quartiles using the percentage of low attenuation area (LAA%). Lowest and highest quartile patients had amino-acid metabolomic profiles. We related LAA% to 3 years decline in lung function (forced expiratory volume in 1 s (FEV1)), body mass index (BMI), fat-free mass index (FFMI) and

Randomized, Placebo-Controlled Trials in Alpha-1 Antitrypsin Deficiency.

Alpha-1 antitrypsin deficiency (AATD) is a condition caused by the inheritance of two mutated SERPINA1 gene alleles. Individuals with AATD are at increased risk of injury to the liver and lungs. The pulmonary manifestations include precocious onset of pulmonary emphysema and bronchiectasis. For nearly three decades, treatment has been available to individuals with emphysema caused by AATD, but this therapy-augmentation of plasma and tissue alpha-1 antitrypsin levels by intravenous administration of human plasma-derived protein-was approved by regulatory authorities based on its biochemical eff…

[Imaging of emphysema].

Emphysema is defined as a condition of the lung characterized by abnormal, permanent enlargement of airspaces distal to the terminal bronchiole accompanied by destruction of the alveolar walls and without obvious fibrosis. It is a very common disease with high morbidity and mortality. Histopathologically, there are two types of emphysema: panlobular emphysema, typically occurring in alpha1-antitrypsin deficiency, and centrilobular emphysema, which is strongly associated with cigarette smoking. Computed tomography (CT) allows detection of emphysema with higher sensitivity than conventional ches

Effects of appropriate-intensity treadmill exercise on skeletal muscle and respiratory functions in a rat model of emphysema.

The number of patients with chronic obstructive pulmonary disease (COPD), a typical respiratory disorder, is rapidly increasing globally. The purpose of this study was to determine the effects of appropriate-intensity treadmill exercise on skeletal muscle and respiratory functions in a rat model of emphysema. Twenty-one Wistar rats were randomly divided into three groups: the sham (SH) group, pulmonary emphysema (PE) group, and emphysema + exercise (EX) group. Cigarette smoke solution and lipopolysaccharide were intratracheally administered for 4 weeks in the PE and EX groups. The rats in the …

[Determination of pulmonary blood flow by acetylene rebreathing method in patients with chronic respiratory disease].

Non-invasive measurement of pulmonary blood flow by acetylene rebreathing was performed in patients with pulmonary diseases. Cardiac output measurement by thermodilution method was also used simultaneously. A fast computation system was coupled with the measurement. Fourteen emphysema patients and 7 pulmonary fibrosis patients were the subjects. Ten patients with normal lung function were normal controls. After the measurement of cardiac output, the subjects rebreathed mixed gas containing acetylene and argon from the end tidal level. The inspiratory and expiratory gas was measured by mass-spe

Treatment of Alpha-1 Antitrypsin Deficiency.

Alpha-1 antitrypsin deficiency (AATD) is a rare genetic disease that creates multiple unique phenotypes of chronic obstructive pulmonary disease. While bronchospasm, cough, dyspnea, and sputum production all occur with AATD, the phenotypic differences require a computed tomographic (CT) scan to decipher. The availability of augmentation therapy in the United States since 1989 has generated both controversy and evidence that informs the science of usual chronic obstructive pulmonary disease (COPD). Because of the predominance of emphysema in AATD, much of the best evidence concerning biomarkers

The role of clinical characteristics and pulmonary function testing in predicting risk of pneumothorax by CT-guided percutaneous core needle biopsy of the lung.

BACKGROUND: We aim to analyze the risk factors for pneumothorax associated with computed tomography (CT)-guided percutaneous core needle biopsy (PCNB) of the lung. Whether the lung function characteristics are related to pneumothorax is unclear. METHODS: We retrospectively evaluated 343 patients who received CT-guided pulmonary PCNBs and underwent preoperative pulmonary function testing. Demographical, lesion-related, procedure-related features and histopathological diagnosis, as well as results of pulmonary function test were analyzed as risk factors of pneumothorax RESULTS: Variables associ

Swyer-James (MacLeod) syndrome with placental transmogrification of the lung: a case report and review of the literature.

Swyer-James (MacLeod) syndrome is an acquired form of unilateral hyperlucency of the lung and is characterized by the development of severe emphysema, bronchiectasis, and/or bronchiolitis obliterans. It may develop as a complication of repeated episodes of pulmonary infection resulting in bronchiolitis obliterans and obstruction of small airways. Most patients with Swyer-James (MacLeod) syndrome can be managed clinically, and the pathologic features of the syndrome have been described in only a few reports. Placental transmogrification of the lung is a rare histopathologic finding that has bee

The carotid bodies enlarge in some cases of cirrhosis of the liver.

The weights of the individual carotid bodies and cardiac ventricles were obtained at necropsy in five series of subjects. The first comprised 10 cases free of cardiopulmonary disease to act as controls. The second consisted of 10 cases of pulmonary emphysema. The third was composed of 8 cases characterized by sustained alveolar hypoxia due to causes other than pulmonary emphysema. The fourth comprised 10 cases of systemic hypertension or severe left ventricular failure. The fifth was made up of 10 cases of diseases of the liver or alimentary canal. The study confirmed that enlargement of the c…

One-lung high-frequency ventilation in the management of an acquired neonatal pulmonary cyst.

Pulmonary cystic lesions presenting in the neonatal period can be congenital in origin, such as cystic adenomatoid malformation and congenital lobar emphysema, or they can be acquired. With recent advances in the support of premature infants with respiratory distress syndrome (RDS), acquired cystic lesions such as pulmonary interstitial emphysema (PIE), lobar emphysema, and parenchymal cysts have become more common. Several treatments have been proposed for these acquired lesions. We report what we believe to be the first case in which unilateral high-frequency ventilation (HFV) combined with

[Chronic obstructive pulmonary disease, periorbital and subconjunctival swelling].

A 70-year-old man with a history of chronic obstructive pulmonary disease (COPD) and dilative cardiomyopathy was referred due to acute dyspnea and chest pain. After spontaneous pneumothorax was confirmed by chest radiography, a chest tube was inserted into the right side. Persistent air bubbles escaping through the water seal of the drainage in synchrony with respiration indicated a bronchopleural fistula. A physical examination revealed orbital and subconjunctival emphysemas. Skull and chest computed tomography (CT) scans showed further massive cervical, thoracic and pulmonary subcutaneous em
