Exclusion of psychiatric disorders in neuropathic pain research: a secondary analysis of 312 randomized controlled trials.

Chronic neuropathic pain frequently coincides with psychiatric disorders. Randomized controlled trials (RCTs), which underpin treatment guidelines, often seek to reduce confounding variables through exclusion criteria. This may affect the representation of populations with psychiatric comorbidities. This secondary analysis of 312 RCTs on neuropathic pain interventions examined how psychiatric-related exclusions are reported. Data were drawn from a published systematic review of neuropathic pain interventions. We identified 312 unique RCTs, and 2 independent reviewers coded psychiatric-related

Exploring equity in audit and feedback trials: Secondary analysis of a systematic review.

BACKGROUND: One potential approach to eliminating or reducing health inequities for health systems is audit and feedback (A&F). A&F involves providing measurements of quality indicators to health professionals to support continuous quality improvement, and to increase clinicians' adherence to clinical practice guidelines. In theory, A&F could help direct efforts toward equity deserving sub-groups (e.g., gender-diverse individuals or those living with low-income) by highlighting factors that may place such sub-groups at higher risk of poor health outcomes. In cases where healthcare professional

Coenrollment of critically ill patients in PROSPECT: A protocol and statistical analysis plan.

INTRODUCTION: The enrollment of a patient into more than one study (i.e., coenrollment) has risks which warrant exploration, particularly with respect to possible effects on trial outcomes. This pre-planned secondary analysis will examine the sensitivity of treatment effects to coenrollment in an international critical care trial (Probiotics: Prevention of Severe Pneumonia and Endotracheal Colonization Trial (PROSPECT). OBJECTIVE(S): The primary objective is to evaluate the sensitivity of the effect of probiotics on the primary endpoint of VAP to patient coenrollment in at least one other stu

Moving beyond gender: a systematic review of diversity data gaps in prospective trauma research.

Background: To better understand heterogeneity in PTSD development and to ensure that knowledge of risk and intervention mechanisms is available for a diverse group of trauma exposed individuals, psychotraumatology needs to consider diversity in sample characteristics. Objective: In a comprehensive overview, we evaluated how nine diversity domains including age, socioeconomic status (SES), race/ethnicity, sexual orientation, care responsibilities, religion/worldview, mental health, physical health, and disability status were considered in prospective PTSD research. In in-depth analyses, we furt

Development of a comprehensive real-world evidence dataset to inform diabetes policy: the diabetes data cell project.

BACKGROUND: Diabetes is a highly prevalent non-communicable disease with a growing global burden, posing significant challenges to healthcare systems. Effective management requires comprehensive monitoring of prevalence, risk factors, and outcomes across diverse diabetes populations. While several European countries have established or are establishing national diabetes registries to guide care and policy, Belgium's diabetes data landscape remains fragmented, lacking a unified national diabetes registry. Existing datasets cover limited populations and care settings, hindering a complete unders

Association between weekday of surgery and length of hospital stay among patients undergoing primary total knee arthroplasty in Singapore: a secondary analysis based on a retrospective cohort study.

BACKGROUND: Total knee arthroplasty (TKA) demand is increasing globally, necessitating the optimization of hospital length of stay (LOS) to increase efficiency. While weekday scheduling has been linked to LOS variations in Western populations, its impact on Asian healthcare systems remains understudied. OBJECTIVE: To investigate the associations between operative weekday length and LOS among Singaporean TKA patients, including nonlinear trends and subgroup differences. METHODS: This retrospective cohort study analyzed 2,394 primary unilateral TKA procedures performed at a Singaporean tertiar

Does physical activity mediate the reduction of depressive symptoms in older adults treated with behavioural activation? A secondary analysis of a cluster randomised trial.

BACKGROUND: Behavioural activation reduces depressive symptoms in older adults but its working mechanisms remain unknown. Physical activity may play a role by decreasing depressive symptoms in older adults. We investigated whether an increase in physical activity explains the effects of behavioural activation on depressive symptoms in older adults. METHODS: Participants were 44 adults aged 65-88 years, who participated in a cluster randomised trial evaluating the effectiveness of an eight-week behavioural activation treatment (n = 25) compared with eight weeks of treatment as usual (TAU; n =

Latent profiles of male peers returning HIV self-test results after secondary distribution by men who have sex with men in Zhuhai, China.

BACKGROUND: Secondary distribution of HIV self-testing (HIVST) can extend testing through social and sexual networks of men who have sex with men, but the peer subgroups reached by this approach remain insufficiently understood. We aimed to identify latent profiles of peers who returned HIVST results, and assess index-peer factors associated with peer class membership. METHODS: We conducted a secondary analysis of pooled HIVST secondary distribution studies and program phases among men who have sex with men in Zhuhai, China, from October 2019 to December 2023. Peers received HIVST kits from i

Mechanisms of sustained mindfulness practice in stroke survivors: A critical realist secondary analysis of the HEADS: UP intervention.

BACKGROUND AND PURPOSE: Post-stroke anxiety and depression symptoms are common after stroke and can persist long-term. Despite this, there is a lack of long-term psychological support for post-stroke anxiety and depression. Mindfulness-based interventions are emerging as feasible, acceptable and potentially effective interventions for post-stroke anxiety and depression. Sustained use of mindfulness practice can help stroke survivors to self-manage their anxiety and depression in the long-term. However, stroke survivors' experiences of continued practice following a mindfulness-based interventi

The Volume of Fluid Corresponding to Each Centimeter of Amniotic Fluid Index Changes throughout Pregnancy: A Prospective Cohort Observational Study.

INTRODUCTION: The normal values of the amniotic fluid index (AFI) throughout pregnancy have been established as 5-24 cm. However, a similar AFI may correspond to a different AF volume at different gestational ages. The aim of this study was to calculate the amount of fluid corresponding to each cm of AFI when a known volume of fluid is infused into the uterine cavity. METHODS: This was a secondary analysis of a prospective cohort observational study. Serial amnioinfusions (AIs) were performed between 10 and 34 weeks of gestation in 19 fetuses with anhydramnios due to severe renal damage. The

Multilevel modelling of structured intersectional inequalities in child anaemia in Peru: a MAIHDA approach.

BACKGROUND: Socioeconomic exposures related to anaemia in Peruvian children have been modelled assuming additive or log-additive relationships, yet such approaches overlook the fact that illness emerges from the complex interplay of multiple, intersecting determinants. Using data from the 2017-2023 Peruvian Demographic and Health Survey, we cross-classified age, wealth index, maternal education, ethnicity and region of residence to estimate the prevalence of anaemia across their intersections and decompose the total intersectional effects into additive and interaction components on the log-odd

Impact of prior intravenous thrombolysis on first-line thrombectomy strategy. A secondary analysis of the VECTOR trial.

INTRODUCTION: Whether prior intravenous thrombolysis (IVT) modifies the relative efficacy of first-line thrombectomy strategies remains uncertain, and no randomised trial has specifically addressed this question. Leveraging the randomised design of the VECTOR trial, a multicentre trial comparing first-line contact aspiration (CA) vs CA plus stent retriever (SR + CA) in patients with anterior LVOs and a positive susceptibility vessel sign, we assessed whether IVT status modified the association between thrombectomy strategy and reperfusion outcomes. PATIENTS AND METHODS: We performed a seconda
