Bilateral Central Retinal Artery Occlusion Immediately After Thoracic Endovascular Aortic Repair.

BACKGROUND: Although complications of thoracic endovascular aortic repair (TEVAR), such as spinal cord ischemia and peripheral embolism, are well known, there have been no previous reports of central retinal artery occlusion (CRAO). CASE SUMMARY: A 69-year-old man was transferred to our hospital with abdominal pain and diagnosed with combined type B aortic dissection and abdominal aortic aneurysm. Endovascular repair for abdominal aortic aneurysm was initially performed. After TEVAR for primary entry closure, he reported bilateral vision loss. Ophthalmological examination diagnosed bilateral …

Simultaneous Bilateral Central Retinal Artery Occlusions: Partial Recovery With Multimodal Therapy.

Central retinal artery occlusion (CRAO) is a rare cause of sudden, profound vision loss, and bilateral involvement is exceptionally uncommon, with no clearly established optimal management. We describe a patient who developed near-simultaneous, painless vision loss in both eyes and underwent urgent multidisciplinary assessment followed by systemic and ocular therapies intended to restore perfusion and limit ischemic injury. Although partial visual recovery was achieved, the course was complicated by retinal neovascularization requiring treatment and resulting in severe long-term visual impairm…

The role of triglyceride-glucose index in central retinal artery occlusion.

PURPOSE: To evaluate the correlation between the Triglyceride-Glucose (TyG) index and newly diagnosed cases of central retinal artery occlusion (CRAO) in patients. METHODS: The research involved 51 patients diagnosed with central retinal artery occlusion (CRAO) and a control group consisting of 50 healthy volunteers matched for age and sex. Following a thorough ocular examination, various blood biochemistry and hematological parameters were documented. The TyG index was derived from fasting plasma glucose and triglyceride values. RESULTS: The average age was 66.1 ± 8.9 years for individuals …

Kynurenine-mediated redox regulation provides neuroprotection in central retinal artery occlusion.

BACKGROUND: Central retinal artery occlusion (CRAO) causes irreversible vision loss through ischemia-reperfusion (I/R) injury, characterized by oxidative stress and retinal ganglion cell (RGC) death. Current therapies are inadequate. This study investigates kynurenine (Kyn)—a tryptophan metabolite—as a novel therapeutic agent targeting dual pathways to combat retinal I/R injury. METHODS: The unilateral pterygopalatine ophthalmic artery occlusion (UPOAO) model was established by inserting a silicone filament embolus into the pterygopalatine artery. The detection of tryptophan metabolites in ser…

Hyperbaric Oxygen Therapy for Branch Retinal Artery Occlusion: A Case Series.

Retinal artery occlusion (RAO) is an ophthalmic emergency primarily caused by systemic cardiovascular diseases. Central RAO (CRAO) may lead to severe visual loss, whereas branch retinal artery occlusion (BRAO) is associated with more benign symptoms and often resolves with less severe visual dysfunction. The etiopathogenesis of RAO is similar to that of ischemic stroke. It involves sudden occlusion of the retinal artery, leading to ischemia of the inner layers of the retina, retinal infarction, permanent damage to sensory cells, and resultant irreversible loss of vision. Although small-sample …

Cystoid macular edema as a complication of central retinal artery occlusion.

PURPOSE: To describe the development of cystoid macular edema (CME) as a complication of central retinal artery occlusion (CRAO) in 2 cases. OBSERVATIONS: The first patient was a 51-year-old female who presented with acute loss of vision in the left eye. Multimodal retinal imaging revealed a CRAO with a perfused cilioretinal artery. CME acutely developed one week after presentation. Cystoid spaces predominantly involved the outer nuclear layer (ONL) on optical coherence tomography (OCT) and completely resolved in two weeks. The second case was a 50-year-old man who presented with acute vision…

Nd:Yag Laser Transluminal Embolysis: A Therapeutic Approach in Retinal Artery Occlusion.

Background: Central and branch retinal artery occlusion (CRAO and BRAO) are critical causes of acute vision loss, predominantly affecting older adults with systemic vascular pathology. These occlusions typically result from embolic events, leading to partial or complete retinal ischemia. Methods: This retrospective case series report details of our 10-year experience using the 1064 nm Nd:YAG laser for Transluminal Nd:YAG Embolysis (TYE) in order to lyse visible emboli within the retinal arteries. Results: We conducted a retrospective case series involving 18 patients treated by TYE for differe…

Long-term mortality after retinal artery occlusion - a single centre study.

INTRODUCTION AND OBJECTIVE: Retinal artery occlusion (RAO) is an ophthalmic and systemic emergency requiring urgent diagnosis and treatment. Data regarding mortality in this group, especially in the European population, are modest. The aim of this study is to assess all-cause mortality in post-RAO patients. MATERIAL AND METHODS: This is a retrospective, single-centre study involving 198 patients following RAO diagnosed in 2004-2020. The control group included 198 patients after cataract surgery matched for gender and age, with the date of cataract surgery corresponding to the date of the RAO.…

Retinal microvascular changes in patients with coronary artery disease and apnea.

BACKGROUND: Optical coherence tomography angiography (OCT-A) allowed visualization of capillary level of retina; however, the relationship between coronary vascular status and retinal microvascular changes in patients with apnea is not known well. Our aim was to assess the retinal OCT-A parameters in patients with ischemia and angiographically proven microvascular disease and compare them with obstructive coronary disease in patients with apnea. METHODS: Our observational study included 185 eyes of 185 patients, 123 eyes of patients with apnea (72 eyes from mild OSAS, 51 eyes from moderate to

Diffusion-weighted magnetic resonance imaging in early central retinal artery occlusion.

INTRODUCTION: Restricted retinal diffusion (RDR) has recently been recognized as a frequent finding on standard diffusion-weighted imaging (DWI) in central retinal artery occlusion (CRAO). However, data on early DWI signal evolution are missing. PATIENTS AND METHODS: Consecutive CRAO patients with DWI performed within 24 h after onset of visual impairment were included in a bicentric, retrospective cross-sectional study. Two blinded neuroradiologists assessed randomized DWI scans for the presence of retinal ischemia. RDR detection rates, false positive ratings, and interrater agreement were e…

Optometric Practice Patterns for Acute Central and Branch Retinal Artery Occlusion.

BACKGROUND: Optometrists are often the first providers to evaluate patients with acute vision loss and are often the first to diagnose a central retinal artery occlusion (CRAO). How quickly these patients present to the optometrist, are diagnosed, and referred for evaluation are major factors influencing the possibility of acute therapeutic intervention. Our aim was to survey the U.S. optometric community to determine current optometric practice patterns for management of CRAO. METHODS: An anonymous seven-question survey was emailed in 2020 to the 5,101 members of the American Academy of Opto…

Combined central retinal artery and vein occlusion associated with antiphospholipid syndrome.

A female patient in early 50s presented to us several months after developing severe visual loss in her right eye. The patient was diagnosed with resolved central retinal vein occlusion (CRVO) based on the clinical picture at the time of presentation, however, retroactive evaluation of fundus imaging and further multi-disciplinary workup led to the rare diagnosis of combined central retinal artery and vein occlusion associated with antiphospholipid syndrome (APS). Only a few cases reporting retinal arterial and venous occlusions in patients with APS are found in the literature. To the best of …

Associations Between Retinal Artery/Vein Occlusions and Risk of Vascular Dementia.

BACKGROUND: Vascular disease is a risk factor for Alzheimer's disease (AD) and related dementia in older adults. Retinal artery/vein occlusion (RAVO) is an ophthalmic complication of systemic vascular pathology. Whether there are associations between RAVO and dementia risk is unknown. OBJECTIVE: To determine whether RAVOs are associated with an increased risk of developing vascular dementia or AD. METHODS: Data from Adult Changes in Thought (ACT) study participants were analyzed. This prospective, population-based cohort study followed older adults (age ≥65 years) who were dementia-free at e…
