In contrast to adults, cardiopulmonary arrest in infants and children is rarely an acute, primary cardiac event. Instead, it is often the terminal event in a progressive deterioration of respiratory or circulatory function. Successful resuscitation from cardiac arrest therefore is unusual in the paediatric patient and most survivors have persistent neurological impairment. Rapid vascular access and recall of drug dosages are major obstacles in treating paediatric emergencies. This paper reviews vascular access and alternative drug delivery methods. The endotracheal and intraosseous routes prov…

After out of hospital CPR thirty three resuscitated patients were studied for bacteremic complications. Thirteen patients (39%) had two or more positive blood cultures during the twelve hours following CPR. Source of superinfection was a central venous catheter in one case (staphylococcus). The twelve other bacteremic patients had fetid diarrhea a few hours after admission. The same organism were found in blood and faeces (streptococcus D, Escherichia coli, Pseudomonas aeruginosa, acinetobacter, enterobacter). Mesenteric ischemia caused by a low cardiac output may explain the diarrhea and the

Cerebral neurons can tolerate at least 20 min of normothermic ischemic anoxia. Cerebral recovery from more than 5 min of cardiac arrest is hampered by complex secondary derangements of multiple organ systems after reperfusion. There is increasing support of our hypothesis that this "postresuscitation syndrome" includes the following: secondary cerebral perfusion failure, cerebral reoxygenation injury (cell-necrotizing cascades), and cerebral "intoxication" from derangements of extracerebral organs. To be optimal for the brain, CPR with optimal perfusion pressure must be started as promptly as …

One hundred nine patients with the diagnosis of pulmonary contusion were studied retrospectively. Thirteen deaths were respiratory related (12 percent of patients). All of the patients were quickly resuscitated with crystalloid solutions as necessary to restore perfusion to normal. Twenty-eight of the most severely injured patients, all of whom were intubated and ventilated and in whom serial PaO2 and total protein determinations were available, were examined for the relationship between crystalloid induced hemodilution as measured by the plasma colloid oncotic pressure and oxygenation as meas

Cardiac arrest in a twenty-year-old patient was caused by hyperkalaemia of 11.8 mmol/l. Cardiorespiratory resuscitation and emergency treatment of hyperkalaemia saved the life of this patient without residual neurological or mental disorders. We discuss pathophysiological mechanisms.

Electromechanical dissociation (EMD) (organized ECG activity without myocardial contractions) is thought to be rare and associated with poor prognosis. Epinephrine, calcium chloride, and atropine are recommended during attempted resuscitation. Epinephrine's effectiveness in other forms of cardiac arrest is due to its alpha-adrenergic action. Its beta-adrenergic effect may be a disadvantage which is not characteristic of methoxamine, a pure alpha-agonist. Forty anesthetized dogs were asphyxiated until asystolic. Five min later each had slow organized ECG activity. Ventilation with air and stern…

The belief that even a few minutes of anoxia result in irreversible brain damage has been put into serious question, so that the possibility of brain resuscitation has taken on a new significance. A major focus for both laboratory and clinical investigation is the development of techniques to protect neurons during the critical reperfusion period after restoration of cerebral blood flow.

A 13-year-old girl was admitted with meningeal signs. A lumbar puncture was followed shortly by cardiorespiratory arrest. In spite of intensive resuscitation she remained comatose and severe polyuria diagnosed as diabetes insipidus appeared. The differential diagnosis of dehydration and polyuria in patients after cardiorespiratory arrest and resuscitation should include hypothalamic injury causing diabetes insipidus.

Under analysis were 40 cases of allergic reactions of the immediate type arising in 38 patients following the application of various pharmacological agents. Urticaria and Quincke's edema were observed in 21 cases and anaphylactic shock--in 19 cases. Rare cases of anaphylactic shock following injections of sombrevin, polyglucin and clinical observations of the recurring character of the course of allergic reactions are described.

Assuming that unrecognized or inadequately corrected hypovolemia results in higher mortality and morbidity rates, we developed a systematic approach to resuscitation that would: 1) identify criteria to aid in the recognition of hypovolemia and ensure the expeditious correction of this defect without interfering with diagnostic workup and management; 2) define criteria to prevent fluid overload which may jeopardize the patient's course, and 3) express these criteria in an explicit, systematic, patient care algorithm, ie, protocol, useful to both the resident and the practicing physician. We are…

Five selected cases of successful prolonged resuscitation (up to 16 hours) after open heart surgery are presented. Four had undergone prosthetic valve insertion, two being double replacements. External cardiac massage did not produce any demonstrable damage to the heart or the implanted valve prostheses, the competent prostheses probably assuring the haemodynamic effectiveness of the massage. Intermittent positive pressure breathing via an endotracheal tube providing 100% oxygen was begun immediately in all cases. The continuous monitoring of blood pressure through an intra-arterial cannula du…
