Clinical profile of patients with anterior nodular scleritis in India.

PURPOSE: To report the clinical profile of a series of anterior nodular scleritis in Indian population. METHODS: We conducted a retrospective review of medical records of 140 eyes of 123 consecutive patients with nodular scleritis who presented to a tertiary eye care institute between 2007 and 2018. RESULTS: The mean age at presentation was 46.8 ± 13.1 years and 70.7% of the patients were female. Bilateral involvement was observed in 14% patients. The most common presenting symptom was redness (92.6%) and ocular pain (69.1%). Twenty-seven patients (22%) had some systemic association and rheu…

METHODS: The charts of 89 patients (representing 101 eyes) who had visited our clinic from January 2003 to July 2010 were retrospectively reviewed. They were diagnosed as having episcleritis or scleritis. The medical charts, slit lamp photographs, and laboratory data were reviewed. Age, gender, laterality, previous surgery history, systemic diseases, follow-up duration, and ocular complications were collected. The patients were classified…

Clinical characteristics and visual outcomes in infectious scleritis: a review.

Infection is a very important but rare cause of scleritis, occurring in about 5%-10% of all patients presenting with scleral inflammation. However, due to the similarity of its presentation, infectious scleritis is often initially managed as autoimmune, potentially further worsening its outcome. The overall visual outcome in infectious scleritis is generally worse than its autoimmune counterparts, perhaps because of the delay in diagnosis or because of the aggressive nature of associated microbes. Thus, there is a definite need for insight into the diagnostic approach and treatment options for…

[Diagnosis and treatment of scleritis associated to inflammatory systemic diseases].

Because of potential vision-threatening complications and the association to inflammatory systemic diseases with an increased mortality, scleritis patients need an adequate diagnostic work-up and early and aggressive anti-inflammatory systemic treatment. New therapeutic options (such as biologicals) help to achieve remission in otherwise refractory cases.

Clinical features and visual outcomes of Japanese patients with scleritis.

PURPOSE: To analyse clinical features, systemic associations, treatment and visual outcomes in Japanese patients with scleritis. METHODS: Clinical records of 83 patients with scleritis who presented between 1998 and 2008 to the Ocular Inflammation Service of the Kyorin Eye Center, Tokyo, were reviewed. RESULTS: Of the 83 patients, 57 (69%) had diffuse anterior scleritis, 9 (11%) had nodular anterior scleritis, 8 (10%) had necrotising anterior scleritis and 9 (11%) had posterior scleritis. There was a slight predominance of women (55%) and unilateral disease (53%). Mean age at presentation wa…

Optical coherence tomography findings in a child with posterior scleritis.

PURPOSE: To present the findings of optical coherence tomography (OCT) in a child with posterior scleritis. METHODS: A 13-year-old boy was admitted to our clinic with symptoms of low degree of vision and ocular pain. Ophthalmologic examination, ocular ultrasonography, magnetic resonance imaging, fundus fluorescein angiography (FFA), and OCT investigations were carried out and the results were evaluated. RESULTS: Visual acuity was 10/10 in the right eye, and finger counting at 1 meter in the left eye. Anterior segments and vitreous were normal in both eyes. Fundus examination revealed the ede…

A case of posterior scleritis: differential diagnosis of ocular pain.

Posterior scleritis is a rare cause of ocular pain, due to scleral inflammation, presenting with periocular pain, pain on movement and decreased vision. Although anterior scleritis may be associated with this condition, ocular signs may be absent. We report a case of posterior scleritis, presenting with right-sided ocular and periocular pain, exacerbated by ocular movements, irradiating to the ipsilateral temple and zygoma, not associated with visual disturbances at onset. Diagnosis was made with ultrasonography and confirmed by brain and orbital MRI. Differential diagnosis of facial pain, in …

Anterior scleritis: three case reports and a review of the literature.

BACKGROUND: Scleritis is a rare ocular condition involving vasculitis. It can result in severe ocular morbidity and has a high association with underlying systemic disease and mortality. Management can be challenging and under-diagnosis is a concern. Treatment usually requires systemic medications and co-management with a medical specialist in the appropriate field to manage any underlying systemic etiology. METHODS: Three cases are presented: idiopathic nodular scleritis with complete recovery; bilateral, diffuse anterior scleritis with rheumatoid arthritis; and scleromalacia perforans treat…

Myositis and scleritis associated with Behcet's disease: an atypical presentation.

We report a case of an 18-year-old female, who was diagnosed as having systemic Behcet's disease with chief complaints of vision loss in the right eye and redness and pain in both eyes. In addition to an anterior necrotizing scleritis, she had several attacks of retrobulbar neuritis and lateral rectus myositis. She responded well to i.v. and topical prednisolone and her subsequent attacks were treated with i.v. cyclophosphamide (500 mg on presentation and on Days 7, 10, 12, 15, and 20; the cycle was repeated every 20 days). She benefited on this treatment and her vision improved dramatically. …

Surgically induced necrotizing scleritis in a patient with ankylosing spondylitis.

We present the case of a 75-year-old man with ankylosing spondylitis who developed surgically induced necrotizing scleritis (SINS) more than 3 years after uneventful extracapsular cataract extraction and posterior chamber intraocular lens implantation. The patient presented with a painful eye and increasing vertical diplopia. To our knowledge, neither the association of SINS and ankylosing spondylitis nor vertical diplopia as its presenting complaint has been described.

Combination intravenous ceftazidime and aminoglycosides in the treatment of pseudomonal scleritis.

BACKGROUND: Pseudomonal scleritis is a serious and potentially blinding infection that usually is resistant to medical management. METHODS: Results for three patients with pseudomonal scleritis who were treated with both topical anti-infectives and a combination of intravenous ceftazidime and aminoglycoside are presented in this case series. RESULTS: All three patients had a rapid response to the addition of combination intravenous drug therapy to topical therapy; eradication of the infection and healing of the ocular surface occurred within 8 weeks. Only one patient, in whom cystoid macular…

Clinical features of episcleritis and scleritis in some Japanese patients.

We retrospectively examined 52 patients (60 eyes) with episcleritis and scleritis. Their mean ages were 57 and 47 years, respectively. Slight female predominance was noted. Most patients had unilateral involvement. The ocular symptoms at the initial visit were redness and ocular pain. Some eyes with scleritis were complicated with uveitis, corneal lesions, and elevated intraocular pressure. In most affected eyes, corticosteroid eyedrops alone resolved the disease. We believe that in addition to ophthalmological examinations, standard physical and specific laboratory investigations should be pe…

Scleritis, pericarditis, and aortic insufficiency in a patient with rheumatoid arthritis.

A 55-year-old black man developed anterior nodular scleritis in his left eye in November 1981. He had no symptoms of systemic disease, and initial laboratory tests revealed only a positive rheumatoid factor. Fourteen months later he presented with pericarditis and aortic insufficiency requiring aortic valve replacement. Examination of excised valvular and pericardial tissue showed changes compatible with rheumatoid disease. Shortly after the surgery he developed florid rheumatoid arthritis. The clinical course of this patient illustrates how scleritis can be the initial sign of severe systemic…
