Outcomes in patients with fractured ribs: middle aged at same risk of complications as the elderly.

AIMS: Rib fractures occur in up to 10% of hospitalised trauma patients and are the most common type of clinically significant blunt injury to the thorax. There is strong evidence that elderly patients have worse outcomes compared with younger patients. Evolving evidence suggests adverse outcomes start at a younger age. The aim of this study was to explore the effect of age on outcomes in patients with rib fractures in Northland, New Zealand. METHOD: A two-year retrospective study of patients admitted to any Northland District Health Board hospital with one or more radiologically proven rib fr…

Bone Autograft Coupled with Locking Plates Repairs Symptomatic Rib Fracture Nonunions.

Symptomatic rib nonunions are a rare complication after rib fractures. Methods used to address these nonunions range from pain management, rib resection, and rib fixation with plates and occasional autologous bone grafting. Given potential complications associated with rib resections such as pulmonary hernia, we hypothesized that plate fixation and autologous bone grafting would yield satisfactory long-term outcomes and a high union rate. Patients (aged ≥18 years) at a single institution with a symptomatic rib nonunion who underwent surgical rib stabilization of the nonunion coupled with bone …

Thoracic Outlet Syndrome in a Volleyball Player Due to Nonunion of the First Rib Fracture.

Fracture of the first rib with ensuing callus formation is a rare cause of thoracic outlet syndrome. We report a case of a 17-year-old female volleyball player who presented with months of chronic arm pain. Radiographic imaging demonstrated nonunion fracture of the first rib. Physical therapy had been unsuccessful in relieving the pain, and surgical management was performed with resection of the first rib through a transaxillary approach with complete resolution of symptoms. Inflammation surrounding such fractures may destroy tissue planes, making dissection more technically difficult.

Intrathoracic fracture-dislocation of the proximal humerus treated with rib fixation and shoulder arthroplasty.

Proximal fractures of the humerus resulting in intrathoracic displacement of the humeral head are rare, but cause serious pain and can lead to pulmonary and glenohumeral complications. Many treatment options exist, but there is still no literature regarding mortality and morbidity associated with rib fixation in concordance with treatment of the proximal humerus. This case describes a patient who was treated using a multi-team approach that involved shoulder arthroplasty with rib fixation, and which resulted in decreased pain and decreased narcotic use while improving the patient's pulmonary f

Chronic pain and its impact on quality of life following a traumatic rib fracture.

PURPOSE: Traumatic rib fractures account for 7-40 % of trauma admissions and most of them heal spontaneously and do not contribute to disability. The prevalence of chronic pain and its impact on quality of life following a traumatic rib fracture has not been studied adequately. METHODS: A retrospective review of electronic medical records of all the traumatic rib fracture admissions from January 2007 to December 2008 was conducted. This was followed up with a brief telephonic survey of the following questions: (1) Do you have pain following the trauma? (2) If YES, how severe is your pain from…

[A case report of solitary bone cyst of rib detected from pathological fracture].

A case of solitary bone cyst of rib was reported. The patient was 28-year-old man who was admitted to our hospital complaining of chest pain caused by pathological fracture of the left 10th rib. Chest x-ray film revealed a solitary cystic lesion of the 10th rib. The tumor was removed by extrapleural rib partial resection and the histological examination confirmed the diagnosis of solitary bone cyst.

[A case of extremely severe chest injury with fracture of several ribs (author's transl)].

A 72-years old man was severely injured when a lorry rolled back and pinned him down, causing contusion of the chest, fractures of ribs 3-10 on the right and haemothorax. Treatment of the chest injuries was by drainage and by positive end-expiratory pressure ventilation because of the development of severe pneumonia with wet lung. Persistent renal insufficiency, a gastro-intestinal haemorrhage and diabetes also required treatment. The patient developed septic endomyocarditis as a late complication, possibly attributable to the central venous catheter. All pulmonary and extrapulmonary injuries

Isolated Rib Response and Fracture Prediction for Young Mid-Size Male, Enabled by Population Specific Material Models and Rib Cross-Sectional Geometry.

Thorax injury remains a primary contributor to mortality in car crash scenarios. Although human body models can be used to investigate thorax response to impact, isolated rib models have not been able to predict age- and sex-specific force-displacement response and fracture location simultaneously, which is a critical step towards developing human thorax models able to accurately predict injury response. Recent advancements in constitutive models and quantification of age- and sex-specific material properties, cross-sectional area, and cortical bone thickness distribution offer opportunities t

Outcomes after isolated rib fractures in end-stage renal disease (ESRD) patients: A propensity matched analysis.

BACKGROUND: Rib fractures are a common traumatic injury among end-stage renal disease (ESRD) patients and major risk factors for poor outcomes need ongoing investigation. METHODS: A propensity matched analysis using the National Trauma Data Bank (NTDB) from 2021 to 2023 including ESRD vs. non-ESRD patients who sustained isolated rib fractures. Patients were matched for demographics, injury severity, trauma mechanism, and comorbidities. RESULTS: The study included 2456 patients with a mean age of 71 years, all of which sustained blunt trauma. In-hospital mortality was 3% in ESRD patients vs 1…

"Anchor-type" incision for surgical stabilization of anterior rib fractures: a single center experience and evaluation.

PURPOSE: Rib fixation is increasingly used, but bilateral anterior fractures are difficult to access. We assessed feasibility and outcomes of a single muscle-sparing "anchor-type" anterior exposure for bilateral parasternal fractures with or without sternal fractures. METHODS: Single-center retrospective study at a level I trauma center from January 1, 2016 to April 30, 2024. Adults with bilateral parasternal rib fractures who underwent surgical stabilization of rib fractures (SSRF) through an anchor-type incision were included; sternal plating was performed when indicated. Collected variable…

A rare case of lung hernia associated with rib fractures and tension pneumothorax post cardiopulmonary resuscitation.

Rib fractures are the most common injury after cardiopulmonary resuscitation (CPR) for non-traumatic cardiac arrest. Pneumothorax can evolve into tension pneumothorax with assisted ventilation leading to hypotension and cardiac arrest. Lung hernia post-CPR is rare but a potentially fatal complication. Here we describe a 67-year-old female who presented to the emergency department (ED) with acute infective asthma exacerbation. She received initial treatment of inhaled salbutamol and ipratropium bromide, oral prednisolone, supplemental oxygen, and intravenous ceftriaxone. Shortly after administr…

Thoracostomy for removal of excess fluid in surgical stabilization of rib fractures: the T-REX trial.

BACKGROUND: Management of the pleural space during and after SSRF is a matter of debate. Tube thoracostomy (TT), intra-operative pleural lavage (PL) and video assisted thoracoscopic surgery (VATS) use varies between surgeons. The purpose of this study is to describe differences in practice patterns of pleural space management (TT, PL, VATS) after SSRF at institutions with extensive experience in chest wall reconstruction. METHODS: Prospective data from adult SSRF patients at eight U.S. trauma centers between January 1, 2020 and September 1, 2022 was collected. Patients were managed according

Surgical fixation of traumatic sternal manubrium dislocation and right-sided rib fractures following a motorcycle accident.

This brief report discusses the diagnosis, management and surgical intervention of a man in his 30s presenting with a rare traumatic sternal manubrium dislocation following a motorcycle crash, accompanied by multiple concomitant rib fractures. The severity and complexity of the patient's injuries necessitated an operative approach for his sternomanubrial dislocation, emphasising the importance of multidisciplinary coordination, accurate diagnosis and prompt surgical intervention. The report provides valuable insights into the successful application of open reduction and internal fixation with …
