Quantitative Benefit-Risk Models Used for Rotavirus Vaccination: A Systematic Review.

BACKGROUND: Although rotavirus vaccines have proven to prevent the risk of rotavirus gastroenteritis (RVGE) in children under 5 years old, they are also associated with an increased transient risk of intussusception (IS). Several quantitative benefit-risk models (qBRm) are performed to measure this balance in hospitalizations and deaths prevented versus the ones induced. METHOD: In this study, our objective was to provide a complete overview of qBRm used for rotavirus vaccination. We systematically searched 3 medical literature databases to identify relevant articles, in English, that were pu

Cholangiocyte secretion of chemokines in experimental biliary atresia.

UNLABELLED: Biliary atresia (BA) is a disease of the newborn that results in obstruction of the biliary tree. The cause of BA remains unknown; however, recent studies using the murine model of biliary atresia have found that rotavirus infection of the biliary epithelial cell (cholangiocyte) triggers an inflammatory response. We hypothesized that rotavirus infection of cholangiocytes results in the release of chemokines, important mediators of the host immune response. METHODS: In vivo, Balb/c pups were injected with rhesus rotavirus (RRV) or saline, and, their extrahepatic bile ducts were mic

Increased bacterial taxonomic and functional diversity is associated with impaired rotavirus vaccine immunogenicity in infants from India and Malawi.

The immunogenicity and effectiveness of oral rotavirus vaccines (ORVs) against severe rotavirus-associated gastroenteritis are impaired in low- and middle-income countries (LMICs) where the burden of disease is highest. Determining risk factors for impaired ORV response may help identify strategies to enhance vaccine effectiveness. In this study, we use metagenomic sequencing to provide a high-resolution taxonomic analysis of stool samples collected at 6 weeks of age (coinciding with the first ORV dose) during a prospective study of ORV immunogenicity in India and Malawi. We then analyse the f

Vaccines: the pentavalent rotavirus vaccine: discovery to licensure and beyond.

Twenty-five years passed between the discovery of the parent strain (WC3) of the pentavalent human-bovine reassortant rotavirus vaccine (PRV) in 1981 and the licensure of PRV in 2006. This orally administered liquid vaccine, which is given as a 3-dose series, is indicated for the prevention of rotavirus gastroenteritis, caused by the G serotypes contained in the vaccine, in infants and children. PRV is recommended by the Advisory Committee on Immunization Practices and the American Academy of Pediatrics for administration to all infants in the United States. These recommendations are supported

Neonatal administration of rhesus rotavirus tetravalent vaccine.

BACKGROUND AND AIMS: Administration of the first dose of rhesus rotavirus-based tetravalent (RRV-TV) vaccine is followed by a transient febrile reaction at 3-4 days postvaccination in about one-third of vaccinees. We hypothesized that giving the first dose of RRV-TV vaccine during the neonatal period might reduce the reactogenicity of RRV-TV vaccine without compromising the utilization of the vaccine. METHODS: A double blind placebo-controlled safety and immunogenicity trial of 90 infants who received RRV-TV vaccine at 0-4-6, 0-2-4 or 2-4-6 months of age was conducted. Reactions were evaluate

[New rotavirus vaccines: a reality at last].

Rotavirus is the most common cause of severe acute diarrhea among children less than 3 years of age. This infection causes 25 million medical visits, 2 million hospitalizations and 440,000 deaths every year worldwide. The development of rotavirus vaccines has followed a long and difficult path seriously hampered by the abrupt withdrawal of Rotashield in 1999 due to its association with intestinal intussusception. After six years of intense research, the world celebrates the licensing of two new rotavirus vaccines, different in formulation and administration schemes albeit clearly safe and not

Investigation of viral and bacterial enteropathogens of diarrheic sheep and goats in Medina, Saudi Arabia.

Diarrhea is a serious problem in sheep and goat farming, causing great economic losses. Most viral and bacterial enteropathogens of diarrheic sheep and goats are considered foodborne pathogens with a potential to be zoonotic. The present study investigated the prevalence of some viral and bacterial enteropathogens in diarrheic sheep and goats between October 2015and February 2016 in Medina, Saudi Arabia. A total of 310 fecal samples were collected from diarrheic sheep (n=193) and goats (n=117). The samples were screened for the presence of rotavirus and bovine coronavirus (BCoV) using Sandwich

Identification of Murine Rotavirus Virulence Determinants Using Bidirectional Selective Passaging and a Reverse Genetics System.

Live-attenuated rotavirus (RV) vaccines are the most effective interventions for preventing RV gastroenteritis (RVGE) in young children. However, the molecular basis of attenuation remains not well understood. Here, we describe a compact but comprehensive strategy to identify RV virulence determinants by combining low-passage bidirectional selection, sequence analysis, and segment-level phenotype testing via a reverse genetics infectious system. Using the virulent murine RV strain EW, virulence was quantified by diarrhea severity/duration and body-weight gain. Serial passaging in cell culture

Prevalence of group A rotavirus G and P genotypes in children with acute gastroenteritis in China from 2020 to 2021.

In China, inpatient/outpatient visits for rotavirus (RV)-associated diarrhea are common among children aged < 5 years. An interim analysis evaluated RV genotype circulation in China in an ongoing observational effectiveness study of a pentavalent (RV5) vaccine. Data were collected (October 2020-March 2021) from children eligible for vaccination with three doses of RV5 and receiving treatment for acute gastroenteritis. RV gastroenteritis was reported in 494 (62.1%) of the 796 children tested (median age, 15 months). The circulating genotypes were G9 (81.8%), G2 (5.5%), G8 (4.6%), and G3 (0.4%);

Development of a rotavirus vaccine: clinical safety, immunogenicity, and efficacy of the pentavalent rotavirus vaccine, RotaTeq.

Initial approaches for rotavirus vaccines were based on the classical "Jennerian" approach and utilized simian and bovine rotavirus strains, which provided cross-protection against human rotavirus strains but did not cause illness in infants and young children because of their species-specific tropism. The demonstrated efficacy of these vaccines was not consistent across studies. Thus, human-animal reassortants containing an animal rotavirus backbone with human rotavirus surface G and/or P proteins were developed, which demonstrated more consistent efficacy than that observed with the non-reas

Evaluation of safety and immunogenicity of a live attenuated tetravalent (G1-G4) Bovine-Human Reassortant Rotavirus vaccine (BRV-TV) in healthy Indian adults and infants.

BACKGROUND: Rotavirus infections, prevalent in human populations worldwide are mostly caused by Group A viruses. Live attenuated rotavirus vaccines are highly effective in preventing severe rotavirus gastroenteritis. However, the cost of these vaccines and local availability can be a barrier for widespread adoption in public health programs in developing countries where infants suffer a heavy burden of rotavirus related morbidity and mortality. A phase I/II study was carried out with the long term aim to produce a locally licensed vaccine which is equally safe and immunogenic as compared to av…

Immune Response to Rotavirus and Gluten Sensitivity.

Rotavirus is a double-stranded RNA virus belonging to the family of Reoviridae. The virus is transmitted by the faecal-oral route and infects intestinal cells causing gastroenteritis. Rotaviruses are the main cause of severe acute diarrhoea in children less than 5 years of age worldwide. In our previous work we have shown a link between rotavirus infection and celiac disease. Nonceliac gluten sensitivity (NCGS) is emerging as new clinical entity lacking specific diagnostic biomarkers which has been reported to occur in 6-10% of the population. Clinical manifestations include gastrointestinal a

Norovirus and Other Viral Causes of Medically Attended Acute Gastroenteritis Across the Age Spectrum: Results from the Medically Attended Acute Gastroenteritis Study in the United States.

BACKGROUND: Acute gastroenteritis (AGE) causes a substantial burden in the United States, but its etiology frequently remains undetermined. Active surveillance within an integrated healthcare delivery system was used to estimate the prevalence and incidence of medically attended norovirus, rotavirus, sapovirus, and astrovirus. METHODS: Active surveillance was conducted among all enrolled members of Kaiser Permanente Northwest during July 2014-June 2016. An age-stratified, representative sample of AGE-associated medical encounters were recruited to provide a stool specimen to be tested for nor
