Gelsemium alkaloids, such as gelsemine and koumine, are highly toxic compounds that can induce rapid respiratory and cardiovascular failure. Their clinical manifestations-dizziness, ptosis, and sudden collapse-closely resemble those of acute myocardial infarction, presenting diagnostic challenges in forensic practice, especially in distinguishing between poisoning and cardiovascular events. We describe a sudden death in a middle-aged male following a restaurant meal. The decedent developed acute dizziness and eyelid drooping shortly after ingestion and died within hours despite resuscitation.

In the United States between 1970 and 1974 there was an increase each year both in the absolute number of foodborne diseases outbreaks of chemical etiology reported to the Center for Disease Control and in the proportion of these outbreaks in the total reported foodborne disease outbreaks. BACKGROUND: Ciguatera fish poisoning (CFP) is a food-borne illness due to the consumption of reef fish containing pathogenic toxins. CFP is endemic to tropical areas and may be described in travelers in non-endemic areas.
METHODS: We describe two clusters of autochthonous cases of CFP in Paris, France. They were related to two fish caught in Guadeloupe (French West Indies) and consumed in Paris after being air-transported in a cooler. In both cases, fish flesh was analyzed and the presence of ciguatoxins by mouse bioassay (MBA) was confirmed.
