Medial Hamstrings Used as Autograft for ACL Reconstruction Show an Abnormal Neuromuscular Pattern at the Time of Return to Sport.

BACKGROUND: Anterior cruciate ligament reconstruction (ACLR) is the preferred treatment for ACL rupture. Medial hamstring (HS) tendons are the most common autografts. An abnormal mediolateral HS muscle activation pattern after ACLR with an HS autograft could influence return to sport (RTS). PURPOSE: To evaluate patients who underwent ACLR with concurrent isokinetic tests and surface electromyography (sEMG) of the medial versus lateral HS compared with a group of healthy controls to identify potential abnormal patterns at RTS. STUDY DESIGN: Comparative study; Level of evidence, 3. METHODS: T

Early functional deficits following first-time acute lateral ankle sprain: A multidimensional assessment framework with clinically relevant cut-off values.

PURPOSE: Lateral ankle sprain (LAS) is one of the most common musculoskeletal injuries associated with sensorimotor impairments that increase the risk of reinjury and chronic ankle instability. Early clinical assessment after acute LAS is often symptom-guided rather than based on objective functional testing. This study aimed to identify the most sensitive functional impairments within the first weeks after injury and to determine cut-off values that support structured clinical assessment. METHODS: In this cross-sectional matched case-control analysis, 36 individuals (14-41 years) with MRI-co

Functional rehabilitation strategies for enhancing neuromuscular control after ACL reconstruction: A systematic review.

BACKGROUND: Arthrogenic muscle inhibition (AMI) and altered neuromuscular control commonly arise after anterior cruciate ligament (ACL) injury and may persist both preoperatively and after anterior cruciate ligament reconstruction (ACLR), potentially impairing functional recovery. OBJECTIVE: To synthesise randomised controlled trial (RCT) evidence on functional rehabilitation interventions targeting neuromuscular control after ACLR, using electromyography (EMG)-derived outcomes. METHODS: PubMed, Cochrane Library, Embase, CINAHL, Web of Science, and SPORTDiscus were searched from January 1, 2

Cutting Technique of Soccer Players After ACLR: On-Field Matched Control Study.

BACKGROUND: Anterior cruciate ligament (ACL) ruptures in soccer most commonly occur during cutting maneuvers with noncontact injury mechanisms. Biomechanical assessment of the cutting technique has become a critical component of return-to-sport (RTS) evaluation after ACL injury, particularly in young football players at elevated risk of reinjury. However, the cutting technique in players with ACL reconstruction (ACLR) has never been examined under sport-specific conditions. PURPOSE/HYPOTHESIS: This study aimed to evaluate the cutting technique in pediatric soccer players who had ACLR after RT

Isolated ACL Reconstruction Versus Combined ACL and Anterolateral Ligament Reconstruction: Functional Outcomes, Return to Sport, and Survivorship: An Updated Meta-analysis of Comparative Studies.

BACKGROUND: Failure, persistent knee instability, and reinjury rates after anterior cruciate ligament (ACL) reconstruction are still concerns. Biomechanical investigations have highlighted the role of the anterolateral ligament (ALL) as a crucial knee stabilizer, and clinical outcomes after combined ACL and ALL reconstruction appear to indicate the success of the procedure. PURPOSE: To compare the functional outcomes, return-to-sport (RTS) rate, and complications between combined ACL and ALL reconstruction and isolated ACL reconstruction. STUDY DESIGN: Meta-analysis; Level of evidence, 4. M

Early muscular training and immobilization in external rotation could reduce the recurrence rate in first-time shoulder dislocators among young rugby athletes.

BACKGROUND: Among rugby players, anterior shoulder dislocation is challenging to treat during the in-season period. It often leads to recurrent shoulder instability and requires prolonged rest postsurgery. No studies have determined the effectiveness of immobilization, early muscle strength training, or both in preventing reinjury in this population. Thus, the present study aimed to determine the impact of treatment outcomes between different immobilization methods and early muscle strength training in the conservative treatment of first-time anterior shoulder dislocation among young rugby pla

Evaluating Donor-Recipient Sex Mismatch in Anterior Cruciate Ligament Reconstruction With Allograft: Outcomes at 2 Years Postoperatively.

BACKGROUND: Female patients tend to suffer a greater risk of reinjury and worse patient-reported outcome measures (PROMs) after anterior cruciate ligament reconstruction (ACLR) than male patients, regardless of the graft type used. PURPOSE/HYPOTHESIS: This study explored the role of donor-recipient sex mismatching to help explain these sex-based disparities in outcomes after ACLR with an allograft. The hypothesis was that allograft donor-recipient sex mismatch would adversely affect surgical outcomes at a 2-year follow-up, with male-to-female graft donations yielding the lowest rates of succe

Clinical Outcomes of Arthroscopic Ligament-Sparing Dorsal Capsulodesis for Partial Scapholunate Ligament Tear.

Introduction  Various wrist arthroscopy techniques can be used in the management of scapholunate ligament (SLL) partial tears but their success has not been proven. Arthroscopic techniques including thermal shrinkage are becoming more popular in the management of partial SLL injuries. We hypothesized that arthroscopic ligament-sparing capsular tightening yields reliable and satisfactory results for the management of partial SLL tears. Methods  A prospective cohort study was conducted on adult (age ≥18 years) patients with chronic partial SLL tears. All patients failed a trial of conservative m

Uneven Treadmill Training for Rehabilitation of Lateral Ankle Sprains and Chronic Ankle Instability: Protocol for a Pragmatic Randomized Controlled Trial.

BACKGROUND: Lateral ankle sprains (LASs) are common injuries among military service members. Approximately 40% of individuals with an LAS progress to develop chronic ankle instability (CAI), a condition that results in substantial mechanical and neurophysiological impairment and activity limitation. Since proprioceptive and balance training improve functional outcomes and prevent secondary injury following LAS, they are recommended in clinical practice. Uneven treadmills are an innovative modality that challenge the sensorimotor system while performing an ecologically valid task simulating env

Early MRI-based quantitative outcomes are associated with a positive functional performance trajectory from 6 to 24 months post-ACL surgery.

PURPOSE: Quantitative magnetic resonance imaging (qMRI) has been used to determine the failure properties of ACL grafts and native ACL repairs and/or restorations. How these properties relate to future clinical, functional, and patient-reported outcomes remain unknown. The study objective was to investigate the relationship between non-contemporaneous qMRI measures and traditional outcome measures following Bridge-Enhanced ACL Restoration (BEAR). It was hypothesized that qMRI parameters at 6 months would be associated with clinical, functional, and/or patient-reported outcomes at 6 months, 24

Safety and Impacts of Physical Activity for Individuals Living With Hypermobility Spectrum Disorders and Hypermobile Ehlers-Danlos Syndrome: Protocol for a Scoping Review.

BACKGROUND: Although physical activity (PA) participation has known health benefits, many individuals with hypermobility spectrum disorders (HSD) and hypermobile Ehlers-Danlos syndrome (hEDS) have difficulties participating in PA. HSD/hEDS affect approximately 1 in 500 people and are more prevalent in females. HSD/hEDS may result in frequent joint injuries, chronic pain, and generalized fatigue. These symptoms, and a fear of new or reinjury, may result in barriers to PA participation. Overall, there is limited research on PA in this population. Previous exercise reviews have not included struc

Markerless 3D motion capture in return to sport decision making for ACL injured high level athletes: a narrative review with proposed clinical implementation pathway.

BACKGROUND AND OBJECTIVE: Anterior cruciate ligament (ACL) injury remains a career-defining event for high-level athletes, and current return to sport (RTS) criteria often fail to detect residual neuromuscular deficits that drive graft failure and contralateral tears. Three-dimensional (3D) markerless motion capture has emerged as a scalable technology that may address these gaps by quantifying movement quality during sport-specific tasks. The objective of this narrative review is to summarize the current evidence on markerless 3D motion capture in ACL rehabilitation and RTS decision-making, w

Outcomes of Anterior Cruciate Ligament Reconstruction With Independently Tensioned Suture Tape Augmentation at 5-Year Follow-up.

BACKGROUND: Reconstruction using autograft remains the gold standard surgical treatment for anterior cruciate ligament (ACL) injuries. However, up to 10% to 15% of patients will suffer a graft failure in the future. Cadaveric studies have demonstrated that the addition of suture tape augmentation to ACL autograft constructs can increase graft strength and reduce elongation under cyclical loading. PURPOSE/HYPOTHESIS: This study aimed to investigate the clinical outcomes and rerupture rates after ACL reconstruction (ACLR) with suture tape augmentation. We hypothesized that augmentation with sut
