In vivo study of the angiogenesis potential of bone marrow-derived mesenchymal stem cell aggregates in their niche like environment.

BACKGROUND: Sufficient blood vessel formation in bioengineered tissues is essential in order to keep the viability of the organs. Impaired development of blood vasculatures results in failure of the implanted tissue. The cellular source which is seeded in the scaffold is one of the crucial factors involved in tissue engineering methods. MATERIALS AND METHODS: Considering the notable competence of Bone Marrow derived Mesenchymal Stem Cell aggregates for tissue engineering purposes, in this study BM-aggregates and expanded BM-MSCs were applied without any inductive agent or co-cultured cells, i

The use of supermicrosurgery in lower extremity reconstruction: the next step in evolution.

BACKGROUND: The purpose of this study was to evaluate the feasibility of supermicrosurgery (perforator-to-perforator anastomosis) in lower extremity reconstruction. METHODS: From January of 2007 to February of 2008, a total of 42 patients were treated for soft-tissue defects of the lower extremity resulting from various causes. They were reconstructed with either anterolateral thigh perforator flaps (38 flaps) or upper medial thigh perforator flaps (four flaps) using the supermicrosurgery technique. The region of the defect was located on the knee, including the upper third of the leg in 17 p

Spontaneous twin anemia-polycythemia sequence complicated by recipient placental vascular thrombosis and hydrops fetalis.

Twin anemia-polycythemia sequence (TAPS) is an atypical form of twin-twin transfusion syndrome (TTTS) that presents as a large intertwin hemoglobin difference with one twin developing anemia and the other developing polycythemia, without oligohydramnios-polyhydramnios sequence (Lopriore et al., Placenta 2007;28:47-51). The prenatal diagnostic criteria for TAPS require that the middle cerebral artery-peak systolic velocity (MCA-PSV) measure greater than 1.5 multiples of median (MoM) in the donor twin and less than 0.8 MoM in the recipient twin (Robyr et al., Am J Obstet Gynecol 2006;194:796-803

Reconstruction of the bones and joints of the upper extremity by vascularized free fibular graft: report of 46 cases.

The vascularized free fibular graft was applied in reconstructing bone and joint structures of the upper extremity in 46 cases between August, 1977 and December, 1989. Among these, 30 were grafts of the fibular shaft and 16 were grafts of the fibular head. There were 19 bony defects after trauma or osteomyelitis, 18 bony or articular defects after resection of tumors, five congenital deformities or defects, and four replacements for fibrous dysplasia. The longest grafted fibula was 22 cm and the shortest, 5 cm. Thirty-one cases were followed for more than two years, and these were evaluated fo

Comparison of right and left grafts in renal transplantation.

This study compares outcomes and graft function of right and left grafts of deceased donor. We studied 120 kidney recipients from 60 deceased donors in Shiraz organ transplantation center from 1988 to 2004. We analyzed data regarding age, gender, side of grafts, duration of pre-transplant dialysis, hospital stay, serial creatinine levels, cold ischemic time, complications, graft function, patient survival rates, and post-operative complications. Recipients were divided into two groups: group 1 consisted of 60 recipients who received right renal graft (43 males, 17 females; mean age: 33.6 +/- 7

Hemolytic uremic syndrome in small-bowel transplant recipients: the first two case reports.

Post-transplant hemolytic uremic syndrome (HUS) is an uncommon but well-described complication in solid organ transplant recipients. Believed to be secondary to immunosuppressive therapy, it has been reported after kidney, liver, pancreas, heart, and lung transplants. In all reported cases, the primary organ affected was the kidney (transplant or native). But until now, no cases after small-bowel transplants and no cases in which the kidney was not the primary organ affected have been reported. We report two cases of HUS in small-bowel transplant recipients. In our first case, clinical present

Intravascular Ultrasonic Imaging of Coronary Arterial Remodeling in Heart Transplant Recipients.

The purpose of this study was to evaluate long-term changes of transplant coronary arteries, including vessel, plaque, and lumen areas. There are limited long-term data on vessel remodeling after heart transplantation. We analyzed serial intravascular ultrasound images of the left anterior descending coronary artery (LAD) in 54 heart transplantation recipients. Nine patients (16.7%) had a history of rejection. Proximal left anterior descending artery segments were matched among time points, a ≥20-mm long segment was analyzed every 1 mm, and results were normalized for analysis length and repor

The Super Thin External Pudendal Artery Free Flap for Buccal Reconstruction: A Case Report.

Reconstruction of buccal mucosal defects requires thin, pliable, and well-vascularized tissue to preserve oral function and cheek mobility. While the radial forearm free flap remains widely used, alternative ultra-thin options are needed, particularly in obese patients where conventional perforator flaps may be excessively thick. We report the case of a 59-year-old obese man (BMI 32 kg/m2) undergoing resection of a superficial squamous cell carcinoma of the left buccal mucosa, resulting in a wide 6 × 6 cm defect. Reconstruction was performed using a super-thin external pudendal artery (STEPA)

The Wingsuit Tadpole Technique for Microvascular End-to-Side Anastomosis: From Bench to Bedside.

BACKGROUND: Microvascular end-to-side anastomosis is a critical component of superficial temporal artery to middle cerebral artery bypass procedures. Suboptimal suturing may result in stenosis or thrombosis. We developed the wingsuit tadpole (WT) technique to enhance intraluminal support, thereby improving suturing precision and efficiency. METHODS: The WT technique utilizes a tadpole-shaped rubber device inserted into the fish-mouth incision of the anastomosis. Detailed suturing protocols were established, and the technique was validated in rat femoral vessel models, with the standard bypass

Renal Transplant Artery Autologous Saphenous Vein Graft Aneurysms: Late Presentation and the Need for Recall and Surveillance.

Autologous saphenous vein grafts are occasionally used in renal transplant recipients, particularly in living donors with short donor vessels or after donor vessel injury during allograft procurement. Autologous saphenous vein graft aneurysm formation is described as a late complication following the use of this conduit in renal transplant. We report a case of a 45-year-old woman who developed an autologous saphenous vein graft aneurysm 21 years after her living donor transplant, which was successfully managed with explantation of the graft, cold perfusion ex situ, and resection of the aneurys

Perforator-to-Perforator SCIP-Based Vascularized Lymphnode Transfer to Reduce Morbidity and Increase Efficacy in Lymphedema Surgery: Preliminary Results With 12 Cases.

BACKGROUND: Vascularized lymphnode transfer (VLNT) is one of the most common surgical treatments for moderate and severe lymphedema. Various techniques have been described for harvesting lymph nodes from different donor sites. However, a standardized harvest procedure is still lacking. The transplantation of inguinal lymph nodes using the perforator-to-perforator technique may represent a significant advancement in this context. This approach relies always on the same vascular pedicle, offers a lower morbidity, and allows for a more superficial inset at the recipient site. PATIENTS AND METHOD

Maximizing Neurovascular Outcomes of Facial Transplantation: A Comprehensive Review.

Facial transplantation is a division of reconstructive surgery which aims to improve the function and appearance of a face that has endured severe disfigurement. Currently, the face transplant procedure uses allogenic tissue, harvested from a brain-dead donor, to replace damaged facial components. To date, 50 face transplants have been performed worldwide. Survival and functional success of the transplant is dependent on multiple factors; however, effective neurovascular recovery of the graft is arguably the most important. An effective vasculature system is essential to avoid necrosis and dea

Treatment strategies for soft tissue injuries associated with blunt brachial artery injury.

INTRODUCTION: Blunt brachial artery injuries (BAI) require reconstruction with an extensive vein graft due to the wide area of arterial damage. In the upper arm, safe options for pedicle flaps are limited, and selecting recipient vessels for free-flap surgery is challenging, complicating the treatment of soft tissue injuries associated with blunt BAI. This study aimed to analyze the characteristics and soft tissue reconstruction of blunt BAI and propose treatment strategies for treating associated soft tissue injuries. METHODS: Patients with blunt BAI who treated between August 2013 and April…

Distally Based Lymphatic Microsurgical Preventive Healing Approach-A Modification of the Classic Approach.

The treatment of breast cancer has seen great success in the recent decade. With longer survivorship, more attention is paid to function and aesthetics as integral treatment components. However, breast cancer-related lymphedema (BCRL) remains a significant complication. Immediate lymphatic reconstruction is an emerging technique to reduce the risk of BCRL, the Lymphatic Microsurgical Preventive Healing Approach (LYMPHA) being the most widely used approach. Despite promising results, it is often difficult to find suitably sized recipient venules and perform the microanastomoses between mismatch
