Breast cancer risk assessment across the risk continuum: genetic and nongenetic risk factors contributing to differential model performance.

INTRODUCTION: Clinicians use different breast cancer risk models for patients considered at average and above-average risk, based largely on their family histories and genetic factors. We used longitudinal cohort data from women whose breast cancer risks span the full spectrum to determine the genetic and nongenetic covariates that differentiate the performance of two commonly used models that include nongenetic factors - BCRAT, also called Gail model, generally used for patients with average risk and IBIS, also called Tyrer Cuzick model, generally used for patients with above-average risk. A Comparison of Perceived Lifetime Breast Cancer Risk to Calculated Lifetime Risk Using the Gail Risk Assessment Tool.

Background: Understanding the accuracy of a woman's perceived breast cancer risk can enhance shared decision-making about breast cancer screening through provider and patient discussion. We aim to report and compare women's perceived lifetime breast cancer risk to calculated lifetime breast cancer risk. Methods: Women presenting to Mayo Clinic in Arizona and Minnesota in July 2016 completed a survey assessing their perceived breast cancer risk. Lifetime Gail risk scores were calculated from questions pertaining to health history and were then compared with perceived breast cancer risk. Results

Behavioral risk assessment for infectious diseases (BRAID): Self-report instrument to assess injection and noninjection risk behaviors in substance users.

BACKGROUND: Infectious diseases such as Human Immunodeficiency Virus and Hepatitis C are a significant problem among substance abusers. Current risk behavior measures [e.g., HIV Risk Taking Behaviour Scale (HRBS) and Risk Assessment Battery (RAB)] were developed for injection drug users and do not include newly identified risks or noninjection drug use behaviors. This study developed and provided initial, internal validation of the Behavioral Risk Assessment for Infectious Diseases (BRAID) to assess infectious disease risk behaviors among alcohol and other drug users. METHODS: A self-report m…

Novel Interactive Tool for Breast and Ovarian Cancer Risk Assessment (Bright Pink Assess Your Risk): Development and Usability Study.

BACKGROUND: The lifetime risk of breast and ovarian cancer is significantly higher among women with genetic susceptibility or a strong family history. However, current risk assessment tools and clinical practices may identify only 10% of asymptomatic carriers of susceptibility genes. Bright Pink developed the Assess Your Risk (AYR) tool to estimate breast and ovarian cancer risk through a user-friendly, informative web-based quiz for risk assessment at the population level. OBJECTIVE: This study aims to present the AYR tool, describe AYR users, and present evidence that AYR works as expected …

Cardiovascular risk prediction in women with gestational diabetes using risk assessment tools: a scoping review.

BACKGROUND: Gestational Diabetes Mellitus (GDM) is a glucose intolerance diagnosed during pregnancy and is associated with an increased risk of developing cardiovascular disease (CVD) later in life. Despite this well-established link, it remains unclear how effectively existing CVD risk assessment tools account for the unique risks faced by women with a history of GDM. This scoping review aims to systematically explore the literature to examine how CVD risk prediction tools have been applied to this population and whether they adequately capture their long-term risk. METHODS: A comprehensive s…

Use of Rotterdam Risk Index in the Assessment of Risk Factors in Abdominal Wound Dehiscence.

INTRODUCTION: Abdominal wound dehiscence (AWD) is a serious postoperative complication characterized by partial or complete disruption of a surgical incision, often leading to increased morbidity, prolonged hospitalization, and higher healthcare burden. Its occurrence is multifactorial, involving patient-related, surgical, and postoperative factors. Risk assessment tools such as the Rotterdam Risk Index (RRI) have been used to evaluate the likelihood of AWD; however, as the index includes postoperative variables, it reflects post-hoc risk stratification rather than purely preoperative predicti…

Risk-enhancing factors and social determinants of health in risk assessment for atherosclerotic cardiovascular disease.

BACKGROUND: The Pooled Cohort Equations (PCEs) do not accurately estimate atherosclerotic cardiovascular disease (ASCVD) risk in certain populations. The 2018 AHA/ACC cholesterol guideline identified risk-enhancing factors as a supplement to PCEs-based risk assessment. However, the role of each risk-enhancing factor in ASCVD risk assessment has not been well quantified. Further, social determinants of health (SDOH) are not included in the PCEs nor considered as risk-enhancing factors in the US cholesterol guideline. We sought to evaluate ASCVD risk associated with each risk-enhancing factor an…

Adapting risk assessments to a changing terrorism landscape: Revising the extremism risk guidance.

The Extremism Risk Guidance-Revised (ERG-R) updates and expands the ERG22+ framework, which has guided terrorism risk assessment in prison and probation across England and Wales since 2011. Developed by His Majesty's Prison and Probation Service (HMPPS) Counter Terrorism-Assessment and Rehabilitation Centre (CT-ARC), the ERG-R responds to key shifts in the terrorism landscape, including the growth of online radicalization, greater involvement of younger individuals, an increase in extreme right-wing convictions, and more fluid ideological affiliations. The revised framework comprises 36 factor…

Assessing Lung Cancer Absolute Risk Trajectory Based on a Polygenic Risk Model.

Lung cancer is the leading cause of cancer-related death globally. An improved risk stratification strategy can increase efficiency of low-dose CT (LDCT) screening. Here we assessed whether individual's genetic background has clinical utility for risk stratification in the context of LDCT screening. On the basis of 13,119 patients with lung cancer and 10,008 controls with European ancestry in the International Lung Cancer Consortium, we constructed a polygenic risk score (PRS) via 10-fold cross-validation with regularized penalized regression. The performance of risk model integrating PRS, inc

Peak Exposures in Epidemiologic Studies and Cancer Risks: Considerations for Regulatory Risk Assessment.

We review approaches for characterizing "peak" exposures in epidemiologic studies and methods for incorporating peak exposure metrics in dose-response assessments that contribute to risk assessment. The focus was on potential etiologic relations between environmental chemical exposures and cancer risks. We searched the epidemiologic literature on environmental chemicals classified as carcinogens in which cancer risks were described in relation to "peak" exposures. These articles were evaluated to identify some of the challenges associated with defining and describing cancer risks in relation t…

A practical approach to breast cancer risk assessment and management of women at elevated risk.

Breast cancer, the most frequently diagnosed malignancy in women both in the United States and globally, represents a significant public health burden. While screening constitutes the cornerstone of early detection, enhanced surveillance, and preventive measures-including risk-reducing medication and lifestyle modification-are underutilized. The well-woman visit remains a pivotal opportunity for preventive care and individualized risk assessment. Assessment should begin with "genetics": evaluation for a potential germline pathogenic variant in a breast cancer susceptibility gene. If prior gene…
