BACKGROUND: Proximal humerus fractures (PHF) are common in adults above 50 years, often following low-energy trauma and underlying osteoporosis. Randomized trials in patients 60 years and older have found no clinically important difference in 1- and 2-year outcomes between surgical and non-surgical treatments. There is limited evidence for the 50-65 age group, who may have different functional demands and even a different overall health status. There is limited knowledge to guide treatment decisions for this age group.
METHOD: A multicenter, parallel-group, randomized, superiority trial compa

PURPOSE: To assess patient outcomes following reverse shoulder arthroplasty in patients with complex proximal humerus fracture and the clinical implications of greater tuberosity malunions.
METHODS: This prospective study included 56 patients who underwent RSA (DELTA XTEND™, DePuy Synthes, Warsaw, IN, USA) to treat proximal humerus fractures. We used a standardized suture technique to reattach the tuberosities. Demographic, comorbidity, and radiological parameters were collected. Assessments at 2-year follow-up (n = 49) are given as follows: range of motion (ROM), pain level, Constant Murley

BACKGROUND: Identifying risk factors for acromial and scapular fractures improves our understanding about which variables are relevant to these fracture complications; however, these data are difficult to integrate into clinical practice because the majority of reverse total shoulder arthroplasty (rTSA) patients have ≥1 risk factor. The goal of this study was to better facilitate preoperative identification of patients at risk of acromial and scapular fractures and quantify the impact of accumulating risk factors on the incidence of fracture.
METHODS: We retrospectively analyzed 9079 rTSA pat

BACKGROUND: Three-part and four-part fracture-dislocations of the proximal humerus are characterized by severe soft tissue disruptions that can compromise the viability of the humeral head. As a result, nonunion and avascular necrosis are more common in these injuries. In such injuries, surgical treatment (internal fixation or arthroplasty) is performed in most patients who are determined to be fit for surgery to potentially restore shoulder function. Although the decision to preserve or replace the humeral head is simple in young patients or those > 65 years, in most other patients, the decis

BACKGROUND: Recent studies have reported the clinical effectiveness of tuberosity healing after reverse total shoulder arthroplasty in patients with proximal humeral fractures with respect to joint stability, long-term survival, and postoperative range of motion. However, it is challenging to achieve robust fixation of the fragile bone fragments in elderly patients. This study aimed to report on the radiographic and clinical outcomes of patients who underwent reverse total shoulder arthroplasty for acute proximal humeral fractures using a Turned stem Tension Band technique-a simple suture conf

Inferior dislocation of the shoulder, also called luxatio erecta, is a rare form of the otherwise common shoulder dislocation. It appears in less than 0.5% of all shoulder dislocations. An awareness of associated potential axillary artery injury, brachial plexus complications, and rotator cuff tears is important in this rare entity and should be excluded with a high index of suspicion. In our case report, we have an 83-year-old female who inferiorly dislocated her dominant shoulder with brachial plexus injury and musculotendinous injury, which was caused by an accidental fall. The dislocation

BACKGROUND: Proximal humerus fractures (PHFs) are among the most common fractures in elderly patients, yet there is insufficient evidence from randomized controlled trials (RCTs) to determine the most appropriate interventions for their management. This study presents the long-term results of the first prospective RCT comparing surgical treatment with reverse shoulder arthroplasty (RSA) versus nonoperative treatment in displaced PHFs in elderly patients.

Periprosthetic humeral fracture is a serious complication after reverse shoulder arthroplasty (RSA). Wright and Cofield classification type B fractures occur around the tip of the humeral stem. In these fractures, the intramedullary canal is occupied by the stem and cement, which reduces endosteal blood supply and makes fracture healing more difficult. As a result, surgical management, such as open reduction and internal fixation or revision arthroplasty, is commonly recommended when displacement is present. However, these procedures are highly invasive and may pose substantial perioperative r

Reverse shoulder arthroplasty (RSA) is commonly performed for complex proximal humeral fractures in older adults when fixation or anatomic reconstruction is unlikely to provide reliable outcomes. Recurrent instability following RSA remains a devastating complication, particularly in medically frail patients with poor bone quality and compromised soft tissue balance. Salvage strategies after repeated failed revisions are limited and often associated with poor functional outcomes. BACKGROUND: Fracture sequelae of the proximal humerus were classified by Boileau into four types. Since there are pathomorphological differences and specific characteristics within the four types, we have developed a subclassification. For elderly patients, shoulder arthroplasty is mostly recommended. Based on the available literature and clinical trial results, a subclassification could be created that suggests a specific therapy for each subgroup. The aim of this study was to evaluate the endoprosthetic therapy according to the proposed subclassification and to provide an overview of the cli

BACKGROUND: Reverse total shoulder arthroplasty (RTSA) is an increasingly popular salvage treatment option for proximal humeral fracture (PHF) sequelae. This meta-analysis aimed to conduct a pooled analysis of functional outcomes of RTSA in PHF sequelae, with subgroup analysis comparing between intracapsular (Class 1) and extracapsular (Class 2) PHF sequelae.
METHODS: A multi-database search (PubMed, OVID, EMBASE) was performed according to PRISMA guidelines on 27th July 2020. Data from all published literature meeting inclusion criteria were extracted and analysed.
FINDINGS: Eleven studies

BACKGROUND: Glenoid bone mineral density (BMD) has important implications for the fixation of glenoid components and may vary based on indications for reverse total shoulder arthroplasty (rTSA).17 Careful consideration of pre-operative bone quality can enable surgeons to improve pre-operative and intraoperative decision-making for glenoid component fixation and reduce associated complications, such as aseptic loosening. There remains a paucity of literature exploring such implications in rTSA. We hypothesize that BMD varies by indication diagnosis for rTSA.
METHODS: A retrospective chart revi

HYPOTHESIS: Common surgical treatment options for proximal humeral fractures in elderly patients include locked plate fixation (LPF) and reverse total shoulder arthroplasty (RTSA). It was hypothesized that secondary RTSA after LPF would be associated with higher complication rates and costs compared with primary RTSA.
METHODS: We analyzed the health insurance data of patients aged ≥65 years who received RTSA for the treatment of a proximal humeral fracture between January 2013 and September 2019 with a pre-study phase of 5 years. Multivariable Cox, logistic, and linear regression models were

BACKGROUND: Periprosthetic fracture following total shoulder arthroplasty (TSA) is a devastating complication. Metabolic bone disease has been shown to increase the incidence of fracture. However, the state of metabolic bone care in this population is poorly studied. The purpose of this study is to elucidate the state of metabolic bone care in patients undergoing TSA. We also aim to study the association between computed tomography (CT)‑derived Hounsfield unit (HU) measurements and Fracture Risk Assessment Tool (FRAX) scores in this population.

BACKGROUND: Iatrogenic fracture during closed reduction of anterior shoulder dislocation with greater tuberosity fracture is a serious complication. This study aimed to quantify its biomechanical risk and evaluate prophylactic Kirschner wire (K-wire) fixation.
METHODS: We retrospectively analyzed 126 patients (age ≥ 50 years). A patient-specific finite element model of the proximal humerus was created from quantitative computed tomography (QCT) data. Parametric models with different greater tuberosity defect sizes (0-40%) were loaded under pure axial traction (300N) or combined (axial 300 N +

BACKGROUND: Proximal humeral fractures are traditionally treated with open reduction and internal fixation (ORIF), but reverse total shoulder arthroplasty (RTSA) has emerged as an increasingly popular treatment option. Although ORIF with angular locking plates is a common treatment for proximal humerus fractures, prior reports suggest high failure and complication rates. Although RTSA has become an increasingly popular option for complex proximal humeral head fractures given its low complication rates, there are concerns it may lead to limited postoperative ROM. Thus, the optimal treatment for
