Part II of this two-part article reviews differential diagnosis of common geriatric knee disorders. Differentiating extra-articular from intra-articular causes of knee pain is stressed, since treatments and prognoses can be quite different. Referred pain from the ipsilateral hip and spine should also be kept in mind. The diagnostic approach to knee pain can be categorized anatomically and according to etiology: Is the pain coming from the bone (patella, femur, tibia, fibula), or the soft tissue (ligament, tendon, capsule, synovium, meniscus, muscle)? Is it degenerative, inflammatory, metabolic…

From the otolaryngologist's point of view, there are multiple causes for the frequent symptom of facial and head pain: headaches due to ear diseases: pain extending to the ear region, with special regard to "referred otalgia" involving the cranail nerves V, IX, X; facial pain due to temporomandibular dysfunction; rhinological causes of facial and head pain, including posttraumatic trigeminal neuralgia and "facial sympathalgies"; the syndrome of the elongated styloid process. The diagnosis and therapy of the "typical" ENT diseases is not described in detail since the paper deals mainly with les

Poor posture is a well-known problem in all age groups and can lead to back pain, which in turn can result in high socio-economic costs. Regular assessment of posture can therefore help to identify postural deficits at an early stage in order to take preventive measures and can therefore be an important tool for promoting public health. We measured the posture of 1127 symptom-free subjects aged 10 to 69 years using stereophotogrammetry and determined the sagittal posture parameters flèche cervicale (FC), flèche lombaire (FL), and kyphosis index (KI) as well as the values standardized to the tr

BACKGROUND: The challenge of managing acute postoperative pain is the well tolerated and effective administration of analgesics with a minimum of side effects. The standard therapeutic approach is patient-controlled analgesia (PCA) with systemic opioids. To overcome problems of oscillating opioid concentrations, we studied patient-controlled analgesia by target-controlled infusion (TCI-PCA) as an alternative.
OBJECTIVE: To compare efficacy, safety and side effects of standard PCA with TCI-PCA for postoperative pain therapy with hydromorphone.
DESIGN: Single-blinded, randomised trial.

The aim of this study was to establish reference values for mandibular movements in 10- to 15-year-olds without dysfunction and compare these values to those in patients of the same age with tempromandibular disorders (TMD) and those found previously in a group of children younger than 11 years old without TMD. Children of both genders who visited the Department of Comprehensive Pediatric Dentistry at Buenos Aires University in 2013 and whose parents or guardians provided consent were evaluated using TMD/RDC by standardized pediatric dentists (Kappa 0.88). Three groups were formed according to

BACKGROUND: Secondary or referred otalgia can represent a diagnostic challenge to the otolaryngologist. Collectively, dental disorders are the most common causes of secondary otalgia presenting to the ENT clinic, and may account for up to 50 per cent of referred otalgia. Temporomandibular joint dysfunction syndrome represents the most common dental cause of referred otalgia. Decay and pulpal inflammation of posterior teeth can also frequently present as otalgia. The common embryological developmental origin of both oral and dental structures and the ear is responsible for their overlapping sen

OBJECTIVES: To evaluate the accuracy of anatomical assumptions made to derive a geometrical, ideal, normal model of the upright, static, sagittal cervical spine, to make comparisons with other spinal models and to discuss the implications of a normal cervical model.
BACKGROUND: Anatomical assumptions were made based on observations to assist in the development of a computerized geometrical model of the ideal upright, static, sagittal cervical spine. These assumptions address the magnitudes of the contribution made by the vertebral bodies and intervertebral discs to the overall magnitude and g

Hot perineal baths have been prescribed for the treatment of painful anorectal conditions such as anal fissures and perianal hematomas or for the postoperative care of hemorrhoidectomy. Despite this widely accepted benefit, no studies have been performed to determine whether there is a rational explanation for this procedure. Anorectal manometry was performed in 40 control subjects with no anorectal complaints before and after a hot perineal bath. No significant difference was found between anal pressures at rest or during voluntary contraction before and after the bath. We conclude from this

OBJECTIVE: To determine the prevalence and features associated with axSpA diagnosis in young adults with chronic back pain (CBP) referred to specialists other than rheumatologists, and to evaluate magnetic resonance imaging (MRI) of sacroiliac joints (SIJ) in this population.
METHODS: A prospective, observational study was conducted from July 2021 to October 2023. Adults aged 18-40 years with CBP referred for spinal MRI by non-rheumatology specialists were included. Participants completed a self-administered questionnaire assessing back pain characteristics and SpA features. The spinal MRI wa

Common anorectal conditions include hemorrhoids, perianal pruritus, anal fissures, functional rectal pain, perianal abscess, condyloma, rectal prolapse, and fecal incontinence. Although these are benign conditions, symptoms can be similar to those of cancer, so malignancy should be considered in the differential diagnosis. History and examination, including anoscopy, are usually sufficient for diagnosing these conditions, although additional testing is needed in some situations. The primary treatment for hemorrhoids is fiber supplementation. Patients who do not improve and those with large hig

The case history presented is of a 32 year old woman suffering with severe occipital and bilateral temporal pain together with autonomic disturbances affecting her vision, balance and breathing, symptoms which have been postulated as 'Barré Liéou syndrome'. She complained of pain referred to the left arm and associated circulatory and sensory disturbance in keeping with the diagnosis of complex regional pain syndrome type I. Traditional Chinese and Western trigger point acupuncture techniques were used in order to treat her pain and autonomic dysfunction. Acupuncture was successful in reducing

Shoulder disorders, typically characterized by pain and diminished range of motion, is a common source of musculoskeletal pain in the elderly. Degenerative changes predominate in the elderly shoulder. Like younger patients rotator cuff and bicipital tendon disorders are common. Unlike younger patients elderly patients are more likely to suffer from shoulder pain as a consequence of a frozen shoulder, polymyalgia rheumatica, degenerative arthritis, referred pain-especially from visceral sources, and hemiplegic shoulder pain. The usefulness of high technological diagnostic tests such as magnetic…

BACKGROUND AND PURPOSE: Pain as a long-term outcome following rectal cancer treatment has received limited attention in previous studies. This prospective cohort study examined pain prevalence up to 5 years post-surgery in patients treated with curative intent. Secondary aims included evaluating pain intensity and its interference with daily life, and comparing pain experiences with a reference population. In addition, potential risk factors for pain were explored. Patient/material and methods: The study is based on the multicenter prospective cohort study QoLiRECT including patients from 16 s

Background: Referred pain frequently co-exists with visceral pain. However, the exact mechanism governing referred somatic hyperalgesia remains elusive. Methods: By injecting 20% acetic acid into the stomach, we established a mouse model of gastric ulcer (GU). Hematoxylin and eosin (H&E) staining was used as the evaluation criterion for the gastric ulcer model. Evan's blue (EB) and von Frey tests detected the somatic sensitized area. The DRG neurons distributed among the spinal segments of the sensitized area were prepared for biochemical and electrophysiological experiments. The CCR2 antagoni…

Patients with lower back or buttock pain that radiates into the posterior or lateral leg are often referred to physical therapy with a diagnosis of sciatica. Often the physical exam does not reveal neurologic findings indicative of radiculopathy. Instead, there is hip abductor muscle pain and weakness. This syndrome involves muscle imbalances that result in overuse strain of the gluteus medius and gluteus minimus muscles, myofascial trigger points, and trochanteric bursitis. This paper describes hip abductor pain syndrome and provides a rationale for the diagnosis and treatment.

Ureteral colic is not generally included in the differential diagnoses of acute scrotal pain, although ureteral pathology is a recognized cause of pain referred to the scrotum. We report 3 patients presenting with acute scrotal pain who had normal scrotal color Doppler ultrasound scans; subsequent ultrasound of the ipsilateral kidney revealed hydronephrosis, which ultimately led to the clinically unsuspected diagnosis of ureteral colic. We conclude that ureteral colic should be included in the differential diagnosis of acute scrotal pain, and we further suggest that a survey ultrasound of the

INTRODUCTION: The International Headache Society divides headaches into primary (vascular and muscular) and secondary (inflammatory). Secondary headaches include headaches secondary to inflammation caused by injuries to the head and neck. Cervicogenic headache (CGH) is thought to be referred pain arising from irritation caused by cervical structures innervated by spinal nerves C1, C2, and C3; therefore, any structure innervated by the C1-C3 spinal nerves could be the source of a CGH. Pain from the lateral atlanto-axial joint (AAJ) is one of several causes of CGH. The pain is perceived in the u…

Atypical facial pain or neuralgia and lower-half headache are confusing terms and should be discarded. Recurrent unilateral, throbbing, frontal headaches should be referred to as facial migraine. Patients whose trigeminal branches have been subjected to repeated surgical procedures and who have relentless unilateral face-jaw pain should be classified as having chronic traumatic trigeminal neuralgia. Effective treatment is available provided surgical manipulations cease.

AIMS: Women with suspected angina but no obstructive coronary artery disease (CAD) may be at risk of frequent healthcare contacts due to persistent symptoms, but evidence is scarce. The study aimed to investigate healthcare use following a first diagnosis of angina or non-specific chest pain among a nationwide population of women with no obstructive CAD, compared to a reference population.
METHODS AND RESULTS: The WOMANOCA cohort (Women with symptoms of angina or non-specific chest pain but no obstructive CAD) included all Danish women with a first diagnosis of angina or non-specific chest pa
