Rotator cuff tear arthropathy encompasses a broad spectrum of pathology, but it involves at least three critical features: rotator cuff insufficiency, degenerative changes of the glenohumeral joint, and superior migration of the humeral head. Although many patients possess altered biomechanics of the glenohumeral joint secondary to rotator cuff pathology, not all patients develop rotator cuff tear arthropathy, and thus the exact etiology of rotator cuff tear arthropathy remains unclear. The objectives of this manuscript are to (1) review the biomechanical properties of the rotator cuff and the

Latissimus dorsi musculotendinous transfer has been described for the treatment of massive rotator cuff defects. The aim of this paper is to present the relevant surgical anatomy of an arthroscopically assisted technique associated with a modified harvesting of the tendon. For tendon harvesting, a skin incision of about 5-6 cm in length is made in the axillary crease orthogonal to the longitudinal axis of the upper arm. For the musculotendinous transfer, we propose to increase vector action of the latissimus dorsi by passing the transferred tendon ahead the triceps muscle under or over the Ter

BACKGROUND: Rotator cuff tears are frequently associated with pathologies of the long head of the biceps tendon (LHBT). Tenotomy and tenodesis of the LHBT are commonly used to manage disorders of the LHBT.
METHODS: We present an arthroscopic soft tissue LHBT tenodesis associated with a Roman Bridge (double pulley - suture bridges) repair
RESULTS: Two medial row 5.5-mm Bio-Corkscrew suture anchors (Arthrex, Naples, FL), double-loaded with No. 2 FiberWire sutures (Arthrex, Naples, FL), are placed in the medial aspect of the footprint. A shuttle is passed through an anterior point of the rotato

Tissue augmentation has been used to reinforce repairs of massive chronic rotator cuff tears. This report describes a new technique for arthroscopic insertion of a biologic rotator cuff tissue augmentation. Double-loaded suture anchors are needed for the technique. One suture is used for the rotator cuff repair and 1 suture is used for the tissue augmentation. The sutures for the tissue augmentation are left exiting through accessory portals. Two free sutures are then passed through the repaired rotator cuff, 1 anteromedially and 1 posteromedially via the anterior and posterior cannulae, respe

Arthroscopic subscapularis repair can be technically challenging. This article summarizes a number of technical tips that can greatly simplify and expedite what otherwise might be a daunting surgical procedure. Specific tips and pearls include the following: 1. A description of a new clinical test--the bear-hug test--for detecting subscapularis tears on physical examination. 2. How to accurately place portals for precise subscapularis repair. 3. How to safely and accurately perform a coracoplasty. 4. How to identify and mobilize a retracted subscapularis tear. 5. How the comma sign can be used

Rotator cuff tears are a common cause of shoulder pain. Clinical and radiographic findings can suggest the presence of a rotator cuff tear. The most sensitive clinical findings are impingement and the "arc of pain" sign. Radiographic findings are usually normal in the acute setting, although the "active abduction" view may show decreased acromiohumeral distance. In more chronic cases, an outlet view may show decreased opacity and decreased size of the supraspinatus muscle due to atrophy. In late cases, the humeral head may become subluxated superiorly, and secondary degenerative arthritis of t

The arthroscopic operation for repair of full-thickness rotator cuff tears is successful and has the advantages of glenohumeral joint inspection, treatment of intraarticular lesions, smaller incisions, no deltoid detachment, less soft tissue dissection, less pain, and more rapid rehabilitation. However, these advantages must be balanced against the technical difficulty of the method, which limits its application to surgeons skilled in open and arthroscopic shoulder operations.

Clinical examination for rotator cuff tears is rarely conclusive and very difficult is the differential diagnosis with lesions of the axillary nerve. In a little hospital we chose to use first non invasive and cheaper tools for this diagnosis. 24 patients were first evaluated by a physiatric MD, and radiological and ultrasound studies were then performed. Ultrasonography (US) was done with a high frequency probe (7.5 MHz). More than 85% of our patients had, in this way a quick, cheap and non invasive diagnosis, and moreover a quick rehabilitation treatment. Only 3 patients were later studied i

Arthroscopic techniques for subacromial decompression have been criticized for lack of precision in resecting the anterior acromial undersurface and evaluating the amount of bone resected. The goal of subacromial decompression is production of a flat undersurface for the acromion and acromioclavicular joint, thus enlarging the supraspinatus outlet and deterring impingement. Achieving this goal using the arthroscope requires preoperative evaluation of the acromial morphology, planning of the dimensions of bony resection, a reproducible acromioplasty method with intraoperative evaluation of the

Previous case reports have highlighted various anomalous origins of the long head of the biceps tendon (LHBT) that do not originate from the superior glenoid labrum or supraglenoid tubercle. Yet, these cases were all reported as incidental findings and were not thought to cause any significant shoulder pathology. We present the magnetic resonance (MR) imaging and clinical treatment of two cases where aberrant intra-articular origins of the long head of the biceps tendon from the anterior edge of the supraspinatus tendon may have contributed to symptomatic rotator cuff pathology. BACKGROUND: The advantage of single-row versus double-row arthroscopic rotator cuff repair techniques has been a controversial issue in sports medicine and shoulder surgery. There is biomechanical evidence that double-row techniques are superior to single-row techniques; however, there is no clinical evidence that the double-row technique provides an improved functional outcome.
HYPOTHESIS: When compared with single-row rotator cuff repair, double-row fixation, although biomechanically superior, has no clinical benefit with respect to retear rate or improved functional outcome.

BACKGROUND: Rotator cuff tears are the most common tendon injury seen in orthopaedic patients. Muscle atrophy and fatty infiltration in rotator cuff muscles are considered among the key factors responsible for the failure of attempted repair of a massive rotator cuff tear. However, the pathophysiology of rotator cuff muscle atrophy and fatty infiltration remains largely unknown, partly because of the lack of appropriate small animal models. The goal of this study was to develop a mouse model of muscle atrophy and fatty infiltration after a rotator cuff tear. We also sought to study the role of

BACKGROUND: It was the objective of this study to evaluate the contact area and pressure between a repaired rotator cuff tendon and bone for several different rotator cuff repair techniques, with and without a subacromial balloon spacer. It was hypothesized that the contact area and pressure at the rotator cuff footprint would increase with a subacromial balloon spacer following all repair techniques evaluated.
METHODS: Ten cadaveric shoulders were mounted onto a biomechanical shoulder simulator. A full-thickness 2-cm rotator cuff tendon tear was simulated, and repaired using single-row, doub

BACKGROUND: The double-row suture bridge (SB) is an effective technique for rotator cuff repair. However, because of high-tension sutures and tendon vascular insufficiency, nonunion or retearing of the rotator cuff after SB repair is common. The low-tension suture and microfracture features of the SCOI Row (SR) technique may help address these issues.
PURPOSE: To compare the biomechanical and histological outcomes of SR and SB techniques in treating rotator cuff tears in rabbits.
STUDY DESIGN: Controlled laboratory study.
METHODS: The study utilized a model of acute bilateral supraspinatus

Subscapularis pathology is difficult to diagnose, in part because of decreased sensitivity and accuracy in identifying tears with magnetic resonance imaging (MRI) when compared to other cuff tendons. Artificial intelligence evaluation of patient physical examination and MRI data using a machine learning model shows that arthroscopically confirmed partial- or full-thickness subscapularis tears are highly associated with abnormal subscapularis tendon length, long head of the biceps tears, and subscapularis fatty atrophy, and on physical examination, with weakness with internal rotation and posit

OBJECTIVE: To develop and validate MRI-based radiomics models capable of evaluating supraspinatus tendon tears within the shoulder joints by using arthroscopy as the reference standard.
METHODS: A total of 432 patients (332 in the training set and 100 in the external validation set) with intact supraspinatus tendon (n = 202) and supraspinatus tendon tear (n = 230, 130 full-thickness tears and 100 partial-thickness tears) were enrolled. Radiomics features were extracted from fat-saturated T2-weighted coronal images. Two radiomics signature models for detecting supraspinatus tendon abnormalitie

BACKGROUND: This study aims to evaluate the significance of preoperative magnetic resonance imaging findings to predict subscapularis tear confirmed at arthroscopic surgery.
METHODS: Sixty-four consecutive shoulders that underwent preoperative magnetic resonance imaging examination and arthroscopic shoulder operations were retrospectively reviewed. Under arthroscopic examination, complete subscapularis tear was defined as a full-thickness tear and incomplete subscapularis tear as tendon detachment larger than 5 mm from the insertion on the joint side.
RESULTS: In arthroscopic findings, they
