[The objective assessment of the determination of the period of unstable hemodynamics in traumatic shock].

It was established that the moment when patients revive from traumatic shock can be objectified from analysis of time series trends. The author shows the expediency of differentiating the concept "stability of hemodynamics" into stably deteriorating and stably nondeteriorating dynamics of the values of systemic hemodynamics. The possibility of prognosticating the quantitative characteristics of circulation parameters for a certain preset period of time was revealed.

[The effect of dalargin on systemic hemodynamics and microcirculation in experimental traumatic shock].

Experiments were conducted on rats and hamsters to study the effect of the synthetic analogue of leu-enkephalin dalargin injected in the early phase of traumatic shock. In experiments on hamsters, short-term increase in arterial pressure (AP) and improvement of microcirculation occurred in the first minutes after the injection. The AP did not increase in rats. This was followed by reduction of AP, deceleration of the blood flow in the microvessels, and increase in the diameter of arterial vessels. These processes developed more rapidly than in the controls. Survival decreased significantly amo

Differences in coagulation and fibrinolysis after traumatic and septic shock in man.

In a prospective investigation of 19 patients with traumatic (n = 11) and septic (n = 8) shock admitted to the Intensive Care Unit of the University Hospital of Uppsala, differences in coagulation and fibrinolysis between the two conditions were evaluated. It was found that plasma coagulation variables such as thrombotest, normotest and antithrombin III were significantly lower in the septic patients, thus indicating a more intense and/or altered intravascular coagulation. Significantly higher levels of the von Willerbrand factor were found in septic patients, indicating a greater release from

Use of the novel cardiotonic and vasodilator agent pimobendan in traumatic shock.

The effect of 4,5-dihydro-6-[2-(4-methoxyphenyl)-1H-benzimidazol-5-yl]-5-methyl- 3(2H)- pyridazinone (pimobendan, UD-CG 115 BS), a novel positive inotropic and vasodilator agent, was studied in a severe model of murine traumatic shock. Noble-Collip drum trauma produced a shock state characterized by a significantly reduced mean arterial blood pressure (MABP), a 5-fold increase in plasma cathepsin D and myocardial depressant factor (MDF) activities, and a survival time of 80 +/- 12 min. Administration of pimobendan (100 micrograms/kg, i.v. bolus) significantly prolonged the survival time to 175

[Disorders of electrolyte metabolism in the early period after traumatic shock].

Inconsiderable hyponatremia and pronounced hypopotassemia were found to accompany the shock. In a mild course of the traumatic disease the concentration of natrium in blood plasma was restored within 3--6 days, potassium within 7--10 days after trauma, and in a grave course hypopotassemia retained for about 14 days. More continuous use of the infusion therapy including potassium and natrium was found to be expedient.

Protective effects of a novel thromboxane analog in lethal traumatic shock.

Pinane thromboxane A2 (PTA2) an analog of thromboxane A2 that inhibits the formation of thromboxanes as well as antagonizes their biological actions, at an infusion rate of 1.0 mumole . kg-1 . h-1, prolonged survival in traumatic shock in rats. PTA2 also prevented the accumulation of thromboxane B2, the lysosomal protease, cathepsin D, and the cardiotoxic peptide MDF in the circulating blood.

[Systemic circulatory reaction in the initial period of Cannon's traumatic shock].

In experiments on dogs it was shown that on the basis of the dynamics of circulatory parameters it is possible to divide the erectile phase of traumatic shock into two periods. The first period, similar to "defence reactions" in dangerous situations and under the action of potent non-specific stimuli, is characterized by an increased blood inflow to the myocardium, rise in the cardiac output and reduction of the tonus in peripheral vessels. The second period is characterized by the beginning of decompensation of the cardiovascular system. It is completed by the development of the torpid phase
