Recently, a range of prescription and over-the-counter (OTC) drugs have emerged as being used recreationally, either on their own or in combination with other substances, both licit and illicit, including new psychoactive substances (NPS). Among them, the misuse of prescription drugs involves not only traditionally recorded substances, such as benzodiazepines and opioid pain relievers, but also gabapentinoids (e.g., pregabalin and gabapentin); some antidepressants, e.g., bupropion and venlafaxine; some second-generation antipsychotics, e.g., quetiapine and olanzapine. Moreover, the use of some

The relationship to treatment outcome, as measured by reduction in drug use, of specific characteristics and elements of 30 drug-free outpatient programs for adolescents is reported. Admission and discharge data were obtained from National Institute on Drug Abuse-Client Oriented Data Acquisition Process on 5789 adolescents in the 30 programs. A partial cross-validation study was conducted by analyzing separately for two annual client subsamples. The program, not the individual clients, was the unit of analysis. While controlling for differences between programs on their client populations, mul

PURPOSE OF REVIEW: Preexposure prophylaxis (PrEP) represents a cornerstone in HIV prevention, yet its broader clinical implications are less frequently explored. This review summarises current evidence on dermatological manifestations associated with PrEP use, including drug-induced reactions, sexually transmitted and contact-acquired infections, and cutaneous effects related to recreational substance use and chemsex practices.
RECENT FINDINGS: Antiretroviral agents approved for PrEP exhibit excellent dermatologic tolerability, with cutaneous adverse drug reactions rarely reported in clinical

BACKGROUND: Understanding the risk behaviors associated with sexually-transmitted hepatitis C virus (HCV) infection among men who have sex with men (MSM) may inform the public health policies and interventions aiming to achieve HCV microelimination.
METHODS: HIV-positive MSM who had one of the following conditions were enrolled to undergo face-to-face questionnaire interviews to collect information on their sexual practices in the past 12 months: (1) elevation of aminotransferases in the past 6 months; (2) acquisition of sexually transmitted infections in the past 6 months; and (3) previous H

New patterns of drug abuse have rapidly appeared in the Western world since the Second World War. These patterns are characterized by the rapid introduction and spread of manufactured substances adopted from medicine. Interest has been sustained among illicit users by the search for new mood altering drugs. Both recreational use and multiple drug patterns are becoming increasingly predominant. Western youth has taken the forefront in the new patterns, especially in the rise of marijuana. Sex differentials are rapidly decreasing as more females are smoking. In the United States, hallucinogens h

Ecstasy, also known as 3,4-methylenedioxymethamphetamine (MDMA), is widely used as a recreational drug known for its stimulating effects. While acute coronary syndrome and coronary vasospasm are frequently observed following cocaine and methamphetamine use, there have been few reported cases linking them to ecstasy use. We present a case of a 27-year-old, previously healthy, male presenting with oppressive retrosternal chest pain and diffuse ST-segment elevation, despite a normal troponin, several days after ecstasy use. His presentation followed by a normal coronary angiography is most consis

BACKGROUND: illegal drugs significantly contribute to global health issues, with health complications often occurring not only in regular users with Substance Use Disorders (SUDs) but also in first-time and occasional users.
METHODS: this study examines five clinical cases from a public hospital in Ibiza, Spain, where patients presented with acute psychiatric symptoms due to recreational drug use.
RESULTS: Contrary to previous studies on SUDs, our patients typically had higher education levels and stable employment. Most of them used multiple substances, with cannabis, cocaine, and alcohol b…

Kratom is an herbal supplement that has gained popularity for recreational use within the United States. Kratom exerts opioid-like effects and, although not US FDA approved, is commonly used for self-treatment of pain, withdrawal management from opioids, and euphoria. Drug-related hepatic injury has been associated with kratom use. All of this raises concern for patient safety and monitoring. The potential for additive liver toxicity must be considered when kratom is used concurrently with hepatotoxic, over-the-counter, herbal, and prescription medications. This case report describes a case of

Young men who have sex with men (YMSM) have high rates of recreational drug use and binge drinking that are related to increases in unprotected sex and HIV risk. We describe the development of a community-level intervention focused on MSM ages 15 to 29 who identify as Black or White and who reported episodic use of alcohol and/or drugs. Intervention content included culturally-relevant role model stories and peer outreach. Outcome measures, including number of partners and a risk score indicating risk for HIV from protected or unprotected insertive or receptive anal sex, were examined over 36 …

BACKGROUND: MDMA (ecstasy) has been a popular recreational drug among young people. MDMA modulates serotonergic activity in the brain and has been shown to be toxic to serotonergic neurons in animals. Disturbances in this neurotransmitter system lead to neuropsychiatric symptoms, hence it has been a concern that use of ecstasy in humans would lead to a higher frequency of psychiatric disease and cognitive impairment. We wanted to review human studies to find evidence for these side effects of ecstasy.
MATERIALS AND METHODS: We investigated 10 studies regarding the connection between use of ec

Gastroschisis, an abdominal wall defect, most often occurs in infants of young mothers. To identify risk factors for gastroschisis, we conducted a case-control study in the population surveyed by the California Birth Defects Monitoring Program (CBDMP). From structured questionnaire data, we compared sociodemographic, reproductive, and lifestyle factors for 110 mothers of infants with gastroschisis with those for 220 age-matched mothers of normal infants. Univariate matched-pair analysis showed significant associations of gastroschisis with mother's education, yearly family income, marital stat

Ketamine is a short-acting general anesthetic with hallucinogenic, analgesic, and amnestic properties. In addition to its anesthetic use, ketamine is commonly abused in rave settings. While safe when used by medical professionals, uncontrolled recreational use of ketamine is dangerous, especially when mixed with other sedative drugs, including alcohol, benzodiazepines, and opioid drugs. Since synergistic antinociceptive interactions between opioids and ketamine were demonstrated in both preclinical and clinical studies, such an interaction could exist for the hypoxic effects of opioid drugs. The recreational use of nitrous oxide (N2O) is increasing in festivals, university parties, clubs, private residences, and parks. The abuse of N2O has serious complications of the central and peripheral nervous system. In this article, we report a case of a 28-year-old previously healthy man who presented with a three-day history of rapidly progressive leg numbness, tingling, and weakness with gait instability and frequent falls. He had a history of marijuana use and daily inhalation of N2O (approximately 20 whippets daily over 2-3 years). He was admitted with a presumptive diagnosis of Guilla

Sir Arthur Conan Doyle's first Sherlock Holmes adventure, A Study in Scarlet, was published 100 years ago. Between 1887 and 1927 he wrote 56 short stories and four novels about his fictional detective. Episodes in the adventures which relate to anaesthetic drugs are described. Use of the drugs was criminal in the case of chloroform, opium, and curare; therapeutic in the case of morphine; and recreational when Holmes himself used cocaine.

BACKGROUND: Previous research has demonstrated associations between substance use and sexual risk behavior in men who have sex with men (MSM). Recent trends in substance use show increasing use of prescription medications (e.g., Vicodin) without a physician's prescription, yet associations between the nonmedical use of prescription drugs (NMUPD) and HIV risk behavior have not been well examined in MSM.
METHODS: MSM attending a gay pride festival completed a self-report measure assessing NMUPD, motivations for use, use of traditional recreational drugs, and HIV risk behavior.
RESULTS: More th

Cannabis is the most widely used illicit drugs and the changing legal, political and cultural climate will likely increase cannabis use further. One factor that may underlie the transition from recreational use to problematic use is stress. The hormone oxytocin (OXT) modulates stress and may have therapeutic efficacy for substance use disorders, but few studies have examined OXT in cannabis users. Another factor is sex; although more men smoke cannabis, the transition from recreational to problematic use is faster in women. Using a within-subjects design, the effects of intranasal (i.n.) oxyto

Delta-9-tetrahydrocannabinol (THC), the main psychoactive cannabinoid in cannabis, may inhibit the cytochrome P450 enzyme CYP2C9. Consequently, cannabis use might infer a risk of drug-drug interaction with substrates for this enzyme, which includes drugs known to have a narrow therapeutic window. In this study, we describe a case report of a 27-year-old man treated with warfarin due to mechanical heart valve replacement who presented with elevated international normalized ratio (INR) value (INR = 4.6) following recreational cannabis use. We conducted a review of the available literature, using

This paper presents, for the first time, community-wide estimation of drug and pharmaceuticals consumption in England using wastewater analysis and a large number of compounds. Among groups of compounds studied were: stimulants, hallucinogens and their metabolites, opioids, morphine derivatives, benzodiazepines, antidepressants and others. Obtained results showed the usefulness of wastewater analysis in order to provide estimates of local community drug consumption. It is noticeable that where target compounds could be compared to NHS prescription statistics, good comparisons were apparent bet

BACKGROUND: Research suggests shifting patterns of recreational drug consumption among MSM in recent decades. The present study evaluates population-level mean trajectories of substance use among HIV-negative MSM from 1995 to 2019.
METHODS: Using open cohort study data following MSM in the Netherlands (n=1495) since 1995, we applied generalized estimating equations to examine population-averaged estimates of five substances - alcohol, cannabis, ecstasy, cocaine, and poppers - from 1995 to 2019, as well as their use during sexual encounters.
RESULTS: Recent alcohol use decreased; predicted pr
